| 抄録 | (1) Earlier beginning of menses and fewer sterilities in the women taking thermal bath habitually than in the other women were observed. (2) No significant difference was proved between the afore-said both groups in the period and duration of menstruation, average number of pregnancies and average term from marriage to first pregnancy. (3) No remarkable difference was shown concerning the above-mentioned actions between the radioactive spring and the other thermals. |

| 抄録 | The use of dental implants has increased together with increases in the human life span and it has become an imperative subject for dentists to familiarize themselves with this treatment modality. Unfortunately, there has been no practical and quantitative method for in vivo evaluation of the stability of dental implants. In the tooth mobility examination, the tactile sense of natural teeth is different from that of dental implants. The authors have developed an automatic diagnosis system of tooth mobility for clinical use. The biomechanical mobility of peri-implantium is measured with a pseudo-random vibration, from which the viscoelasticity c1, c2, k of peri-implantium is obtained. The diagnosis system has been applied to the quantitative evaluation of the stability of implants : endodontics endosseous implants (titanium pin), endosseous implants (Bioceram). It has also been applied to the evaluation of the long-term prognoses of dental implantation (Bioceram) and the examination of Intramobile implant (IMZ), and the satisfactory results have been obtained. |

| 抄録 | In this paper we provide some theoretical results on stability and sensitivity analysis in convex vector optimization. Given a family of parametrized vector optimization problems, the perturbation maps are defined as the set-valued map which associates to each parameter value the set of minimal points (properly minimal points, weakly minimal points) of the perturbed feasible set with respect to an ordering convex cone. Sufficient conditions for the upper and lower semicontinuity of the perturbations map are obtained. We also provide quantitative properties of the perturbation maps under some convexity assumptions. |

| 抄録 | Mutation is one of the important operators in genetic algorithm. In traditional genetic algorithm, mutation is activated stochastically. In this way it is unknown and cannot be controlled for which individuals to be mutated. Therefore, it is unavoidable that some good individuals are destroyed by mutation and then the evolutionary efficiency of the genetic algorithm is dampened. Owing to this kind of destructivity of mutation, the operator of mutation has to be limited within a very small probability, and the potentiality of mutation is consequently limited. In this paper, we present an evolutionary chain-based mutation and a control strategy of reasonable competition, in which the heuristic information provided by the evaluation function is well utilized. This method avoids the blindness of stochastic mutation. The performance improved in this method is shown by two examples, a fuzzy modeling for the identification of a nonlinear function and a typical combinatorial optimization problem-the traveling salesman problem. |

| 抄録 | A monoacylglycerol lipase (MGL) was purified from Pseudomonas sp. LP7315 by ammonium sulfate precipitation, anion-exchange chromatography, and preparative electrophoresis. The purified enzyme was homogeneous on an SDS-polyacrylamide gel with a molecular mass of 59 kDa. Its hydrolytic activity was confirmed to be specific for monoglycerides: the enzyme did not hydrolyze diand triglycerides. MGL was found to be stable even after l-h incubation at 65℃. The hydrolytic activity depended not only on temperature and pH but also on the type of monoglyceride used. MGL also catalyzed monoglyceride synthesis at 65℃ in a solvent-free two-phase system, in which fatty acid droplets were dispersed in the glycerol phase with a low water content. The synthetic reaction proceeded at a constant rate for approximately 24 h and reached an equilibrium after 48 h of reaction. The initial rate of the synthetic reaction depended on several factors: the type of fatty acid used as the substrate, the amounts of fatty acid and glycerol, and the concentration of MGL in the glycerol phase. To analyze the effects of these factors, a kinetic model was developed based on the assumption that the adsorption equilibrium of MGL molecules at the interface between the two phases is the rate-determining factor for the synthetic reaction. The model was found to yield a good approximation of the initial synthetic rate under various reaction conditions. The analysis suggests that the adsorption behavior of MGL onto the interface had a large effect on the initial rate of the monoglyceride synthesis. |

| 抄録 | 光ディスクとパーソナルコンピュータを組み合わせた内視鏡画像ファイリングシステムは電子内視鏡の画像の保存･管理,検索･再生などに威力を発揮する｡今回,われわれはオリンパス社製内視鏡ファイリングシステムSDF-3を使用する機会を得たので,その有用性や問題点の検討を行った｡有用性としては,①内視鏡画像を検査終了後直ちに再生し,再検討できる,②多数の項目についての検索や統計処理が可能である,③限られたスペース内で大量の画像データの一括保存･管理が可能であるなどが挙げられる｡一方,問題点としては,①内視鏡再生画像は画質が劣化する,②検索機能を十分に活用するにはかなり煩雑な人力操作を必要とする,③光ディスクの清掃が必要であるなどがあげられる｡本内視鏡ファイリングシステムに今後必要な改良点やその展望についても合わせて考案を加えた。 |

