The histopathological analysis of the biopsied specimen led to the diagnosis of duodenal-type follicular lymphoma. The lesion showed spontaneous improvement ; however, 14 months later a few small whitish granules were observed by esophagogastroduodenoscopy. Observation under magnification revealed slightly elevated opaque white spots and whitish granular protrusions of roughly the same size that were uniformly distributed in the duodenal bulb. The duodenal bulb lesions were histopathologically diagnosed as lymphoid hyperplasia. The patient also exhibited lymphangiectasia (which presented as scattered white spots) and lymphangioma (which presented as a soft submucosal tumor with white spots on the surface) in the second portion of the duodenum. This patient presented with four white lesions in the duodenum : duodenal-type follicular lymphoma, lymphoid hyperplasia, lymphangiectasia, and lymphangioma. en-abstract= kn-abstract=The stucture of the human lympl vessel wall was studied with a special reference to the muscular architecture, and also to the regional and individual differences. Twenty-eight preserved cadavers of Japanese adults and one fresh corpse were used. The specimens were prepared from them within the range macroscopically possible of obtaining necessary materials. Therefore, the smallest vessels were not involved in the present study. Besides ordinary histological procedures, film specimen method and graphical reconstruction were also applied. 1. The muscle bundles of the lymph vessel wall, ever changing their course, branching out, and anastomosing with one another, form a flat network (muscle bundle group or muscle plate). At the thicker portion, the muscular coat consists of multiple muscle plates. Their bundles intersect with one another obliquely and form diamond meshes, but there never interlace with one another like textile fabrics. Often a group of muscle bundles branches out from one muscle plate and form new musucle plate, the bundles of which are directed in another direction. Such communications of msucle bundles can be observed commonly in between each layer of the muscular coats (see below). 2. Elastica interna of the lymph vessel is poorly developed. It spreads out around the bundels of the inner longitudinal muscle, forming the elastic sheathes. Therefore, it is not necessarily of a single layer, but is often made up of a rather wide zone including longitudinal muscle bundles. 3. The muscles of the intima and the adventitia are thin, generally consisting of a single or double muscle plates running approximately longitudinally. The muscular coat of the media is thick. The muscle bundles rung enerally longitudinally in the inner layer and circularly in the outher layer with ones in transitional state in between. However, some of the vessels show all the muscle bundles of the media running longitudinally, while in others all circularly. The longitudinal muscle bundles of the intima and inner layer of the media, connecting with each other, constitute the inner longitudinal muscle of the lymph vessel. The course and obiqueness of the muscle bundles of the media are extremely irregular so that the coat offers variable patterns. 4. The regional differences in the structure of the lymph vessel are most markedly exhibited in the amount of the longitudinal and circular muscles. Furthermore, systematic differences are involved to a great extent in them, and local ones seem to be less significant. 5. Systematic differences. a) In the most peripheral lymph vessels the wall is thin, and contains only a small quantity of longitudinal muscle, suggesting the transformation to muscleless vessels. However, more proximally the wall becomes fairly thicker, the vessel showing considerable contraction. In this type the muscular coat is composed mostly of longitudinal bundles and only a few circular bundles can be recognized. Still nearer to the center the vessels are now made up of three muscular coats, namely, the inner logitudinal, the middle ciruclar and the outer longitudinal. As compared with the lumen the wall in rather thin, and contains less muscles, the contraction of the vessels growing weaker. According to these differences in the muscular architecture, author divided lymph vessels into five types. Of them the vessels consisted of three muscle coats seem to be designated as conducting lymph ducts. The thoracic duct and other lymph trunks as weel as vessels in the upper and lower limbs (excepting those in the periphery) and the chain of vessels ascending from the deep subinguinal nodes to the lumbar trunks belong to this category. We developed a method of direct cannulation into subepicardial lymph vessels, which allowed us to study regional myocardial lymph circulation. Control values for the lymph flow rate (LF) ranged from 5.3 to 10.6 μ 1/ml. Adenosine, serotonin, histamine, and