| 抄録 | Early mobilization is advocated to prevent intensive care unit-acquired physical weakness, but the patient's workload and its changes in response to body position changes have not been established. We used indirect calorimetry to determine the energy expenditure (EE) in response to body position changes, and we assessed EE's correlation with respiratory parameters in healthy volunteers: 8 males and 8 females, mean age 23.4±1.3 years. The subjects started in the resting supine position followed by a 30° head-up position, a 60° head-up position, an upright sitting position, a standing position, and the resting supine position. EE was determined in real time by indirect calorimetry monitoring the subject’s respiratory rate, tidal volume (VT), and minute volume (MV). The highest values were observed immediately after the subjects transitioned from standing to supine, and this was significantly higher compared to the original supine position (1,450±285 vs. 2,004±519 kcal/day, p<0.01). Moderate correlations were observed between VT and EE (r=0.609, p<0.001) and between MV and EE (r=0.576, p<0.001). Increasing VT or MV indicates an increasing patient workload during mobilization. Monitoring these parameters may contribute to safe rehabilitation. Further studies should assess EE in critically ill patients. |

| 抄録 | 本研究の自的は，自己決定理論に基づいて開発した留学生を対象としたアルバイト動機づけ尺度(閻・堀内， 2017）の因子的妥当性について，確認的因子分析を用いて検討することである。3つの研究において得られた363名分の在日中国人留学生のデータを分析に使用した。自己決定理論に基づく理論的検討および先行研究，探索的因子分析，二次因子分析の結果をもとに， 6つのモデルを想定した。磁認的因子分析を行った結果， 5因子構造を基にした3つのモデルが高い適合度を示し，アルバイト動機づけ尺度が5因子構造を有することが確認された。さらに，5因子情迭のモデルの中でも，内発的調整，統合的調整と同一化的調整からなる「自律的動機づけ」と，取り入れ的調整と外的調整からなる「統制的動機づけ」という2つの高次因子を仮定した高次因子分析モデルが最も高い適合度を示すことが明らかとなった。 |

| 抄録 | Around April, total precipitation in southern Kyushu already attains rather large value due to the increase in contribution of the “heavy rainfall days” (with daily precipitation more the 50 mm/day, referred to as HRD). The present study examined the rainfall features and atmospheric conditions on the HRDs at Kagoshima in southern Kyushu, based on the operational observation data from 1990 to 2009. The HRDs there in April appeared mainly associated with the extratropical cyclone passage, together with tentative appearance of the stationary front. The mean daily precipitation on the HRDs was 82 mm and contribution of the intense rainfall more than 10 mm/h attained up to 38 mm, which accounted for 46 % of the daily total precipitation. In April, the high specific humidity area in the time mean field was still located far to the south of the Japan Islands and the stratification around the Japan Islands was very stable for deep moist convection. | 抄録 | We developed a surface matching-type computed tomography (CT)-based navigation system for total hip arthroplasty (the N-navi; TEIJIN NAKASHIMA MEDICAL, Okayama, Japan). In the registration step, surface matching was performed with digitizing points on the pelvic bone surface after coarse paired matching. In the present study, we made model bones from the CT data of patients whose acetabular shapes had various deformities. We measured the distances and angles after surface matching from the fiducial points and evaluated the ability to correct surface-matching registration on each pelvic form, using several areas and numbers of points. When the surface-matching points were taken on the superior area of the acetabulum, the correction was easy for the external direction, but it was difficult to correct for the anterior and proximal directions. The correction was difficult for external and proximal directions on the posterior area. Each area of surface-matching points has particular directions that are easily corrected and other directions that are difficult to correct. The shape of the pelvis also affected the correction ability. Our present findings suggest that checking the position after coarse paired matching and choosing the surface-matching area and points that are optimal to correct will improve the accuracy of total hip arthroplasty and reduce surgical times. |

| 抄録 | To date, many animal studies have indicated the neuroprotective effects of mesenchymal stem cell (MSC) transplantation in ischemic stroke. Several clinical studies have also revealed the safety, feasibility, and neuroprotective effects in ischemic stroke patients. In this review, we present the main approaches of MSC transplantation in ischemic stroke, the mechanisms of MSC therapy, and the current clinical studies on MSC transplantation in ischemic stroke patients. We also explore the safety of MSC transplantation and conclude that MSC therapy will play an important role in the future treatment of ischemic stroke. The optimal timing, approach, and cell dose in the transplantation are important issues for successful clinical application. |

| 抄録 | Esophageal achalasia is a disorder of the lower esophageal sphincter muscle. Patients present with dysphagia, chest pain, vomiting, and aspiration. Esophageal achalasia had traditionally been treated with esophageal achalasia balloon dilatation and the Heller-Dor method, but in recent years, the use of per-oral endoscopic myotomy (POEM) has increased. Our patient, a 39-yr-old male, began experiencing dysphagia 4 years prior to his referral to our hospital. Based on the results of esophagogastroduodenoscopy, esophageal radiography and high-resolution manometry, we made the diagnosis of esophageal achalasia (Chicago classification type I) . After informed consent from the patient and his family and approval from our hospital's ethics committee were obtained, we performed a POEM. The patient was discharged on the 4th day post-surgery. At the 1-year post-operative examination, no worsening of symptoms and no relapse were observed. POEM is an excellent treatment method for esophageal achalasia from the perspective of therapeutic effect and prevention of invasion. We recommend that it be considered as the first-choice treatment for achalasia. However, accessibility to the procedure itself is limited due to the few adequately trained operators worldwide. POEM should thus be performed by an expert operator at a high-volume center. |

| 抄録 | Electroencephalogram (EEG) data include broadband electrical brain activity ranging from infra-slow bands (< 0.1 Hz) to traditional frequency bands (e.g., the approx. 10 Hz alpha rhythm) to high-frequency bands of up to 500 Hz. High-frequency oscillations (HFOs) including ripple and fast ripple oscillations (80-200 Hz and>200 / 250 Hz, respectively) are particularly of note due to their very close relationship to epileptogenicity, with the possibility that they could function as a surrogate biomarker of epileptogenicity. In contrast, physiological high-frequency activity plays an important role in higher brain functions, and the differentiation between pathological / epileptic and physiological HFOs is a critical issue, especially in epilepsy surgery. HFOs were initially recorded with intracranial electrodes in patients with intractable epilepsy as part of a long-term invasive seizure monitoring study. However, fast oscillations (FOs) in the ripple and gamma bands (40-80 Hz) are now noninvasively detected by scalp EEG and magnetoencephalography, and thus the scope of studies on HFOs /FOs is rapidly expanding. |

| 抄録 | 本稿は、日本の大学での韓国語学習者のニーズを把握し、どのような授業形態と教育が彼らの学習意欲を向上させ、さらに彼らの学習目標の達成に貢献できるのかを把握することを目的とし、岡山県内の大学の教養外国語科目としての韓国語学習者のアンケート調査の結果をもとに、日本の韓国語教育の問題を明らかにすると同時に、今後の韓国語教育をより向上させるための方向性を提示した。方向性の提示では、韓流コンテンツの活用、eラーニングの導入及び活用が学習意欲の向上に寄与するものと示し、また授業以外の学習時間の活用と、言語と文化の同時教育という面から韓流コンテンツは不可欠であることを明らかにした。韓国語教育は、学習者の要望に応える教育方法の研究と教育カリキュラムの確立、また教養外国語教育としての特性に合った教科書の開発と学習者の目標達成が可能となるカリキュラムの確立が必要であることを主張した。 |
