| 抄録 | 高齢者喘息の病態的特徴を臨床的に観察した。その結果は以下のごとくであった。 1．血清IgE値や抗原に対するIgE抗体の陽性率は一般的に低いが，しかしアレルギー疾患の家族歴を有する症例の頻度は高く，アトピー性の素因を有する症例の頻度がかなり高いこと，そして，このような症例ではIgE系反応の関与が示唆された。 2．気道過敏性は加齢とともに低下する傾向が見られた。 3．換気機能，ガス交換能は加齢とともに低下し，特に70才以上 の症例では若青年者喘息と比べ有意の低下が見られた。 4．肺容積は，加齢とともに増大し，70才以上の症例では有意の増大傾向が見られた。また，この肺容積の増大は，HRCT上の－950HU以下の肺% UIAと有意の相関が見られ，肺の過膨張の状態が存在することが示唆された。 |

| 抄録 | The serum levels of some pancreatic enzymes have been reported to be affected by age and gender. Currently, serum total amylase, pancreatic isoamylase (P-amylase) , lipase , trypsin (ogen) , pancreatic phospholipase A(2) (PLA(2)) , and elastase I are utilized in diagnosing pancreatic diseases. We here compared age and gender-related alterations of these six pancreatic enzymes in healthy subjects to delineate different properties among the enzymes. Subjects were 155 males and 172 females between ages 20 and 79 years who were apparently healthy, and were stratified by age and sex. PLA(2) and elastase I were assayed by RIA, trypsin (ogen) by EIA and others by activity. The pancreatic enzymes, except PLA(2), were significantly elevated with age, although they declined in males in the 260 age group. There were significant sex differences in total amylase and P-amylase. Total amylase was significantly higher in females than in males in the 260 age group, P-amylase in the 40-49 age group. Age should be considered in the valuation of serum enzymes except PLA(2), sex difference should be considered in the valuation of amylase (total and P-amylase). |

| 抄録 | 新規ロイコトリエン（LT）受容体桔抗薬であるモンテルカスト服用によるテオフイリン服用患者の処方動向，臨床検査値ならびに患者の服用に対する意識について検討を行った。 その結果，テオフイリン服用患者177名の内，51％がLT受容体抵抗薬を併用しており，併用率はモンテルカストが最も多く，プランルカスとザフィルルカストの順であった。また，プランルカストからモンテルカストに変更された症例が多かった。臨床検査値はモンテルカスト服用により血中好酸球数の減少作用が認められた。肝機能検査値（AST・ALT・ALP）は影響しなかった。テオフイリン服用量はモンテルカスト併用服用により減量され，それに伴い血中濃度も低下した。しかしながら，血中濃度／投与量比は影響しなかったことより，モンテルカストはテオフイリンクリアランスに影響しないと考えられる。モンテルカストヘの変更により，患者の服薬コンプライアンスは良好に維持され，自覚する副作用の発現は認められ なかった。以上の結果，テオフイリン服用患者へのモンテルカスト併用投与は良好な服薬コンプライアンスを維持させ，さらにテオフイリンの体内動態に影響を及ぼすことなく，テオフイリンの投与量を減少させ，患者のQOLの向上につながると考えられる。 |

| 抄録 | 近年QOLが重視されるようになってきている。MOS short form 36 health survey（以下SF-36と略す）は，国際的レベルでの基準とされるべく開発された非疾患特異的HRQOL尺度である。今回，我々は当院でリハビリテーションをうけている腰痛症患者を対象に温泉療法のQOLに対する効果をSF－36を用いて調べた。SFL36の8項目をそれぞれ算出し，温泉療法前後での比較を行った。PCSは41.1から43.6へ, MCSは49.1から5l.0へ上昇したことより，身体・精神面ともに効果があると考えられた。 |
