We reported that taurine had an anti-fibrotic effect in a dibutyltin dichloride (DBTC)-chronic pancreatitis model. However, the effect of taurine on apoptosis of pancreatic acinar cells is still unclear. Therefore, we examined apoptosis in DBTC-chronic pancreatitis and in the AR42J pancreatic acinar cell line with/without taurine. Pancreatic fibrosis was induced by a single administration of DBTC. Rats were fed a taurine-containing diet or a normal diet and were sacrificed at day 5. The AR42J pancreatic acinar cell line was incubated with/without DBTC with taurine chloramines. Apoptosis was determined by using terminal deoxynucleotidyl transferase-mediated dUTP-digoxigenin nick end labeling (TUNEL) assay. The expression of Bad and Bcl-2 proteins in the AR42J cells lysates was detected by Western blot analysis. The apoptotic index of pancreatic acinar cells in DBTC-administered rats was significantly increased. Taurine treatment inhibited pancreatic fibrosis and apoptosis of acinar cells induced by DBTC. The number of TUNEL-positive cells in the AR42J pancreatic acinar cell lines was significantly increased by the addition of DBTC. Incubation with taurine chloramines ameliorated these changes. The objectives of this study were to clarify colon distributions on plain abdominal radiographs. Forty-three healthy volunteers underwent gastric fluoroscopy, and 2 hours later, plain abdominal radiography in the supine position. A region of interest (ROI) was defined uniformly on each X-ray image to divide the image into 600 zones. The area corresponding to the large bowel within the ROI was divided into 4 segments (ascending colon, transverse colon, descending colon, and sigmoid colon+rectum). The percentage of barium in each segment relative to the total volume of barium used was calculated to evaluate the percent ROI occupancy. The large bowel covered 76.7% of the entire ROI, with the percent duplication being 55%. The duplicated area corresponded to the transverse colon region. When the method proposed by Arhan et al. was used, the percentage of the colon actually present in each segment relative to that determined theoretically was 99.6% for the right colon segment, 92.2% for the left colon segment, and 92.2% for the sigmoid/rectal segment. However, in cases in which the transverse colon descended partially from the fifth lumbar vertebra, the percentage occupied by the sigmoid colon+rectum decreased to 57.2%. We applied a new large bowel segmentation method especially for patients with ptosis, by devising a line joining the lateral side of the right lesser pelvis and the lower ends of both sacroiliac Following results were obtained. 1) Hyponatremia and hypochloremia, decrease of urinary excretion of both electrolytes, increase of potassium-sodium ratio and increase of urinary excretion of potassium in accordance with decrease of urine volume were found during postoperative period in dogs who underwent operation under normothermia. However, increase of urine volume occurred on about 5th or 6th postoperative day resulting in increase of urinary excretion of sodium and chloride, and decrease of potassium excretion. Increase of water, sodium and chloride content of the tissue which had operative intervention, were found on the 4th postoperative day, though decrease of potassium was noted. 2) Decrease of serum sodium and chloride, and increase of potassium were noticed in hypothermic group, though these changes were not so significant as in nomothermic group. Urinary excretion of sodium and chloride increased in accordance with increase of urine volume and chloride-sodium ratio were diminished. However, changes after 5th to 6th postoperative day were same in both groups. There were found no significant changes in water and electrolytes content of tissue, which had operative intervention, on the 4th postoperative day in hypothermic group. Increase of water, sodium, and chloride in tissue were slight and no decrease of potassium was found. It was anticipated based on these findings that excretion of ADH and function of adrenal gland were suppressed resulting in increase of urinary excretion of sodium and chloride with less retention of water, sodium and chloride in tissue during postoperative period in operation performed under hypothermia. Following results were obtained. 1) Changes in serum sodium and calcium are not found, though potassium decreases and chloride increases in hypothermia. 2) No marked changes in sodium and potassium in erythrocytes are found. 3) Urinary excretion of sodium and chloride decrease, while potassium increases. No changes in excretion of calcium are noted. Total amount of electrolytes excreted in urine decrease because urine volume is markedly diminished in hypothermia below 28°C. 4) No significant changes in water content in tissue are found. There is found no changes in sodium of tissue. Chloride in tissue decreases, while potassium in liver increases though no changes are found in other organs. Calcium in tissue remains constant. Decrease of serum potassium and increase of chloride are remarkable changes in electrolytes in hypothermia. It is anticipated that deposition of potassium develops in liver because of decrease of glucose utilization due to low metabolism and decreased consumption of glycogen with resulting glycogen formation from excessive glucose which carries potassium from the extracellular fluid into the cells. The chest roentgenogram showed an enlargement of the cardiac silhouette. The