腎移植者のQOLに関係する諸概念の測定用具の作成および信頼性と妥当性の検討

腎移植者のQOL向上をめざした系統的なアプローチを検討するにあたり,QOLに関係があると考えられる諸要因を導き出した｡本研究の目的は,それらの諸要因の関係を明らかにするために測定用具を作成し,その信頼性と妥当性を検討することである｡作成した測定用具は身体の状態,自己概念,不確かさ,ソーシャルサポート,対処およびQOLである｡測定用具の信頼性と妥当性は,4施設に通院をしている20オ以上の腎移植者210名を対象に行った｡信頼性はクロンバックαによる内部一貫性ないしは評定者間信頼性を検討し,妥当性は,構成概念妥当性ないしは併存妥当性を検討した｡自己概念,不確かさ,ソーシャルサポートおよび対処の尺度は,それらの項目の因子分析の結果,因子項目のクロンバックα信頼性係数が0.7以上で内部一貫性が支持され,さらに確認的因子分析により構成概念妥当性が支持された｡身体の状態およびQOLの尺度は,主成分分析の結果,各々の項目が総合化された主成分であることが示され,またそれらの尺度の評走者間信頼性ないしは併存妥当性が支持された｡したがって,上記の結果から,作成した尺度は腎移植者に適用可能な測定用具であることが示された。

The purpose of this study was to assess the reliability and validity of the instruments developed to measure physical state, self-concept, uncertainty, social support, coping, and QOL. The sample consisted of 210 kidney posttransplant recipients aged 20 or more and they were the cases from four hospitals. In respect to each item of self-concept, uncertainty, social support, and coping, factor analysis was made to examine the factor structure. Cronbach's alphas were calculated for each of their subscales. Construct validity was assessed using the confirmatory factor analysis. The results supported the internal consistency reliability of their subscales (alphas=0.7 or more) and construct validity. Principal component analysis was used to examine the principal components of physical state and QOL. Concurrent validity and interrater reliability also were assessed and the results supported them. Therefore, all of them were applicable instruments for Japanese kidney transplant recipients.