isoproterenol increased LF and protein efflux in cardiac lymph (PEF), and decreased protein concentrations (PC) concomitantly with coronary blood flow (CBF) increments. The relative increase of LF to CBF change was significantly greater upon administration of histamine and isoproterenol than with adenosine and serotonin. PC in the lymph were decreased by adenosine and serotonin, while no significant changes in PC were seen upon histamine or isoproterenol infusion. These results indicate that increased water release was greater than any protein efflux into cardiac with increased CBF, and that histamine and isoproterenol can modify capillary permeability directly or by changes in myocardial metabolism. Old (90)YCl(3) colloid at pH 7.0 (20mμ diameter) and new type of a mixture of YCl(3) and (90)YCl(3) colloid at pH 7.0 (3-15μ diameter) were administered into peritoneal cavity of mice and rabbits. The distributions of administered (90)Y in serum and organs were compared. Also, these two colloids were examined by intralymphatic administration for clinical use, and the distributions were observed in serum and the excretion of urine. 1) (90)Y in peripheral blood by peritoneal administration of mice were higher in old(90)YCl(3) colloid than that of new type of YCl(3) and (90)YCl(3) mixture. The c. p. m. in peripheral blood were generally decreased by addition of YCl(3). Minimum distribution in peripheral blood was obtained by addition of 1mg/ml of YCl(3). 2) The same results were obtained from rabbit experiment. Old (90)YCl(3) colloid was 2 times of c. p. m. than that of new type mixture colloid at 7th day. (90)Y excretion in urine of old (90)Y colloid was 51 times than that of new type colloid after 3 hrs colloid administration. The distributions of old (90)YCl(3) colloid were spleen, kidney, liver, marrow, lung, heart muscle, and thigh, in the orders, 7 days after administration, and that of new type colloid were spleen, liver, marrow, lung, kidney, heart muscle, and thigh. The chief findings are as follows: 1. The mortality rate was almost the same in both cases, but the average survival period was shorter in the cases of injection into the efferent lymphatics than in the cases of injection into the afferent lymphatics. 2. Metastasis of cancer in the abdominal cavity occurred to a greater degree in the former cases than in the latter cases. 3. When carcinoma cells were injected into the efferent lymphatics, a retrograde metastasis of cartinoma was observed in the popliteal lymph nodes on the in ected side in two of four cases. 4. When BPCC were injected into the popliteal lymph nodes of normal rabbits which had not been sensitized with BPCC, the injected BPCC which surrived after injection into the nodes may proliferate rapidly and the nodes were replaced by proliferating BPCC nearely completely by the 14th day after injection. 2. In the lymph nodes in which an extensive proliferation of the injected BPCC had occurred, a large number of pyroninophilic blast cells appeared in the visinity of growing tumors. This is considered to be a response of lymph nodes characteristic of immune mechanisum against BPCC. 3. The chief results obtained are as follows: 1. In normal rabbits that had not been sensitized with CRBC, the injected CRBC underwent hemolysis and their nuclei were found diffusely scattered over the wide area of the popliteal nodes, particularly in the lumen of lymphatic sinuses until 2 hours after the injection 2. At 6 hours after the injection the nuclei of the injected CRBC were mostly phagocytized by the reticular cells in the lumen of lymphatic sinuses or other parts of the nodes. Thereafter, the nuclei of CRBC had been digested nearly complitely by the reticular cells and disappeared by the 12th hours after the injection. 3. In the sensitized rabbits that had been intravenously injected with CRBC two weeks ago, phogocytosis of the injected CRBC by the reticular cells occurred at 2 hours after the injection and CRBC disappeared nearly completely by the 6th hours, after having been digested by the reticular cells. 4. About 5 hours later the abdominal aorta was taken out, and it was cut into longitudinal and cross celloidin sections of about 100μ in thickness, which were stained with aldehyde-fuchsin or light-green solution and the distribution of lymph vessels in the arterial wall was observed. With some of the above specimens transparent samples were also prepared. By the forced reverse flow of lymph it was found that the excretory lymph vessel group arising from lymph capillary immediately adjacent to the internal elastic membrane of the aortic wall runs gradually along collagenous fibers through the fenestrations of each elastic membrane of the tunica media, and appears at the tunica adventitia and after confluencing they become a ralatively large excretory lymph vessel, but they further confluence