| 抄録 | ERCP直後にCTを撮影するERCP後CTはERCPの際に膵内に注入された造影剤をCTで検出するため,ERCPでは得られない情報が得られる利点がある｡われわれはERCP後CTが診断や治療方針の決定に有用であった膵嚢胞の4例を経験した｡膵嚢胞では主膵管との連絡,嚢胞の部位,性状診断にERCP後CTは有用な検査法であり,文献的考察を含めて報告した｡ |

| 抄録 | 2重可移群には,near-domainを解釈することができ(定理13), またnear-domainから2重可移群を構成することができる｡つまり,2重可移群の研究はnear-domainの研究と同値になる｡ここで,有限のnear-domainがnear-fieldになることは知られているが,無限のnear-domainがnear-fieldになるかどうかは知られていない｡これに関連して,無限の2重可移群についても多くの未解決問題が残されている｡このノートでは,これらの問題にたいするモデル論的なアプローチ(Morley rank有限の場合の構造析,geometricな方法など)をいくつか紹介する｡ |

| 抄録 | 看護教育では,看護者として必要な知識,技術のみならず看護するのにふさわしい態度や姿勢等を育成することを重視している｡このような看護者にふさわしい態度形成を援助する教育の成果を評価するために看護学生の適性に関する尺度開発を試みた｡尺度に用いる項目は,看護者にふさわしい態度や性格特性及び適性等に関する過去の文献調査と筆者等の臨床看護及び看護教育の経験とで検討し決定した｡最終的に絞り込んだ項目は,22項目であった｡この項目の信頼性･妥当性を検証するために,医療技術短大の看護学生206人を対象に質問紙調査を実施した｡信頼性については,76人を対象に再検査を実施し安定性を検討した｡その結果,相関係数の一番低い項目は｢研究心｣｢自制心｣で,0.36であった｡残りの項目は,相関係数0.40以上で,一番高い項目は｢冷静｣の0.71であった｡内的整合性を示すCronbach's α信頼性係数は,全項目では0.83であった｡因子分析の結果得られた5因子に基づく下位尺度のCronbach's α信頼性係数は,最も高いものは<リーダーシップ>0.80であり,最も低いものは<精神的成熟>0.56であった｡構成概念の妥当性を検討するために因子分析をおこなった結果,5因子が抽出された｡それぞれを<リ ーダーシップ><社会性><探求心><精神的成熟><忍耐>と命名し,下位尺度とした｡ |

| 抄録 | 地域で生活する高齢者･障害者にとって,住宅環境の整備は必須の要件である｡津山市では在宅生活を支援する立場にある保健婦やホームヘルパー,作業療法士等保健･福祉関係者と,建築関係者が共同で支援チームをつくり住宅改造に関わってきた｡本研究では,この支援チームが関わった事例と直接関わらないで当事者と業者のみで改造を実施した事例を比較し,支援チームが関わることの効果をみた｡調査方法は,各事例の家庭を訪問して面接聞き取り調査を行った｡調査内容は調査票を用いて改造箇所,生活空間,介護量,外出の状況について尋ねた｡改造前後の変化を比較して,支援チームの効果について以下のことが明らかになった｡ 1.支援チームの関与による浴室の改造は,入浴が可能となり生活空間が広がるなど改善が認められた｡ 2.離床を目的にした玄関･廊下･居室の改造は,外出などの日常生活行動を広げ,本人のQOLを高めることに役立つ｡以上より住宅の改造が,介護者の介護負担を必ずしも直接軽減するとは限らないが,高齢者･障害者の介護の質に変化を与えることが示唆された。 |

| 抄録 | チーム医療に対して高い関心が寄せられていることから,チーム医療として認識される具体的な事柄や各職種に対する認識,その利点や職種間の認識の違いなどを明らかにする目的で,岡山県内の病院の看護師,診療放射線技師,臨床検査技師,計652名に対してアンケート調査を行った｡有効回答者数470名で,その内訳は看護師207名,診療放射線技師91名,臨床検査技師172名であった｡結果はチーム医療と認識される具体的な事柄については,異業種間カンファレンスがチーム医療を促進する事柄として認識されているが,整備･実施されていないことがうかがえた｡各職種に対する認識については,職種別･経験年数別にみても,医師,看護師,理学･作業療法士はチーム医療のメンバーとして認識されている割合が高かった｡また,経験年数別では,10～14年の人までは統計上ややばらつきがみられるが,経験年数15年以上の人はあらゆる職種をチーム医療のメンバーであると考え,その重要性を認識していることがわかった｡チーム医療の利点については,どの職種も共通して｢患者中心の医療｣と考えている割合が高かった｡ |

| 抄録 | In 360 females, Bone mineral density (BMD) in lumbar vertebrae (L2-L4) was assessed by quantitative computed tomography (QCT), and the values obtained were compared with the frequency of vertebral transformation or fracture as assessed by lateral scan image (scanogram) by X-ray CT. A correlation was observed between the frequency of vertebral transformation (or fracture) and lumbar BMD values : BMD under 125 mg/cm(3) was observed over 90% of women with vertebral transformation, and BMD under 70mg/cm(3) was found about 50% of them. These results suggest that decrease in BMD in lumbar vertebrae leads to vertebral transformation or fracture. Thus, measurement of BMD by QCT would be very useful in predicting vertebral transformation or fractures. |

