抄録 We evaluated the efficacy and toxicity of cisplatin/docetaxel (CDDP/TXT) chemotherapy and identified prognostic factors in Japanese patients with cancer of unknown primary site (CUP). Twenty-eight consecutive patients seen at a single institute were reviewed retrospectively. Sixteen patients were treated with TXT 80mg/m2, followed by CDDP 75mg/m2. The overall response rate to CDDP/TXT treatment was 62.5%, with a median survival time (MST) of 22.7 months. Common adverse reactions were myelosuppression and hyponatremia. The MST of all 28 patients with CUP was 8.3 months, and the 1-year overall survival rate was 45.6%. Univariate analysis identified 5 prognostic factors:performance status, liver involvement, bone involvement, pleural involvement, and lymph node involvement. In conclusion, CDDP/TXT chemotherapy is effective with tolerable toxicity in patients with CUP. Japanese patients with CUP might be chemosensitive and may survive longer.

抄録 Anemia of moderate severity is commonly present in patients suffering from rheumatoid arthritis, but one to which little attention has been paid and despite its frequency the exact mechanism of its production is uncertain. Anemia in rheumatoid arthritis is refractory to anti-anemic therapy. The presence of anemia may hinder adequate physical or orthopedic treatment and may produce a degree of lassitude that considerably aggravates the joint disability. This anemia, therefore, seemed worthy of study and investigations of three aspects are reported here: 1. Changes in peripheral blood, bone marrow, liver function and gastric acidity. 2. Alternations in iron metabolism. 3. Balneotherapy and anemia in rheumatoid arthritis. I. Peripheral Blood Picture, Liver Function, Serum Protein Fraction and Gastric Acidity. 1. The peripheral blood and bone marrow. The number of erythrocytes is 558~216×10(4)/c. mm. (average:402×10(4)/c. mm.), hemoglobin-content:110~40% (average: 78.6%), and the color index is about 1.0. The anemia of rheumatoid arthritis treated is mostly normochromic. The white blood cell count is between 11200 and 3720/c. mm. (average: 7504/c. mm.), and the number of leucocytes in 80% of the cases ranges within the normal limits. Eosinophilia is observed 27% of the cases. Films of the sternal marrow showed no major abnormality in the eight cases examined. Several minor points, however, deserve mention. The nucleated cell count was diminished in seven cases examined, and the decrease of normoblasts in bone marrow would explain the reduced red cell count in peripheral blood. The myeloid series showed a relative increase. 2. Liver function tests and gastric acidity. To illustrate the possible appearance of liver function disturbances in connection with rheumatoid arthritis, the author has made different liver function tests on patients suffering from this disease. From the results it appears that in protein metabolism tests more than 83% of the cases gave abnormal albumin/globulin ratios and Takata's and erthrocyte sedimentation reactions. The detoxication capacity determined by hippuric acid synthesis tests had decreased in 25% of the cases examined. The dye excreting capacity, determined by the bromsulfalein tolerance test, was found to be reduced in 64% of the cases examined. The urin-urobilinogen test showed positive in 28% of the cases. In all cases the insulin sensitivity test gave pathological readings. Of 40 cases examined, anacidity of gastric juice was observed in 17 cases, subacidity in other 10 cases and normacidity, hyperacidity in remammg 13 cases. Thus, a decrease in the gastric acidity was found to occur in 67.5% of the cases. 3. Serum protein fraction. The total serum protein of 31 cases with rheumatoid arthritis appeared to be on the same level as in normal material. Serum protein fraction was determined in 19 cases by paperelectrophoresis. In rheumatoid patients, albumin fraction of serum protein decreased and γ-globulin fraction increased in all cases examined, compared with the data of normal subject as a control. Alubumin/globulin ratio of rheumatoid arthritis was below the 5% rejection limit of normal value.

抄録 The term atypical lipomatous tumor (ALT) is synonymous with well-differentiated liposarcoma (WDL). This tumor occurs very rarely in the tongue. Thus, it is difficult to predict its prognosis. Although recurrence of ALT/WDL is thought to be unlikely after complete excision, long-term follow-up is necessary when considering the pathologic conditions of this tumor at other sites. Here, we report a case of an ALT of the tongue, with a review of the literature. A 68-year-old man was referred to our hospital because of a tumor on the left side of his tongue. Upon palpation, the tumor was 12mm in diameter, circumscribed, elastic and hard, well demarcated, movable, and painless. We diagnosed the lesion as a lipoma and extirpated the tumor under local anesthesia. Because the specimen was histopathologically diagnosed as an ALT, as a precaution, we excised an additional 5mm from the area surrounding the original tumor under general anesthesia. Three years after the operation, the tongue demonstrated good healing without paresthesia or dysfunction, and to date there has been no evidence of recurrence.