electrocardiogram showed slight abnormalities including mild low voltage of R wave in V6 and flat T wave in V5 and V6. M-mode and cross-sectional echocardiography revealed a low echoic mass which mainly situated between left ventricular posterior wall and the pericardium. The left and right ventricular wall motion was not affected. Computed tomography of the heart revealed a large mass backward of the left ventricle and right side of the right atrium. The attenuation values of the mass ranged around-118HU and was compatible to lipoma. We diagnosed as a lipoma of the heart possibly arising from the pericardium. A palpable mass the size of an infant's head was tender on palpation and identified as an epigastric lesion. Colonoscopic examination revealed stenosis of the transverse colon, although no intraluminal growth of the tumor was found. The histologic findings of the biopsy material were poorly differentiated and/or undifferentiated cells. Abdominal CT scan showed an irregular-shaped tumor with a diameter of 10cm invading the stomach and jejunum. We performed an operation under a diagnosis of extramurally growing cancer or malignant lymphoma of the colon. Partial resection of the transverse colon was done by distal gastrectomy and partial resection of the jejunum. Histologic examination of the operative specimens revealed moderately differentiated adenocarcinoma of the transverse colon, prominently proliferating into the surrounding tissues. Serum samples were obtained from 201 HCV antibody-positive volunteer blood donors in and around the Myanmar city of Yangon. Of these, the antibody titers of 101 samples were checked by serial dilution using HCV antibody PA test II and Terasaki microplate as a low-cost method. To compare antibody titers by this method and RNA identification, we also checked HCV-RNA using the Amplicor 2.0 test. Most high-titer groups were positive for HCV-RNA. Of the 201 samples, 110 were successfully polymerase chain reaction (PCR) amplified. Among them, 35 (31.8%) were of genotype 1, 52 (47.3%) were of genotype 3, and 23 (20.9%) were of type 6 variants, and phylogenetic analysis of these type 6 variants revealed that 3 new type 6 subgroups exist in Myanmar. We named the subgroups M6-1, M6-2, and M6-3. M6-1 and M6-2 were relatively close to types 8 and 9, respectively. M6-3, though only found in one sample, was a brand-new subgroup. These subtypes were not seen in Vietnam, where type 6 group variants are widely spread. Gastrointestinal (GI) transit was also assessed by a noninvasive radioopaque marker method. Exocrine pancreatic function was evaluated by analyzing pure pancreatic juice and by analyzing the autonomic nervous system by cardiovascular reflex tests. Patients with CP showed impaired gallbladder contraction at 15 min and hastened gastric emptying. The cause of the former is unclear, whereas the latter was closely related with decreased pancreatic lipase output, but not with autonomic dysfunction. GI transit time did not differ between controls and patients with CP. In conclusion, we succeeded in clearly demonstrating impaired gallbladder contraction and hastened gastric emptying in patients with CP by a single noninvasive test, ultrasonography. Offensive odor of decoyed eels is similar to that of offensive odor fish caught in this sea and new peak considerd to be offensive odor substances was revealed in gas chromatogram with gas evaporated from offensive odor eels. This new peak is corresponded to tha peak with sea water and that with industrial waste. Degree of offensive odor and also offensive odor peak in gas chromatogram were scarcely found in the eels cultured in the sea at a very distanee of about 1Km. from the harbor. Two cases of multiple filling defects of the duodenal bulb are reported in this paper. Case 1. A 42 year old female complained of abdominal fullness and epigastralgia. On examination, generalized peripheral lymphadenopathy and marked hepatosplenomegaly were noted. Cervical lymphnode biopsy revealed a diffuse, poorly-differanciated lymphocytic lymphoma. X-ray examination of the gastrointestinal tract showed multiple filling defects localized at the duodenal bulb. Endoscopy disclosed multiple well-defined protrusions with small mucosal ulceration over the the whole circumference of the duodenal wall. Case 2. A 36 year old female had no subjective symptoms. At gastric mass screening, numerous polypoid elevations of the duodenal bulb were pointed out. Detailed gastrointestinal x-ray examination showed numerous, tiny, round polypoid nodules localized at the duodenal bulb. These nodules were 2-3mm in size with no mucosal ulceration. We have experienced four cases of pancreatic cysts in which post-ERCP CT was useful for diagnosis and in deciding on treatment plans. In addition, the need for an animal model of chronic pancreatitis is further strengthened by the relative inaccessibility and paucity of the human pancreatitis tissue. Whereas various models of acute pancreatitis and also of exocrine pancreatic tumor have been described, chronic pancr-eatitis has not been consistently reproduced in experimental animals. Many researchers attempted to establish an experimental model of chronic pancreatitis either by partially obstructing the drainage of pancreatic secretion in dogs and cats or by feeding alcohol to dogs and rats with and without temporary occlusion of the biliopancreatic duct or by surgically inducing ischaemia in the pancreas of the dogs. But, none of these