Abstract | Rainfall distribution in the Japan Islands associated with the approach of Typhoon No.23 around 20 October 2004 (referred to Ty0423, hereafter) showed considerably different features from those in late summer of this year. The present study examined the detailed rainfall features around the Japan Islands brought by Ty0423 and the atmospheric processes based on the operational observation data by the Japan Meteorological Agency (JMA), comparing with those in association with Ty0418 around 7 September 2004. During the stage when Ty0423 was approaching or landing on the western part of the Japan Islands, the areal mean precipitation from Kyushu to Kanto District attained much larger than that for Ty0418, with wider extension of the area with the large amount of precipitation. It is interesting that, although the intense rainfall was observed only at the upstream side of the mountain range from Kyushu to Honshu District for Ty0418 except for the area near its center, strong rainfall with 10~30 mm/h persisted in wider regions from the western to the eastern part of the Japan Islands, resulting in the considerably large total rainfall for Ty0423. As for the case for Ty0423, the surface front with stable frontal surface was located just to the east of the Ty0423 center just before its landing at the Japan Islands. Thus, the huge moisture inflow mainly in the eastern region from the typhoon center seems to be redistributed widely over the Japan Islands area associated with the large-scale convergence around the stable frontal surface. In late October, the colder air associated with the high pressure system in the eastern Siberia can cover the northern part of the Japan Sea area as the seasonal march. Such basic field might be favorable for sustaining the synoptic-scale front just around the southern coast of the Japan Islands, even when the strong southerly wind invades associated with the typhoon approach there. |

Abstract | The purpose of this study was to compare the positive surgical margin (PSM) rates of 2 techniques of robot-assisted radical prostatectomy (RARP) for pT2 (localized) prostate cancer. A retrospective analysis was conducted of 361 RARP cases, performed from May 2005 to September 2008 by a single surgeon (KHR) at our institution (Yonsei University College of Medicine). In the conventional technique, the bladder neck was transected first. In the modified ultradissection, the lateral border of the bladder neck was dissected and then the bladder neck was transected while the detrusor muscle of the bladder was well visualized. Perioperative characteristics and outcomes and PSM rates were analyzed retrospectively for pT2 patients (n=217), focusing on a comparison of those undergoing conventional (n=113) and modified ultradissection (n=104) techniques. There was no difference between the conventional and modified ultradissection group in mean age, BMI, PSA, prostate volume, biopsy Gleason score, and DʼAmico prognostic criteria distributions. The mean operative time was shorter (p<0.001) and the estimated blood loss was less (p<0.01) in the modified ultradissection group. The PSM rate for the bladder neck was significantly reduced by modified ultradissection, from 6.2% to 0% (p<0.05). In conclusion, modified ultradissection reduces the PSM rate for the bladder neck. |

Abstract | Cholestasis and thrombocytopenia are complications that affect infants born small for gestational age (SGA). In SGA infants, other vital organs develop at the expense of the liver, and the thrombopoietin produced by the liver is low, often resulting in cholestasis. We hypothesized that thrombocytopenia at birth can be used to predict cholestasis in very-low-birth-weight infants (VLBWIs) with SGA. This retrospective cohort study enrolled VLBWIs with SGA admitted to a tertiary neonatal intensive care unit. A platelet cutoff value predictive of cholestasis was determined using receiver operating characteristic analysis. Multivariate logistic regression analysis was performed to evaluate the platelet cutoff value, and adjusted odds ratios (ORs) and 95% confidence intervals (CIs) were calculated. Regarding the onset of cholestasis, survival analysis was performed by calculating the adjusted hazard ratios (HRs) and 95% CIs. A total of 87 infants were evaluated, and the platelet cutoff value was determined as 88×10(3) cells/μl. The adjusted OR for this platelet cutoff value was 10.52 (95% CI 2.26-55.93, p=0.003), and the adjusted HR was 7.76 (95% CI 2.51-23.50, p=0.0006). Thrombocytopenia is a useful predictor for cholestasis in VLBWIs with SGA. |

