| 抄録 | In order to re-examine the formation process of the climatological precipitation difference between Kochi (Pacific side of Shikoku District) and Okayama (Seto Inland Sea side) characterizing the Seto Island Sea Climate in warm season, synoptic climatological analyses of the daily precipitation features and atmospheric fields were performed for the warm season (April to September) of 1985 - 2015, based on the daily and hourly precipitation data and weather maps by the Japan Meteorological Agency (JMA), and the NCEP/NCAR re-analysis data. The large climatological difference of precipitation at Kochi from that at Okayama throughout the warm season was greatly contributed to by the days with the precipitation difference between Kochi and Okayama (ΔPR) with greater equal to 30 mm/day. In addition, such large daily ΔPR was mainly due to the intense rainfall at Kochi with greater equal to 10mm/h throughout the warm season, except for a part of the cases in April when the “not so intense rain” with less than 10mm/h at Kochi contributed to the large ΔPR. However, it is noted that the features of the synoptic-scale atmospheric fields and possible roles of the mountain ranges in Shikoku District causing the large ΔPR were rather different among August (midsummer), September (Autumn rainfall season) and April (Spring) . |

| 抄録 | Climatological features and seasonal feeling in the season called by the same word such as “summer”would be rather different from region to region. Comparison of these features among the different regions, including the regions which are not so familiar to the students, would help to promote their fundamental ESD literacy, especially relating to the “Understanding of heterogeneous others”. Based on that concept, this paper will report an interdisciplinary lesson study for the university students on a theme how different the summertime climate and seasonal feeling among Germany, northern Europe and Japan are. In this study, the lesson practice was made in 2015 and 2018. In both classes, after the lecture on the summertime climate and seasonal feeling in these regions, the students created substitute songs with use of the melody of “Alles neu macht der Mai” (“The May makes all things new”, the same melody as a Japanese school song “Butterfly”) for the comparison between Germany and Japan. As for that between northern Europe, the melody of a Japanese school song “Furu-Sato” (“My country home”) was used for the creation of the substitute songs. It is noted that, not only the seasonal mean temperature or solar radiation condition, but also the features in association with the large day-to-day temperature variation was strongly reflected in the students’ works. |

| 抄録 | Mucinous carcinoma (MC) of the breast is a rare and special type of malignancy, with a substantial amount of extracellular mucin. We compared the clinicopathological features and the long-term survival of MC patients with those of invasive ductal carcinoma-no special type (IDC-NST) patients, and we examined prognostic factors of MC. A total of 116 patients with mucinous carcinoma and 3,258 patients with IDC-NST who underwent surgery at our hospital (1977-2008) were enrolled. The 10-year overall survival rate and breast cancer-specific survival rate (BSS) of the MC patients (88.3%, 93.7%) were both significantly higher than those of IDC-NST patients (81.6%, 85.0%) (p=0.015, p=0.005, respectively). A Cox regression analysis demonstrated that MC tended to be an independent prognostic factor (hazard ratio 0.44, p=0.098). The BSS of the MC patients with positive lymph node (LN) metastasis was significantly poorer than that of the patients without it, by univariate analysis (p=0.002). The tumor size in the MC patients with positive LN metastasis (mean 3.2 cm) was significantly larger than that in the patients without it (mean 1.9 cm) (p=0.0004). Although a Cox regression analysis revealed no independent factor, MC patients with positive LN metastasis should be treated for advanced invasive ductal breast cancer. |

| 抄録 | We assessed the usefulness of ChemoCalc, a software package for calculating drug costs, in helping patients understand these costs. We randomly assigned, in a 1 : 1 ratio, 20 women who had undergone surgery for early breast cancer to a group that discussed adjuvant treatment with their physicians using the ChemoCalc software (ChemoCalc group) or a group that discussed adjuvant treatment without ChemoCalc (Usual Explanation group). The participants completed a five-grade evaluation questionnaire after these discussions. The primary endpoint was the intergroup comparison of the questionnaire scores regarding participants’ understanding of their treatment-associated drug costs. Median age was not significantly different between the ChemoCalc group and Usual Explanation group (57 vs. 50, respectively; p=0.27). Patients in the ChemoCalc group had a significantly higher perceived level of understanding of the drug cost than those in the Usual Explanation group (5 [4-5] vs. 2.5 [1-5], respectively; p=0.002). Scores related to the patients’ perception that understanding drug costs is an important part of breast cancer treatment were also higher in the ChemoCalc group than the Usual Explanation group (5 [2-5] vs. 3 [1-5], respectively; p=0.049). ChemoCalc was found to be useful for understanding drug costs. |

| 抄録 | We aimed to clarify the prevalence of polypharmacy among elderly individuals in Japan. We used the data obtained from a large-scale population-based representative database of health insurance claims in a single prefecture in Japan. We examined all of the outpatient and pharmaceutical health insurance claims for National Health Insurance and those for Late-stage Elderly Health Insurance in Nagasaki Prefecture, Japan between April and June 2016. When two or more claim forms were issued for a patient in a single month, we combined the data and identified the number of prescribed drugs for each person. The definition of polypharmacy is a the prescription of six or more drugs per month. We investigated the prevalence of polypharmacy among the beneficiaries of the two insurance systems. Of the 605,406 beneficiaries of the 2 insurance systems, 121,033 (20.0%) patients with polypharmacy were identified. The prevalence of polypharmacy increased with age, especially among the beneficiaries aged > 85 years, with about half of the beneficiaries having polypharmacy status. About half of the people aged > 85 years in the database had polypharmacy status. When a drug is prescribed to an elderly individual, it is necessary to consider the possibility of polypharmacy-related problems. |

| 抄録 | Iron is a trace but vital element in the human body and is necessary for a multitude of crucial processes in life. However, iron overload is known to induce carcinogenesis via oxidative stress. Cancer cells require large amounts of iron for their rapid division and cell growth. Iron was recently found to play a role in cancer stem cells (CSCs); it maintains stemness during development. Iron also plays an important role in stemness by moderating reactive oxygen species. Thus, iron metabolism in CSCs is a promising therapeutic target. In this review, we summarize the roles of iron in cancer cells and CSCs. We also summarize anti-cancer therapeutic studies with iron chelators and describe our expectation of a new therapeutic strategy for CSCs on the basis of our findings. |
