Abstract The importance of a well-fitted, comfortable brassiere to overall quality of life after breast reconstruction has not been evaluated. Our aim was to determine the impact of a semi-customized brassiere on patients’ health-related quality of life after breast reconstruction. The subjects were prospective patients with mastectomy who were to undergo immediate or delayed breast reconstruction at our hospital. After surgery, a professional bra fitter sized each patient for a semi-customized brassiere and provided follow-up consultations. A self-reported questionnaire on breast aesthetics, postoperative pain, and satisfaction was used to assess the primary outcomes. Data were prospectively collected at baseline (before surgery) and at 1, 3, 6, and 12 months after surgery and analyzed. Forty-six patients (50 breasts) were included in the analysis. Consistent wearing of the brassiere reduced pain (p<0.05), with good overall satisfaction (p<0.001). Aesthetic scores on breast shape and size were higher with than without the custom brassiere at 3 months (p=0.02) and 6 months (p=0.03) after surgery. Wearing the brassiere reduced anxiety at all time points of measurement. A well-fitting brassiere ensured safety and provided a high degree of satisfaction without anxiety for patients after breast reconstruction.

Title Alternative Cases of patient discharge from the Kyushu Leprosarium in the Early Showa period
JaLCDOI 10.18926/64200
FullText URL jdc_002_032.pdf
Author MATSUOKA, Hiroyuki|
Abstract Although it has been said that modern Japanese santoria for Hansen's disease quarantined patients for their entire lives, there were a small number of cases of "arrested discharges" (a "provisional discharge" when progress of the disease was halted) in the 1920s. To examine the reasons why this practice did not spread in Japan, this paper considers the background that led to the admission of patients who were later discharged from the Kyushu Sanatorium (九州療養所) to analyze two types of discharges: "Arrested discharges" and "discharge by relatives." To do so, this paper draws on the "Patient Identification Records"(身分帳) created by the Fifth Ward United Kyushu Sanatorium (now National Sanatorium Kikuchi Keifuen) to manage patients' admission and discharge from the sanatorium. This analysis clarified specific procedures of "arrested discharges" from the facility, which had previously been completely unknown, and it further revealed that regardless if the patients were discharged as an arrested case or by relatives, emphasis was placed upon their prospects for life after discharge, such as the availability of people to support their care and a minimum standard of living

Title Alternative The beginning date of wet rice cultivation at the of Okayama University site: Tree ring oxygen isotope dating and radiocarbon 14 age of weirs from the mid-I stage of the Yayoi period
JaLCDOI 10.18926/64199
FullText URL jdc_002_018.pdf
Author FUJIO, Shinichiro| SAKAMOTO, Minoru| SANO, Masaki|
Abstract This article clarifies the date when paddy field rice cultivation began at the archaeological site located within the Okayama University campus. The analysis used tree ring oxygen isotope dating and radiocarbon dating. When the oxygen isotope ratios of wood used in paddy weirs detected in the 23rd excavation season were examined, peaks could be seen in two places: in the 10th century BC and the 6th century BC. Since the weir was accompanied by mid-I stage Yayoi pottery, which was dated to the 6th century BC using radiocarbon dating, tree ring dating determined that the weir was made from wood cut down soon after 540 BC. Therefore, it was reaffirmed that one of the points of the mid-I stage can be dated to the middle of the 6th century BC.
Next is the age of the early-I stage, when paddy field rice cultivation began on the Okayama Plain. In the Tsuruba area of the Nishikawazu archaeological site in Shimane Prefecture, where paddy field rice cultivation is thought to have begun at the same time as on the Okayama Plain, tree ring oxygen isotope dating of wood accompanied by early-I stage pottery has been reported. The date of 649 BC date means that one of the earliest stages of the early-I period dates to the middle of the 7th century BC. Therefore, we reaffirmed the view that paddy rice cultivation in the Chugoku region, such as Okayama and Shimane, began in the 7th century BC.
It also became clear that the beginning of paddy field rice cultivation in the Chugoku region occurred at a time when the climate that had been the base of cold in the 10th century BC gradually warmed and the relatively humid climate began to turn to arid.

Title Alternative “Heritagization” and Boundary Politics of Traditional Textiles: A case of Indonesia
JaLCDOI 10.18926/64198
FullText URL jdc_002_001.pdf
Author NAKATANI, Ayami|
Abstract This paper focuses on the process of "heritagization" within nation-states, which runs parallel to the global heritage regime initiated by UNESCO. Involved in this process of heritagization are multiple actors with competing political and economic interests, and the formation of collective identities both internally and externally. Indonesia, the subject of this paper, is a country where a central government-led cultural policy has been pushed forward in the process of national integration. After the end of the "New Order" regime of the second President Suharto, which embodied this vision immediately after independence and permeated the nation to its very core, local cultural practices became a new tool for asserting the uniqueness of regions separated by certain boundaries, as a competition among provinces and regions accelerated with the decentralization of power. At the same time, however, such cultural practices stimulated a process in which traditional region-specific crafts became an easily consumable resource.
The act of listing cultures within the framework of a heritage regime means that some cultural expression is lifted out of their original context and made visible alongside other cultural expressions. As a result, local cultural practices are elevated to a higher context and made visible, thus becoming objects of consumption beyond the local framework. The boundaries that enclose the cultural elements being listed are elastic and contractible, which is why consumption based on these cultural events is possible on a wider scale.