Abstract | Struma ovarii is a rare neoplasm that accounts for approximately 0.3オ of ovarian tumors. Due to its ultrasound morphology, which is quite similar to that of malignant ovarian carcinoma, most struma ovarii cases are open operated with laparotomy rather than laparoscopy. We present 3 cases of struma ovarii, which were diagnosed preoperatively by imaging studies and removed by laparoscopic surgery. All patients were premenopausal women between ages 31‒50. The magnetic resonance imaging (MRI) findings were complex masses composed of multiple cysts and solid components with T2-hypointense regions as well as multiple T1-hyperintense cystic areas, findings that are typical for struma ovarii. A combination of plain computed tomography (CT), positron emission tomography (PET)-CT, and scintigraphy was useful for diagnosis. Laboratory examination revealed elevated serum thyroglobulin, which led to the diagnosis of struma ovarii. Laparoscopic surgeries were performed without rupturing the tumors. Although it has been difficult to differentiate between struma ovarii and malignant tumors by conventional methods, recently MRI techniques appear make it possible to diagnose struma ovarii preoperatively from the abovementioned imaging characteristic, together with laboratory data. As for treatment, we think laparoscopy could be successful for struma ovarii, but the surgeon must be careful not to rupture the tumor intra-abdominally in order to prevent dissemination, which could lead to malignancy. |

Abstract | Cervical misalignment after upper cervical fusion including the occipital bone may cause trismus or dysphagia, because the occipito-atlanto joint is associated with most of the flex and extended motion of the cervical spine. There are no reports of dysphagia and trismus after C1-2 fusion. The purpose of this paper is to demonstrate the potential risk of dysphagia and trismus even after upper cervical short fusion without the occipital bone. The patient was a 69-year-old man with myelopathy caused by os odontoideum and Klippel-Feil syndrome, who developed dysphagia and trismus immediately after C1-2 fusion and C3-6 laminoplasty. Radiographs and CT revealed that his neck posture was extended, but his symptoms still existed a week after surgery. The fixation angle was hyperextended 12 days after the first surgery. His symptoms disappeared immediately after revision surgery. The fixation in the neck-flexed position is thought to be the main cause of the patientʼs post-operative dysphagia and trismus. Dysphagia and trismus may occur even after short upper cervical fusion without the occipital bone or cervical fusion in the neck-extended position. The pre-operative cervical alignment and range of motion of each segment should be thoroughly evaluated. |

Abstract | Recent efficient pairings such as Ate pairing use two efficient rational point subgroups such that π(P) = P and π(Q) = [p]Q, where π, p, P, and Q are the Frobenius map for rational point, the characteristic of definition field, and torsion points for pairing, respectively. This relation accelerates not only pairing but also pairing–related operations such as scalar multiplications. It holds in the case that the embedding degree k divides r − 1, where r is the order of torsion rational points. Thus, such a case has been well studied. Alternatively, this paper focuses on the case that the degree divides r + 1 but does not divide r − 1. Then, this paper shows a multiplicative representation for r–torsion points based on the fact that the characteristic polynomial f(π) becomes irreducible over Fr for which π also plays a role of variable. |

Abstract | A case study on persistent appearance of the ensemble of precipitation bands in the warm sector of the Baiu front in Kyushu District, the western part of Japan around 19 June 2001 was performed based on the operational observational data. Around 00UTC (09JST) 19 June when the Baiu front on the surface weather map was still located about a few 100kms to the north of Kyushu, the number and the total area of the meso-β or γ-scale line-shaped precipitation bands increased around Kyushu rapidly. These precipitation bands had been sustained there and gradually gathered each other to change into the Baiu frontal precipitation zone extending from the northern Kyushu to Seto-Naikai (Seto Inland Sea) around 09UTC (18JST) 19 June (the intense rainfall zone was still located in the warm sector of the Baiu front). Until ~09UTC (18JST) 19 June, the moist air flow with the latently unstable stratification had been sustained around Kyushu corresponding to the low-level southerly wind toward the surface Baiu front to the north of Kyushu. This large-scale situation would contribute greatly to the maintenance of the ensemble of the line-shaped precipitation bands in the warm sector of the Baiu front. |