with the excretory lymph vessel arising from the lymph capillary of fat tissue in the tunica adventitia, and they form an aggregative lymph vessel which runs over the exterior layer of the tunica adventitia, and when it comes out of the tunica adventitia it enters into the aortic lymph node. What is noteworthy is the fact that both the aggregative and excretory lymph vessels have peri-lymphvessel spaces. Embryologically, such a peri-lymphvessel space is a kind of closed space formed by the peri-arterialwall connective tissue consistig of double membrane that covers a part of the aortic lymph nodes to make an aggregative lymph vessel, and then excretory lymph vessel, which should be considered as a lymph space. The resulting cast was observed under an SEM. The outer surface of the cast, corresponding to the marginal sinuses, consisted of an arrangement of dome-like structures of about 1mm in diameter. A cross section of the cast showed that the marginal sinuses penetrate deeply into the cortex down to the trabecular and cortical sinuses, surrounding the secondary lymphoid nodules and connecting with the medullary sinuses. The cast of the diffuse lymphoid tissue showed numerous structures like tangled strings of beads, each bead with a maximum diameter of 40u and minimun of 10u. These were lymph pathways running through the reticulum cell network between the marginal sinuses and medullary ones. The cast of the primary lymphoid nodules showed a coral-like or irregular menbranous structure of 5u in thickness just under the dome-like marginal sinuses. Blind ending lymph vessels filled with resin were demonstrated at the center of the secondary lymphoid nodules. The cast of the medullary sinuses showed tortous tubes of 100-150u in diameter branching from or anastmosing with each other. The structures thus revealed were compared with those embedded in Epon by a light microscope. On the mucosal layer of the rat stomach, the lymphatic casts showed various structures, i.e., fusiform casts from the subepithelial lymphatics, a basket-shaped structure from the interglandular lymphatics and a horizontal network of leaf-like structures from the subglandular lymphatics. These casts also showed several nuclear depressions about 10 μ in diameter, a number of constrictions, and numerous small holes representing reticular or collagen fibers interrupting the Mercox filling. The submucosal lymphatic casts of the rat stomach showed irregular tubular or flat shapes about 20-40 μ in diameter, nuclear depressions about 15 μ in their long diameter with sawtoothed figures around them, bead-like swellings, and occasional V-shaped incisura on parts of the columns. Similar findings were also observed in the submucosal layer of the rabbit small intestine. On the mucosal layer of the rabbit small intestine, the central lacteal casts showed peculiar figures, i.e., a dome-shaped structure about 70-80 μ in diameter at the topmost portion, swelling to about 100-200 μ in diameter in the middle, and an evident constriction to 30-60 μ in diameter at the bottom. In initial invasion, lack of the basement membrane and cytoplasmic projections into the adjacent connective tissues was observed. The mode of penetration of cytoplasmic protrusion into the stroma was also studied in cases of benign cells. As a result, invasion by carcinoma cells was thought to occur with its ameboid and proteolytic enzymic action against the surrounding tissues. The invasive mode into the lymphatic vessels was divided into the following 2 types; a destructive invasion type, and an endothelium type. The influencee of ligation of the common bile duct appeared earlier in the thoracic-duct lymph than in the serum. The portal pressure did not increase in the dog with a ligated common bile duct, but the lymph flow of the thoracic duct was 3 times that before ligation, which would prevent the rapid stagnation of bile pigment in the liver. The portal pressure was not elevated after ligation of the hepatic artery. The cholesterol level in the thoracic-duct lymph was about half of that in the serum. In the dog with ligature of the common bile duct, the serum total cholesterol level was elevated with the elevation of serum bilirubin, whereas that in the lymph of the thoracic duct was not elevated with the elevation of the bilirubin level in the lymph. These findings suggested that cholesterol would not easily enter into the lymphatic route. The serum GPT level increased during the first week after ligation of the hepatic artery, and decreased thereafter. However, after ligation of the common bile duct, the GPT level increased for several weeks after the first week of ligation. The pattern of GPT by both ligations seemed to be cross-crossed. The marked change in the total bilirubin level in the lymph of the thoracic duct suggested its close relation with the lymphatic system.
