| 抄録 | Combined low-dose therapy of oral capecitabine (Xeloda) and cyclophosphamide (XC) has been demonstrated to be useful for long-term control of lesions in patients with metastatic breast cancer (MBC) and is aimed at symptomatic alleviation and prolongation of survival. Here, a retrospective review was conducted of MBC patients administered XC at the Okayama University Hospital (OUH), to evaluate responses to XC, adverse events and time to progression (TTP). Twenty patients with MBC received XC between 2006 and 2009. With the exception of 2 elderly patients who were over the age of 70 at the initial examination, all of the patients had received prior treatment with an anthracycline and/or a taxane. No complete response (CR) cases were observed, but partial response (PR) was achieved in 6 patients (30%) and SD in 9 (45%), of whom 5 (20%) sustained SD status for >12 months. The median TTP was 6 months (range:3-27 mo.). Three patients developed Grade 3 adverse events (diarrhea, nausea and stomatitis), but no other patients developed adverse reactions causing interruption of the therapy. XC was safe even in previously treated and elderly MBC patients;moreover, it yielded remarkable clinical responses. |

| 抄録 | We evaluated the efficacy and toxicity of cisplatin/docetaxel (CDDP/TXT) chemotherapy and identified prognostic factors in Japanese patients with cancer of unknown primary site (CUP). Twenty-eight consecutive patients seen at a single institute were reviewed retrospectively. Sixteen patients were treated with TXT 80mg/m2, followed by CDDP 75mg/m2. The overall response rate to CDDP/TXT treatment was 62.5%, with a median survival time (MST) of 22.7 months. Common adverse reactions were myelosuppression and hyponatremia. The MST of all 28 patients with CUP was 8.3 months, and the 1-year overall survival rate was 45.6%. Univariate analysis identified 5 prognostic factors:performance status, liver involvement, bone involvement, pleural involvement, and lymph node involvement. In conclusion, CDDP/TXT chemotherapy is effective with tolerable toxicity in patients with CUP. Japanese patients with CUP might be chemosensitive and may survive longer. |

| 抄録 | Two cases of chronic Folidol intoxication were described. Both patients complained of oppressive feeling in upper abdomen, breast and head, since exposure to Folidol-spray. Case 1. A farmer aged 21. Two months after Folidol-spraying, investigation revealed a slight fever, a slight hyperchromic anaemia with relative lymphocytosis, achylia gafotrica, a decreased concentration in bile with a few gall-sands and an occasional tenderness on gall-bladder. Bromsulfalein test slightly positive (8%). A hypoglycaemia, hypotension, high sensitivity to pilocarpine, vanished patellar and Achilles tendon reflexes and a positive Sawada test in urine were also observed. None of the treatments given showed a favourable result. Case 2. A farmer, aged 27. Three months after Folidol-spraying, investigation revealed a slight fever, a hypochromic anaemia and a decrease in serum-cholinesterase activity (40%). |

| 抄録 | As the method for the diagnosis of metastasis of breast cancer to internal mammary nodes, 198Au colloid was used in parasternal scintigram. In the internal mammary nodes, the rate of correct diagnosis was 90% in the cases where diagnosis of positive metastasis was obtained, 44. 5% in whom metastasis was suspected, and 100% in whom diagnosis of negative metastasis was made. In the subclavian nodes, corresponding values were 69.0%, 34.5%, and 100% indicating poorer diagnostic rate. Examination on the relationship between metastasis to the lymph nodes and scintigram revealed more intense spots in the lymph nodes with very small metastasis than on normal side. As the metastatic portion increases in size, spots become less intense until finally the spots are cold as the whole lymph nodes are occupied by the cancer tissue. As the site of the tumor not only the medial side but also lateral side are involved in Stage I in a high rate of lymph node metastasis. Need of complete removal of internal mammary nodes was urgently suggested. |
