| Abstract | 本稿では開化期の韓国語学習書である『実地応用朝鮮語独学書』(1896)について、韓国語の文とその仮名音注を分析し、本書に現れる19世紀末の韓国語の特徴(音注・語彙・助詞・語尾など)を概観した。また『実地応用朝鮮語独学書』を基に約20年後の1915年に著者の弓場重栄により京城で刊行された『ポケット朝鮮語独学』((1915))と本書との相違点をまとめ、韓国語の変遷などを考察した。本書には開化期のソウル方言の口語的な特徴がよく反映されており、言語学的な価値を持っているといえる。また開化期の商業関連用語が多く現れることと、「第四編 会話」には同時期の他の韓国語学習書と比べて独自的な文が多いことも特徴である。今後、明治期の他の韓国語学習書や韓国人のための日本語学習書を総合的に考察することで、現代韓国語の形成過程をより明らかにできると期待される。 |

| Abstract | ある時代のある特定の時期に書かれ発表されたさまざまな文学テクストをながめたばあい、ときにジャンルさえ越えて、そこに流行語のようにいくつかのある特徴的な語や表現が時を同じくして出現してくることがある。それはもちろん作家（詩人）から作家（詩人）へのなんらかの影響関係がそこにあったからだとまずは考えられよう。しかしこれは必ずしもたんに一方から他方への単方向的な影響というのではなく、作家（詩人）たちのあいだで、いくつかの条件があわさって、ほとんど同時発生的にある共通ないし類似の語や表現が生まれたり用いられたりすることもあったのではないか、また、あくまで個々の作品から遡及的にしか見いだされぬとしても、そうした共通ないし類似の語や表現を生みだすことを可能にした、ある時代のある特定の時期に成立していたと想定される表現可能態の言語空間がそこに潜在していたのではないか―― 本稿は、その空間をピエール・ブルデューの用語を借りて「文学場」の名で呼んで、20 世紀初頭における日本の近代詩と19 世紀中葉におけるフランスの近代詩をそうした「文学場」という観点からとらえなおす試みである。 |

| Abstract | We retrospectively reviewed 29 hips in which intertrochanteric osteotomies were performed for severe slipped capital femoral epiphyses. Mean age at surgery: 12.6 years. Mean follow-up period: 6 years. At the final follow-up evaluation, one patient had coxalgia, and six hips showed a limited range of motion. A pistol-grip deformity was observed in 13 hips, osteoarthritis in two hips, and a bump existed in 19 hips on the latest radiographs. Gradual remodeling of the bumps was observed post-operatively in 22 hips. The mean α and β angles and offset α and β improved over time. The remodeling proceeded rapidly for 1 year post-surgery. We compared hips classified as β angles of ≥ 63° to < 63° at the final follow-up evaluation, the mean β angle 1 year post-surgery, and the mean ratio of improvement of the β angle per year from 1 year post-surgery to the final follow up, which differed significantly. Nearly all of the patients who underwent intertrochanteric osteotomies had residual morphologic abnormalities, but few had clinical symptoms. The β angle 1 year post-surgery and the ratio of remodeling of the bump from 1 year post-surgery to the final follow-up can be regarded as a potential predictor of morphologic results after intertrochanteric osteotomy. |

| Abstract | Esophageal achalasia is a disorder of the lower esophageal sphincter muscle. Patients present with dysphagia, chest pain, vomiting, and aspiration. Esophageal achalasia had traditionally been treated with esophageal achalasia balloon dilatation and the Heller-Dor method, but in recent years, the use of per-oral endoscopic myotomy (POEM) has increased. Our patient, a 39-yr-old male, began experiencing dysphagia 4 years prior to his referral to our hospital. Based on the results of esophagogastroduodenoscopy, esophageal radiography and high-resolution manometry, we made the diagnosis of esophageal achalasia (Chicago classification type I) . After informed consent from the patient and his family and approval from our hospital's ethics committee were obtained, we performed a POEM. The patient was discharged on the 4th day post-surgery. At the 1-year post-operative examination, no worsening of symptoms and no relapse were observed. POEM is an excellent treatment method for esophageal achalasia from the perspective of therapeutic effect and prevention of invasion. We recommend that it be considered as the first-choice treatment for achalasia. However, accessibility to the procedure itself is limited due to the few adequately trained operators worldwide. POEM should thus be performed by an expert operator at a high-volume center. |

| Abstract | A 58-year-old Japanese man visited a local clinic for the evaluation of a stomachache. He was diagnosed with intestinal obstruction. His medical history included a proctocolectomy at the age of 38 years, due to familial adenomatous polyposis (FAP). He was referred to our institution, where he underwent a computed tomography examination and endoscopy of the small intestine. The pathological diagnosis was adenocarcinoma. No invasive or metastatic lesions were observed. Therefore, partial resection of the ileum with lymphadenectomy and reconstruction of the ileostomy were performed. Pathological examination revealed that the tumor was type 2, pT3 (SS) , pN1, pPM0, pDM0, pRM0, INFb, ly1, v1, pEX0, PN0. Twenty-nine days after the surgery, the patient was diagnosed with lung metastasis and he underwent lung radiofrequency ablation. We suggest that long-term follow-up is necessary for patients after surgery for FAP, because of the risk of malignant disease developing in other organs. |