| 抄録 | Tropical calcific pancreatitis is a nonalcoholic type of chronic pancreatitis affecting the childrens and young adults characterized clinically by recurrent abdominal pain in childhood, diabetes in adolescent and death in early childhood. Although the exact etiology is not known, malnutrition and chronic cassava toxicity either singly or in combination are presumed to be the prime factor in pancreatic injury unopposed by detoxification of free radical. Moreover micronutrients deficiency, oxidant stress and antioxidant deficiency might play substantial role. Diabetes secondary to tropical calcific pancreatitis is a distinctive and frequent problem, being named by W.H.O. study group as 'fibrocalculous pancreatic diabetes (FCPD) and classified as one of the variant of the so-called malnutrition related diabetes mellitus (MRDM). |

| 抄録 | Total colonoscopyの前処置における被検者の負担軽減と良好な腸管洗浄を得る目的で,50例の大腸内視鏡検査において,前日の食事制限せず, sodium picosulfate 20mℓ前夜服用,検査当日PEG腸管洗浄液1ℓ以上服用する前処置法の有用性について検討した。本前処置法によってPEG腸管洗浄液平均1230mℓの服用により,50例中48例で観察可能な腸管洗浄が得られ,PEG腸管洗浄液の服用量の減量が可能であった｡腹痛,嘔気,腹鳴などの症状出現例は認めたが,重篤な副作用は認めなかった｡腸管洗浄度の点で,高齢者の大腸内視鏡検査の前処置として有用である｡以上よりSodium Picosulfate 20mℓをPEG腸管洗浄液と併用することにより,優れた腸管洗浄度を得られると同時にPEG腸管洗浄液服用量の減量か可能であり,total colonoscopyの前処置として有用であることが示された。 |

| 抄録 | Experimental model of pancreatitis is mandatory for elucidating the pathobiology of the disease and also to see the response of a novel treatment. In addition, the need for an animal model of chronic pancreatitis is further strengthened by the relative inaccessibility and paucity of the human pancreatitis tissue. Whereas various models of acute pancreatitis and also of exocrine pancreatic tumor have been described, chronic pancr-eatitis has not been consistently reproduced in experimental animals. Many researchers attempted to establish an experimental model of chronic pancreatitis either by partially obstructing the drainage of pancreatic secretion in dogs and cats or by feeding alcohol to dogs and rats with and without temporary occlusion of the biliopancreatic duct or by surgically inducing ischaemia in the pancreas of the dogs. But, none of these models is identical with human disease. A consistently reproducible model of human chronic pancr-eatitis probably does not exist. In this expanding era of molecular biology which promises us to enhance greatly our understanding of this disease, a right experimental model of chronic pancreatitis is still in progress. |

| 抄録 | 組織のコラーゲン沈着にはコラーゲン合成系と分解系の不均衡によって生ずる｡従来,主としてコラーゲン合成系が注目されていたが,最近の研究の進歩により,コラーゲン分解系が重要な役割を演ずることが明らかになってきた｡コラーゲンの分解系には細胞内と細胞外の二つの経路が存在する｡それぞれcollagenolytic cathepsinおよびmatrix metalloproteinases( MMP)がコラーゲン分解能を有する重要な酵素である｡その調節因子については細胞外の経路についての解明か進んでいる｡MMPの遺伝子の発現にはサイトカインや成長因子が関与し,IL-1やTNF-αは強力な誘導因子である｡一旦,遺伝子か発現すれば,MMPは合成され,細胞外に不活性型(latent form)で分泌される｡不活性型のMMPが活性化する過程にはplasminogen activator inhibitorやtissue inhibitors of metalloproteinases(TIMP)などの阻害因子が存在し,MMP活性を調節する｡TIMPの遺伝子の発現にもサイトカインや成長因子が関与する｡MMPがTIMPを上回るような病態では組織破壊が,逆にTIMPがMMPを上回るような病態では綿維化が生ずる｡コラーゲン分解能の障害が線維化の維持や不可逆性に関与することが推察される。 |

| 抄録 | 膵癌の早期診断を行うために,最近の進歩の著しい遺伝子診断を用いて,膵液中のK-ras遺伝子の点突然変異の検討がなされている｡われわれは膵液の細胞診は陰性であるが,K-ras遺伝子の点突然変異を認め,膵全体に多発する膵嚢胞の1例を経験した｡本例は悪性であるとの確診が得られないことや切除するとなれば膵全摘となることなどのために,経過観察を行っているが,18カ月後の現在,嚢胞の増大など認めていない｡膵癌の遺伝子診断の文献的考察を含め,報告する｡ |