models is identical with human disease. A consistently reproducible model of human chronic pancr-eatitis probably does not exist. This may occur as a consequence of alterations in the synthesis of collagen, their degradation, or both. Recent investigations revealed that a decrease in collagen degradation plays a crucial role in fibrogenesis. Two pathways exist in collagen degradation : extracellular and intracellular. Each pathway has an important enzyme; that is, matrix metalloproteinases (MMP) and collagenolytic cathepsin, respectively. Collagenolytic activity is regulated at several levels. Expression of MMP and tissue inhibitors of metalloproteinase (TIMP), which act as inhibitors of MMP, is regulated independently by a number of cytokines and growth factors. MMP, which is synthesized in the cell, is secreted in a latent form. Activation of the latent MMP is controlled by TIMP and plasminogen activator inhibitor. TIMP also inhibits activated MMP which can degrade connective tissue matrices including collagens. Mutation of K-ras oncogene at codon 12 has been reported in pancreatic adenoma, hyperplasia of the pancreatic duct, and also in pancreatic cancer, Recently, detection of K-ras point mutation in pure pancreatic juice is underinvestigation as a potential tool of early diagnosis of pancreatic cancer by setting a certain cut-off value. We recently experienced a case of multiple pancreatic cysts without any malignant cells in pancreatic juice, but with a positve point mutation of K-ras oncogene. Operation was deferred after obtaining informed consent from the family, because the lesions were so multiple and extensive as to require total pancreatectomy. Recently, there has been several reports suggesting that the cause of these disorders is postprandial dysmotility in the gallbladder and stomach. However, there has been no report suggesting incoordination of postprandial gastrobilialy motility as the cause of tha above mentioned complaints in these disorders. The aim of this study was to define the difference of postprandial gastrobiliary motility between patients with NUD and controls. Eight patients with NUD and 10 controls were studied. Gastric emptying time of liquid meal and gallbladder contraction were measured, simultaneously. There was no significant difference between study patients and controls when gastric emptying time and gallbladder contraction rate were compared in isolation. However, when these two parameters were assessed in combination, gastric emptying time was linearly correlated with minimum ballbladder contraction time in controls but not in patients. Blood samples were drawed before the preparation and after the colonoscopic examination and serum RBP was determined by laser nephrometory. Group A consisted of 10 patients who were administered intestinal lavage solution (Niflec) without no low residual diet. Group B consisted of 27 patients who were adminstered low residual diet for 1 day. Group C consisted of 27 patients who were administered low residual diet for the two consecutive days before colonosocopic examination with intestinal lavage solution and magnesium citrate because sufficient cleanup of intestine were not expected by using the ordinary preparations. The values of serum RBP significantly decreased in group C after the preparation although no significant changes were observed in group A and Group B. Firstly, intraductal secretin test by using this method can be substituted for duodenal secretin test, which is a gold standard examination for estimation of exocrine pancreatic function. In addition, endoscopic aspiration of protein pluge eliminates abdominal pain of some painful patients with chronic pancreatitis. A patient, 54 year-old-woman, presented to our clinic with abdominal pain and diarrhea. Esophagogastric endoscopy and biopsy were normal. Contrast barium enema examination and colonic endoscopy showed a stenotic segment in the transverce colon. Biopsy specimens obteind from the stenotic segment revealed moderate eosinophilic infiltration in the colonic mucosa. Oral intake of sea shells exacerbated her symptoms. Avoiding sea shells has resulted in the disappearance of symptoms and peripheral blood eosinophilia. Whole gut transit was measured by radiopaque markers method. Fifteen chronic pancreatitis patients (2 females, 13 males; age range 40-78 years) and 17 controls (4 females, 13 males, 32-73 years) were studied. Additionally, we also looked for evidence of autonomic neuropathy in the chronic pancreatitis patients by using cardiovascular tests. In chronic pancreatitis, whole gut transit was shorter than controls. These abnormalities were not influenced by the degree of autonomic neuropathy. We reviewed studies on free radicals in chronic pancreatitis in both c1inical and experimental aspects. Many clinical studies have provided circumstantial evidence for the close relation between free radicals and chronic pancreatitis. However, few experimental studies in animals on relation between free radicals and chronic pancreatitis have been reported, because adequate experimental models for elucidating a pathological role of free radicals in chronic pancreatitis have not been established. Histologically, infiltrations of IgG4-positive plasma cells and lymphocytes occur in the lacrimal gland, salivary gland , retroperitoneum , pancreas , and biliary tract. IgG4-related disease is a systematic disease presenting symptoms such as Mikulicz's disease, retroperitoneal fibrosis, autoimmune