Title Alternative Information and communication technology equipment and services for remote monitoring and health management: Survey of services and their classification for use in home care nursing

Abstract This study aims to improve the quality of home nursing care for the elderly. The report presents web-based information about devices and services for remote monitoring and health management using information and communication technology and about how they can improve home care nursing station efficiency in Japan. The search terms "elderly people," "watch over," and "service," plus "sensor" were used to narrow the search. After repeating the search using a chained search for materials and links extracted from those search results, services for home care stations particularly addressing the remote monitoring of elderly people were selected and extracted. Early detection of abnormalities for support by home health nurse responsibilities for observing the general condition of the patient was chosen as the main evaluation index of usefulness. The contractor and service cost were secondary evaluation axes. Results show 9 information communication devices and services for real-time monitoring of the elderly persons’ vital signs in real time for 24 hr, 2 of which are highly useful for early detection of abnormalities and subsequent response. Results suggest that other services might be more useful, depending on how they are used, and that general-purpose services with minimum functions and low cost of introduction and maintenance can provide more opportunities for utilization. Providing these services on a BtoBtoC model and responding to patients’ medical needs can supplement home care nursing station work, increase human resource and time efficiencies, and give patients "the feeling that a nurse is always at their bedside,” even when far away.

Title Alternative Consideration about recognition of factors causing lack of confirmation in the nursing scene

Abstract In the work process of nurses with high risk, it is very urgent to take countermeasures against the “lack of confirmation” of the main factors. Therefore, the purpose of this study is to clarify the details of the factors leading to the lack of confirmation, grasp the number of near misses, and to explore what kind of factors the individual characteristics of the nurse who acts as a stopper. We recruited 500 nurses on the Web and asked about the frequency of near misses due to lack of confirmation, the awareness of the causes of lack of confirmation, self-evaluation of stoppers, and the reasons why near misses did not lead to accidents. As a result, three factors were extracted regarding the recognition of the factors of lack of confirmation: "lack of consciousness", "inability to concentrate", and "lack of knowledge / experience". Three factors were extracted for self-evaluation of stopper characteristics: "awareness," "verification," and "cautiousness". In order to reduce the lack of confirmation, it is necessary to secure and allocate appropriate personnel and create an environment where treatment can be concentrated. Nurses should also raise knowledge and the awareness of the importance of confirmation. In addition, it is necessary to improve the clinical judgment of new nurses in order to prevent near misses. And it is thought that new nurses will become more confident by accumulating experience and affirming their abilities from their superiors, seniors and colleagues.

Title Alternative A survey of nurses’ and physicians’ safety awareness in CT/MRI examinations

Abstract To perform computed tomography (CT)/magnetic resonance imaging (MRI) scans safely and reliably, adherence to a pre-scan checklist is crucial. However, physicians and nurses may differ in their recognition of the usefulness of this checklist. We conducted the present study to investigate possible differences between physicians’ and nurses’ awareness of CT/MRI scan safety in terms of their adherence to the pre-scan checklist. We prepared an independently developed self-administered 23-item questionnaire about attributes of subjects, CT/ MRI scan safety recognition and current situation for CT/MRI examinations. The survey was distributed to 468 participants who combined nurses and physicians at Okayama University Hospitalin Okayama, Japan. We analysis the responses of the 224 participants (117 nurses and 107 physicians)who returned the survey with on missing data (a 65.1% completion rate). The overall safety recognition scores were significantly higher for the nurses than for the physicians (p<0.001). In addition, the physicians did not sufficiently know or implement the guidelines in the CT/MRI scan safety manual used at our hospital. Nurses and physicians demonstrated marked differences in their awareness and knowledge of safety regarding CT/MRI scans. Measures for improving safety recognition should thus be designed for individual healthcare occupations.

Title Alternative The development of a scale to measure stress recognition during the treatment of diabetes patients

Abstract The purpose of this study was to establish a measurement scale for "stress recognition in receiving treatment" in patients with diabetes. A self-completed questionnaire was distributed to 149 type-2 diabetes outpatients in March-May 2015 after authorization from Okayama Prefectural University and the ethics committee of the hospital. The "stress recognition in receiving treatment" scale was designed as a second-order factor model consisting of 14 items and the following four factors : the respondent's sense of (1) the burden of being sick, (2) the burden on interpersonal relationships, (3) the burden of treatment, and (4) the burden of medical expenses. Stress recognition in treatment means recognition of being stressed in the burdens related to the illness, interpersonal relationships, treatment and medical expenses. The suitability of the questionnaire data was then evaluated with a structural equation model. The suitability of the factor model to the data satisfied the statistically acceptable standards as Comparative Fit Index (CFI) =0.931, Root Mean Square Error of Approximation (RMSEA) =0.096, Tucker-Lewis Index (TLI) =0.946. As the construct validity was not examined by the scale created in this study or by existing scale, it was verified by using the degrees of mental healthiness and HbA1c that were proved to be associated with the sense of burden. In addition, the construct validity of the questionnaire was supported by a significant correlation between the Japanese version of the WHO-Five Well-being Index (S-WHO-5-J) and the patients' HbA1c levels. The use of this measure is expected to contribute to the early detection of a decline in a diabetic patient's activities of daily living and to the early confirmation of patients' support status.