タイトル(別表記) The involvement of ear and throat in rheumatoid arthritis II. On the Rheumatoid Arthritis of Cricoaryenoid Joint

抄録 Until 1955 a few reports had been made concerning this problem. On December 1955. Montgomery, Perone and Schall reported four cases of rheumatoid arthritis of cricoarytenoid joint. It seemed to have interested other authors so that Saunders, Pearson, BakerBywaters, Copeman and Polisar reported their clinical findings. In 1957 Pearson presented a histological section of cricoarytenoid joint, showing villous synovial proliferation. In 1959 Montgomery reported another series of additional cases and microscopical findings of the cricoarytenoid joint of two patients with rheumatoid arthritis. This study did not show a complete fibrous ankylosis but severe arthritic changes were seen. In the same year Copeman et al. reported an autopsy case of rheumatoid arthritis with ankylosis of cricoarytenoid joint. In early 1960's Grossman et al. examined 55 patients with rheumatoid arthritis and found 18 patients with some suggestive evidence of cricoarytenoid arthritis. Lofgren and Montgomery examined 100 patients of rheumatoid arthritis, finding 26% of them had involvement of cricoarytenoid joint. They stated that this disease seemed to be more common and more severe in female. Bienenstock stated 17 cases of 64 randomly selected patients with rheumatoid arthritis had one or more symptoms considered to be characteristic of cricoarytenoid arthritis. He claimed that arthritis of cricoarytenoid joint occured much more frequently in patients with rheumatoid arthritis than had been suspected. At postmortem examination Grossman saw histological changes of cricoarytenoid joint characteristics of rheumatoid arthritis in 5 of 11 autopsied patients with rheumatoid arthritis and Bienenstock found the same in 7 of 8 patients. In 1963 for the third time Montgomery discussed of cricoarytenoid arthritis, based on his own experiences and many available literatures. Pathologic changes range between the mere synovial thickning and the complete disruption. Montgomery believed as others did, that in ankylosing chronic stage inspiratory bowing of both vocal cords and absolute fixation of arytenoid cartilage were diagnostic signs of rheumatoid arthritis of cricoarytenoid joint. In 1958 Darke, Wolman and Young reported five cases of laryngeal stridor, where in four cases tracheotomy became necessary. Two of them were brought to autopsy and histological examination. They found evidence of the nerve degeneration and mobile cricoarytenoid joint. The cause of nerve degeneration in the second case was an ischaemic neuropathy from rheumatoid arteritis of vasa nervorum. These findings were different from those of Montgomery and others. In 1965 Wolman, Darke and Young had an opportunity of six more autopsy examples. They confirmed from their own pathologic findings that laryngeal stridor occuring in rheumatoid arthritis was due mainly to arteritis of the vasa nervorum causing ischaemic neuropathy and that polymyositis and joint disease might augment this effect. Woldorf and Webb et aI. introduced both of these opinions. Anyhow, in U. S. A. and in England 25%-28% of rheumatoid arthritis seems to have rheumatoid arthritis of cricoarytenoid joint. On the other hand in our country there has been seen no report on rheumatoid arthritis of cricoarytenoid joint. In order, therefore, to investigate this problem author examined in medical clinic of Misasa branch hospital of Okayama university school of medicine 76 patients of classical and definite rheumatoid arthritis. Their complaints were as follows. 1) dryfeeling of throat 17 (22.4%) 2) slight hoarseness 11 (14.5%) 3) choking by long conversation 1 (1.32%) 4) slight dysphagia 2 (2.6%) The larynx was mirrored indirectly by Türk's, Killian's and Avellis's position. Any redness, swelling of mucous membrane, abnormality of vocal cords and arytenoid cartilage were not found.

抄録 Phagophobia is a disorder characterized by a conditioned excessive fear of eating and is initiated by an event such as vomiting or choking. During childhood, vomiting often occurs as a result of infection or overeating, and painful experiences bring about maladaptive eating behavior like food refusal. There have been few reports of phagophobia, and patients have sometimes been misdiagnosed with anorexia nervosa (AN). The objective of this study was to elucidate the psycho-pathology and current treatment of patients with phagophobia by analyzing case studies. We describe 6 cases with phagophobia. Patients with strong obsessions were refractory to treatment, indicating that evaluation of premorbid personality is crucial to the prognosis. It is important to classify this disorder according to psycho-pathology into "post-traumatic type" and "gain-from-illness type" to make a treatment plan. A solution focused approach is also effective for patients and their family. Paying close attention to these conditions and to the diagnostic concept referred as "hagophobia" is useful in achieving these aims.