pancreatitis, diabetes and sclerotic cholangitis, and oral steroid therapy is effective. In this paper, we propose diagnostic criteria for IgG4-related disease that have not been established yet. Currently, serum total amylase, pancreatic isoamylase (P-amylase) , lipase , trypsin (ogen) , pancreatic phospholipase A(2) (PLA(2)) , and elastase I are utilized in diagnosing pancreatic diseases. We here compared age and gender-related alterations of these six pancreatic enzymes in healthy subjects to delineate different properties among the enzymes. Subjects were 155 males and 172 females between ages 20 and 79 years who were apparently healthy, and were stratified by age and sex. PLA(2) and elastase I were assayed by RIA, trypsin (ogen) by EIA and others by activity. The pancreatic enzymes, except PLA(2), were significantly elevated with age, although they declined in males in the 260 age group. There were significant sex differences in total amylase and P-amylase. Total amylase was significantly higher in females than in males in the 260 age group, P-amylase in the 40-49 age group. The experimental factors wewre the pen diameter (three levels:8mm, 11mm, and 13.8mm). The length of pen tip was fixed to 14mm. Surface EMG was recordes from extensor digitorum and flexxor digitorum superficialis. The EMG before and after a long-hour writing task was measured to evaluate the fatigue of forearm using % MVC (Mean Voluntary Contraction), mean power frequency (MPF) and psychological rating on usability. Concerning % MVC and MPF, the difference before and the experimental task were used for the evaluation. The evaluation value corresponded to subtraction of value after the experimental task from that before the experimental task. As a result of a one-way (pen diameter) ANOVA, no significant main effects of pen diameter were for both EMG evaluation measures. As for the psychological rating on usability, Kruscal-Wallis non-parametric test was carried out. The psychological rating on ease of grip revealed a significant main effect of pen diameter. Therefore, another mechanism for cytoplasmic beta- catenin accumulation might exist in ESCCs. Materials and Methods: Human ESCCs cell lines and the 293 stable transfectants expressing Wnt-1, Wnt-5A, and Wnt-7A were cultured under standard conditions. The TOPFlash or FOPFlash reporter plasmids were transfected. Results: Transfected mutant beta-catenin as well as an axin fragment harbing the GSK3 beta interaction domain, the latter a potent GSK3 beta inhibitor. both robustly activated pTOPFlash in ESCCs cells. When pTOPFlash/pFOPFlash reporters were transfected in ESCCs cell lines followed by cocultivation with 293 cells that stably express Wnt -1, all cell lines except one demonstrated TCF mediated transcriptional. But, cells were co- cultured, Wnt - 7A or Wnt - 5A did not activate TCF mediated transcription in a cell number dependent fashion. A marked difference was observed in seasonal fluctuation of mosquito population between the two sites of collection. During the period of observation, the incidence of Japanese encephalitis patients fluctuated in parallel with the average population of mosquitoes collected from June to September. The first incidnece of Japanese encephalitis patients in Kochi Prefecture generally starts from south-western Kochi Prefecture including Tosashimizu City and gradually moves north. There were 27(55%) steroid resistant patients who could not maintain platelet counts of 5×10(4)/μl with prednisolone doses of less than 10mg/day. Among the 16 steroid resistant patients treated with colchicine, there were 3 patients with complete remission and 3 others who had good to excellent response, that is, 6(37.5%) improved overall. This therapy was more effective than splenectomy (33.3%) and danazol (25%). Colchicine was did not effective in patients who did not respond to prednisolone, but was useful when patients showed at least a transient rise of the platelet count with prednisolone. In many cases, a response to colchicine was seen after a month, and in the patients who failed to show a rise in the platelet count after 2 months of colchicine therapy, no benefit was seen even when the therapy was continued. Of 208 questionnaires sent, 125 (60.1%) were returned. Sixty-seven (53.6%) of the respondents had experience traveled outside. People who participate in functional trainning for disability hope to have an attendant to accompany them when traveling. One primary factor that hinders traveling outside and using public transportation is that they cannot ask for help because of shyness. Another factor is the construction of entrances (e.g.,bumps) for vehicles which cannot be traversed easily. Reasons differed among those who had traveled outside and those who had not. Atomospheric SPM influences the local ecosystem including various plants and animals. It is necessary to examine the distribution characteristics of such trace metals in fine particulates (especially those with a diameter (φ) of less than 2.1μm). The Environment Agency of the Japanese goverment will complete analysis of the a concentration of suspended particulate matter less than 2.4μm by the end of 1999, and will be examining the relationship between the concentration of suspended particulate matter (0.1-9.0μm) with consideration given to the particle size throughout two years. It became clear that the metals of artificial origin such as Pb, V, Cd, Ni, Cr, existed in fine particulates (0.1 or more 2.0μm).