| Abstract | We elucidate the fate of cases clinically suspected of carcinoma in situ (Cis) of the upper tract with serial ureteroscopy. Of 143 patients who underwent ureteroscopy for suspected upper tract urothelial carcinoma (UTUC) between January 2008 and February 2016, 12 cases with consistently positive urine cytology and poorly detectable upper-tract malignancies by imaging were reviewed. In these 12 patients, 19 ureteroscopy procedures (25 renal units) were performed. Vesical random biopsy was performed before the 1st ureteroscopy to exclude malignancy of the bladder in all 12 cases. Median follow-up was 42 (13-67) months. Positive biopsy results at the 1st ureteroscopy were obtained in 3 (25%) patients and all were diagnosed wth Cis of the upper tract. Two (17%) of 9 patients who were negative or inconclusive at the 1st ureteroscopy were finally diagnosed as UTUC, but plural ureteroscopy procedures were needed for the diagnoses in both. Carcinoma of the bladder appeared in 5 (42%) patients during follow-up, despite the earlier ruling out of vesical malignancy. Four (33%) of those 5 patients never developed upper-tract urothelial carcinoma during follow-up. Caution is required before undertaking radical surgery for cases clinically suspected of Cis of the upper tract. In our experience, only 42% of such patients developed UTUC; another 33% eventually developed carcinoma of the bladder without UTUC. |

| Abstract | 中高緯度における季節サイクルの多様性や日々の変動に伴う季節の中の幅にも注目して，ドイツ付近での冬の追い出しの行事「ファスナハト」を題材に学際的授業を開発し，大学での教科横断的授業の中での実践結果を検討した。授業では，「ドイツではなぜ冬を追い出さずにはいられないのか」を気候データからイメージして，それを音楽で表現する活動（ファスナハトのオリジナル作品の創作）を行なった。ドイツの冬の厳しさは，平均気温が低いだけでなく，日々の変動が大きい中での極端な低温日の頻出にも特徴づけられていた。音楽での表現活動では，気候データの把握とファスナハトの行事の映像の視聴等により，その行事の存在の意義を考えることが出来た。更に，それらを踏まえた創作と演奏により，日本と比較したドイツ付近の季節の特徴や移り変わりの何に自分が注目しているのかを学生自身が再認識して，気候やそこに住む人々の感じ方を想像する機会となった。 |

| Abstract | Anthraquinone, an anthracene derivatives, is widely used as a purgative medicine. The long-term use of anthraquinone is known to cause melanosis coli and elevated lesions in the colon. However, the clinical course of patients who take anthraquinone over a long term and those who discontinue the drug has not been fully investigated. Here we investigated 22 patients who had colonic mucosal changes ( i.e., melanosis coli and elevated lesions) who had been taking anthraquinone for at least one year and discontinued it. We classified the elevated lesions into two subtypes:the small-lesion, which included multiple small nodules that were ＜ 2 mm in dia., and the large-lesion, which included a single or multiple nodules of ≥ 2-mm dia. We used colonoscopy to investigate the changes of colonic lesions between before and after the discontinuation of anthraquinone. The results indicated that the nodules of the small-lesion group were lymphoid follicle hyperplasia. In the large-lesion group, adenoma was most frequently observed ( n＝118) , followed by hyperplastic polyp ( n＝52) and inflammatory changes with edema ( n＝22) . Melanosis coli and elevated lesions were decreased or invisible after the discontinuation of anthraquinone. Our findings thus suggest that the long-term use of anthraquinone may cause some type of elevated lesions due to chronic inflammation. It is recommended that the use of anthraquinone be limited to a short term. |

| Abstract | We prospectively assessed sexual function before and after a tension-free vaginal mesh ( TVM) procedure for pelvic organ prolapse ( POP) . From April 2007 to March 2009, a total of 42 patients, who reported having an active sex life preoperatively underwent TVM for POP. Female sexual function was evaluated with the self-reporting, multiple-domain Female Sexual Function Index (FSFI) . We administered the FSFI to all of the patients before and at 3, 6, and 12 months after surgery. The mean age at surgery was 61.3±7.1 years. Before the TVM surgery, the FSFI score was very low ( 12.5±9.0) . The total FSFI score was improved significantly at 12 months after surgery ( 17.4±7.7) . The Arousal, Lubrication and Orgasm domains were significantly improved at 12 months after surgery. POP appears to have a significant negative impact on female sexual function. Our findings indicate that TVM for POP improved sexual function in a group of Japanese women with POP. |
