| 抄録 | We sought to clarify the survival and neurodevelopmental outcomes of very low birth weight infants (VLBWIs) and to identify risk factors for death or neurodevelopmental impairment (NDI) in VLBWIs at our hospital. The total study population was 217 infants born in 2005-2012 weighing ≤1,500 g. We compared their outcomes with those from previous reports analyzed the causes of death. Risk factors for death after discharge or NDI were evaluated by a multivariate logistic regression analysis. The incidences of death or NDI reported revealed in this study and the database of Neonatal Research Network of Japan were 25.3% and 19.6% (p＝0.039), respectively. The main causes of death before discharge were intraventricular hemorrhage, sepsis, and persistent pulmonary hypertension of the newborn. The significant risk factors for death after discharge or NDI were early gestational age (weeks) and periventricular leukomalacia (adjusted odds ratio [95% confidence interval, p-value], 0.72 [0.54-0.94, 0.017] and 6.90 [1.35-38.25, 0.021], respectively). These factors must be addressed in order to improve treatment strategies for VLBWIs. |

| 抄録 | Facies analysis and analysis of stratigraphy together with folded structure of the Norian Jito Formation were studied in Jito Area, Kawakami, Okayama Pref., SW Japan. Results of regional mapping and measured sections reconstruct three-dimensionally distribution of the Jito Formation. The Jito Formation is sub-divided into the four new members; the Shimohira Muddy Sandstone Member, the Otofuji Coarse-grained Turbidite Member, the Misawa Fine-grained Turbidaite Member, and the Ide Conglomerate Sandstone Member in ascending order. The shimohira Muddy Sandstone Member is interpreted to be deposited in an inner-bay. The Otofuji Coarse-grained Turbidite Member and The Misawa Fine-grained Turbidite Member are considered to be deposited on a continental shelf. The Ide Conglomerate Sandstone Member is inferred to be a shallow marine deposit. As a result, the Jito Formation is regarded as one transgressive-regressive package. |

| 抄録 | Since the introduction of the seven-valent pneumococcal conjugate vaccine (PCV7) in 2007, invasive pneumococcal disease has declined, but the incidence of Streptococcus pneumoniae serotype 19A has risen worldwide. The present study examined changes in the features of invasive pneumococcal disease since the introduction of the PCV7 in Kochi, Japan. Pediatric cases of invasive pneumococcal disease were investigated before and after vaccine introduction (January 2008 to December 2013). Cases of invasive pneumococcal disease tended to decrease after PCV7 introduction. In addition, before introduction of the vaccine, most serotypes causing invasive pneumococcal disease were those included in the vaccine. However, after the introduction, we found cases infected by serotypes not covered by vaccine. Penicillin-resistant S. pneumoniae was the predominant serotype causing invasive pneumococcal disease before introduction of the PCV7, and the susceptibility of this serotype to antibiotics improved after vaccine introduction. Serotype isolates identified after vaccine introduction were also relatively susceptible to antibiotic therapy, but decreased susceptibility is expected. |

| 抄録 | We report a successful laparoscopic resection of a cecal tumor in a 95-year-old Japanese man. The patient visited an initial hospital with a complaint of constipation in March 2014. Computed tomography scan and colonoscopy showed a stenotic ileocecal cancer with pericolic lymph node metastases, and he was referred to our department for management. Since his general condition was maintained, we performed a laparoscopic ileocecal resection with regional lymph node dissection for the patient. The operation achieved curative resection, and the tumor was diagnosed as a moderately differentiated adenocarcinoma and graded as pStage IIIa (pT3, pN0, pM0) according to the Japanese Classification of Colorectal Carcinoma, eighth edition. He was discharged on the 11th postoperative day without perioperative complications. Several large-scale randomized controlled trials （RCTs） revealed that laparoscopic surgeries for colorectal cancers have some advantages compared to open surgeries, including superior short-term outcomes and comparable long-term outcomes. Unfortunately, since these RCTs did not include enough elderly patients, the safety and feasibility of laparoscopic surgery for extremely elderly patients are still unknown. With respect to less-invasive procedures, these advantages of laparoscopic surgery are also thought to be the advantages for elderly colorectal cancer patients. |

| 抄録 | Fall accidents are a common cause of femoral fracture in the elderly. The greater trochanter of the femur is often subjected to impact loading by a sideways fall, and thus it is worth studying the impulsive stress waves propagating in the femur. In this study, the impulsive stress was analyzed by the dynamic finite element method using a 3-dimensional model of the femur, and the influence of the fall configuration on the stress was discussed. The stress was concentrated around the femoral neck during the propagation of the stress wave, and the tensile maximum principal stress changed into compressive minimum principle stress on the anterior and medial sides of the neck. On the other hand, the compressive minimum principal stress changed into tensile maximum principle stress on the lateral side of the neck. The largest maximum principal stress during the impact loading was always larger in the neck than in the impact region. The largest absolute value of the minimum principal stress was found in the neck or the impact region depending on the fall configuration. The largest absolute values of the maximum and minimum principal stress were nearly equal, indicating that the bone fracture due to the tensile stress may occur around the femoral neck. |

| 抄録 | A 49-year-old Japanese man visited our hospital with chief complaints of difficulty with and bleeding during defecation. After a detailed examination, he was diagnosed with familial adenomatous polyposis (FAP) with multiple (five) colorectal cancers. The tumors were located in the right-sided, left-sided, and sigmoid colon, and the lower rectum. Regional lymph node involvement was observed, but no metastasis to other organs was detected. We, therefore, performed a laparoscopic total colectomy with superior mesenteric artery (SMA) and inferior mesenteric artery (IMA) lymph node dissection. We were able to perform minimally invasive and cosmetically acceptable surgery using laparoscopy instead of highly-invasive open abdominal surgery. Our search of the literature revealed no reported cases of laparoscopic total colectomy with lymph node dissection for FAP with multiple colorectal cancers, making the present case the first to be reported in the literature. |

| 抄録 | Preliminary analyses on the seasonal cycles of the large-scale atmospheric mean fields and the cyclone activity in Europe were performed based mainly on the NCEP/NCAR re-analysis data, at the viewpoint comparing with those in East Asia. As for the climatological monthly mean fields (1981~2010), the meridional temperature gradient in East Asia presents striking seasonal change with the maximum in winter and the minimum in summer. It is interesting that such transition from winter to summer and that from summer to winter occur rather rapidly accompanied by the great temperature increase around April to June and the decrease around October to November, respectively, in a wide region of 40~70N. On the other hand, the temperature gradient in Europe is significantly smaller with slight seasonal change than that in East Asian winter. By the way, in spite of the above difference of the horizontal temperature gradient between Europe and East Asia, the cyclone activity does not seem to be so weak even around Europe. Thus, the seasonal cycles of the characteristics of the cyclones and their environmental fields around Europe were examined. At the first step, we analyzed the daily cyclone activity in Europe for January and July of 2000. Many low pressure centers at the surface level appeared in both January and July. Referring also to the daily maps of sea level pressure and geopotential height at 500 hPa level, we found that not only the cyclones corresponding to the baroclinic instability waves, but also to the cold vortices which amplitudes increased with height appeared frequently. Moreover, it is interesting that the large-scale cold vortex, which embeds the shortwave trough with the low center at the surface level, appeared in the northern Europe in July as in East Asian winter, when such multi-scale systems sometimes bring the persisting heavy snowfall events in the Japan Sea side of the Japan Islands. |
