抄録 | We retrospectively evaluated the safety and efficacy of artificial pneumothorax induction to perform computed tomography (CT)-guided radiofrequency ablation (RFA) for sub-diaphragm hepatocellular carcinomas (HCCs). From June 2008 to October 2010 at our institution, 19 HCCs (16 patients) were treated using CT-guided RFA after artificial pneumothorax induction. A 23-G needle was inserted into the liver surface at a site of 2 connected pleurae without lung tissue. After a small amount of air was injected, the pleural space widened, creating a small pneumothorax. Additional air was insufflated via a newly inserted 18-G cannula to raise the lung away from the planned puncture line for RFA. The electrode was then advanced transthoracically. Ablation was performed using a cool-tip electrode with manual impedance control mode. The injected air was then aspirated as much as possible. Artificial pneumothorax was successfully induced in all cases. The average total volume of injected air in each case was 238ml. No artificial pneumothorax-related complication occurred; lung injury occurred in one case during RF electrode insertion. No local progression occurred during follow-up. Recurring HCCs were observed in eight patients. Artificial pneumothorax induction is safe and effective for CT-guided RFA of sub-diaphragm HCCs, which are difficult to locate on US. |

抄録 | In neurosurgery in particular, the recommended placement of electrodes for monitoring depth of anesthesia during surgery sometimes conflicts with the surgical site or patient positioning. Therefore, we proposed this study to evaluate the agreement and correlation of bispectral index values recorded from the usual frontal area and the alternate, post-auricular areas in neurosurgery patients. Thirty-four patients scheduled for neurosurgery under general anesthesia were included. Bispectral index (BIS) sensors were placed at both the frontal and post-auricular areas. The anesthesia given was clinically adjusted according to the frontal (standard) BIS reading. The BIS values and impedance were recorded;Pearsonʼs correlation and Bland-Altman plots were analyzed. The bias ± 2SD for the electrode placement before, during, and post-anesthesia were 0±23.32, 1.5±10.69, and 2.1±13.52, while the limits of agreement were -23.3 to 23.3, -12.2 to 9.2, and -17.7 to 13.5, respectively. The correlation coefficient between frontal- and post-auricular-area electrodes was 0.74 with a p-value <0.001.The post-auricular placement of a BIS electrode is a practical alternative to frontal lobe placement. Nevertheless, proper electrode location is important to minimize error. |

抄録 | Immunosuppressive agents are generally administered to treat kidney diseases. However, it is unclear whether renal stem/progenitor cells are directly affected by the immunosuppressive agents. We used normal rat kidney cells, ureteric bud cells and rat kidney stem/progenitor cells in this study. Mizoribine (MZR), cyclophosphamide (CPA) and cyclosporine (CyA) were added to the culture media of these cells. We evaluated the effects of these immunosuppressive agents on cell proliferation using an electrical cell-substrate impedance sensing system (ECIS) and their effects on the process of renal regeneration using the ischemia-reperfusion (I/R) injury rat model. The ECIS data showed that proliferation of each of the 3 types of cells was significantly suppressed by MZR. MZR treatment enhanced renal tubular injury in ischemia-reperfusion (I/R) injured rats, and significantly decreased levels of M-phase cells and Nestin-positive cells. These results suggested that MZR inhibits the cell cycle of renal stem/progenitor cells;thus, physicians should take note that MZR might affect not only inflammation but also renal regeneration. |

抄録 | Resistivity distribution sounding of the non-homogeneous earth is important for electrical ground system design, geophysical prospecting and survey or monitoring the groundwater flow level. The previous paper presented that the direct inversion of the electric resistivity distribution in a domain is possible from the impedance data measured over the domain boundary using the dual reciprocity boundary element modelling in two-dimentional field [1]. The proposed inversion technique is extended to the distribution in three-dimensional space [2]. This technique is capable of inversion without iteration and meshing of the domain. Electric field with spatially varying conductivity is governed by Laplace equation, which is transformed into a Poisson-type expression with an inhomogeneous term involving the conductivity difference as a source term. Dual reciprocity method (DRM) is a technique for transforming the domain integral associated with the inhomogeneous term in Poisson equation into the boundary integral expression. The resistivity distribution in the field can thus be identified from the data observed over its boundary, for which some examples are demonstrated [2]. In this paper, the examination is extended to the case where only the data measured over the single surface is used for the inversion. |