抄録 A 64-year-old woman was admitted to our hospital with lower abdominal pain. Routine laboratory values were unremarkable except for the white blood cell count (15,000/micro litter) and the C-reactive protein (CRP) value (22.5 mg/dl). A Computed tomography (CT) scan revealed air collection in the middle of the anterior pararenal space. One day later, CT revealed air collection in the anterior pararenal space spread to the right side and abscess in the sigmoid mesentery. Because an intramesocolic perforation of the sigmoid colon was suspected, an emergency operation was performed. Abscess formation was recognized in the sigmoid mesentery, and sigmoidectomy including the contaminated mesentery and Hartmann.s procedure were performed. The perforation was 3 cm in diameter, and some diverticula were present in the vicinity of the perforated site. The specimen microscopically revealed perforation at the edge of the diverticulum in association with sudden disruption of the proper muscle layer. Based on pathological findings, intramesocolic diverticular perforation of the sigmoid colon was diagnosed. The present case is a very rare condition. However, it was possible to make a diagnosis preoperatively by detecting air collection in the anterior pararenal space on CT scan. If a sigmoid perforation occurs between the leaves of the mesocolon, air extends into the root of the sigmoid mesocolon and within the anterior pararenal space.

抄録 Three cases of colon cancer accompanied by unusual abscess formation are reported. Case I : A 77-year-old man was diagnosed with a paracolic abscess formation behind the cecum and a swollen appendix by computed tomography (CT) scan. Case II : An 85-year-old woman was diagnosed with an abscess formation of the right iliopsoas muscle, a swollen appendix, and a thickened right colon wall by CT scan. After antibiotic therapy failed, both patients underwent ileocecal resection urgently under suspicion of appendicitis, but cecal cancer around the entrance to the appendix caused secondary appendicitis in both cases. Case III : A 50-year-old woman was diagnosed with sigmoid colon cancer with an abscess formation in the pelvic cavity concomitant with ovarian tumor. A Hartmann procedure was performed, and a pathological examination revealed that a subserosal abscess behind the sigmoid colon cancer perforated the rectum with abscess formation. All cases were definitively diagnosed intraoperatively. The cancer recurred in cases I and III. We emphasize that precise surgical evaluation has an important role in the diagnosis of these complicated diseases. In addition, surgery affords the patient the best chance of recovery, and in these advanced cases radical treatment is recommended as early as possible.

抄録

We evaluated the need for transurethral biopsy at first follow-up after intravesical bacillus Calmette-Guerin (BCG) therapy for superficial bladder cancer. The records of 84 patients with superficial bladder cancer who received a 6- or 8-week course of BCG were reviewed. Pathological results before BCG, cystoscopic findings, urinary cytology, and biopsy results for evaluation of BCG therapy were reviewed. All 19 patients with positive urinary cytology had evidence of positive bladder biopsy results. Fifty-three of 54 patients (98.1%) with no visible recurrent tumor and negative urinary cytology demonstrated negative pathological results on bladder biopsy. When not found in conjunction with positive urinary cytology, erythematous mucosa on cystoscopy was not an indicator of tumor recurrence or residual cancer. In conclusion, routine transurethral biopsy of the bladder for evaluating the response to BCG intravesical therapy is not necessary in patients who have no visible tumor on cystoscopy and negative urinary cytology./

抄録 We analyzed the prognostic factors influencing survival after surgeries for upper urinary tract urothelial carcinoma (UUT-UC) with longer follow-up periods than in previous studies. Between January 2000 and December 2004, 386 patients underwent nephroureterectomy for UUT-UC. The data for the 221 patients with UUT-UC were retrospectively reviewed. Nine variables were evaluated for association with the survival outcomes of cause-specific survival. The prognostic significance was tested univariately with the log-rank test. The simultaneous effects of multiple prognostic factors were estimated by multiple regression analysis using the Cox proportional hazards model. The median follow-up was 38.4 months. The 5-year over all survival was 62.3%. Significant prognostic factors for disease-specific survival rate on univariate analysis were pathological stage (p0.0001), tumor grade (p0.0324), and venous invasion (p0.0001). Multivariate analysis revealed that only venous invasion was significant for disease-specific survival rate (p0.0205). Venous invasion was the only independent prognostic factor in pathologically localized UUT-UC.
