Post-transplant cyclophosphamide (PTCy) is now being increasingly applied to HLA-matched donor (MD) transplantation. Prior studies in Western countries have demonstrated that allogeneic hematopoietic cell transplantation (allo-HCT) employing PTCy yields better outcomes with HLA-matched donors (MDs) than with haploidentical donors (HIDs). However, the effect of HLA mismatch may differ among racial groups. We retrospectively analyzed adult patients with hematological malignancies who underwent their first allo-HCT with PTCy from MDs or HIDs registered to the Japanese registry database between 2013 and 2021. Among 63 (related, n = 33; unrelated, n = 30) and 1261 patients who received MD and HID allo-HCT with PTCy, 50 (related, n = 30; unrelated, n = 20) and 100 patients were assigned to MD and HID groups by 1:2 propensity score matching (PSM). The results showed that MD recipients had better neutrophil recovery (hazard ratio [HR], 1.48; 95% confidence interval [CI], 1.04–2.10; P = 0.031) and lower risk of non-relapse mortality (NRM) (HR, 0.19; 95% CI, 0.05–0.81; P = 0.024) than HID recipients. Multivariable analyses in the entire cohort before PSM confirmed these findings. Fatal infection was the primary cause of NRM in the HID group. This study is the first to demonstrate that, within a homogeneous Asian cohort, MD may have an advantage over HID in PTCy-based allo-HCT in facilitating neutrophil engraftment and reducing the risk of NRM.

Patients with hepatocellular carcinoma (HCC) and extensive peritoneal dissemination generally have a poor prognosis and are often resistant to systemic therapy. We report the case of a 47-year-old woman with HCC and massive peritoneal dissemination who presented with malignant ascites requiring repeated cell-free and concentrated ascites reinfusion therapy and peritoneovenous shunt placement, as well as malignant pleural effusion requiring pleurodesis. Combined immunotherapy with durvalumab/tremelimumab was initiated;however, disease progression was observed after three treatment courses, prompting a switch to lenvatinib therapy. Two months after initiation of lenvatinib, CT imaging demonstrated complete disappearance of arterial enhancement in the primary hepatic lesion, along with reduction in the size of peritoneal dissemination nodules. Thirteen months after switching to lenvatinib (16 months after the initial diagnosis), the alpha-fetoprotein level continued to decrease, and the disease remained stable under treatment. Despite the extremely high tumor burden, lenvatinib achieved disease stabilization and symptomatic improvement.

Background Idiopathic multicentric Castleman disease (iMCD) is a rare polyclonal lymphoproliferative disorder characterised by systemic inflammation resulting from overproduction of interleukin 6 (IL-6). While iMCD primarily affects the lymph nodes and related tissues, it can also rarely involve the central nervous system.<br>
Case presentation We report the case of a 58-year-old female patient with at least a 3-year history of iMCD, who experienced acute thunderclap headaches due to reversible cerebral vasoconstriction syndrome (RCVS). RCVS occurred 3 months after initiating treatment with tocilizumab, a humanised anti-IL-6 receptor monoclonal antibody, and was accompanied by focal cortical subarachnoid haemorrhage (SAH). Elevated IL-6 levels were found in both serum and cerebrospinal fluid. MR angiography revealed multiple diffuse stenotic lesions in the bilateral middle and posterior cerebral arteries, which, along with bilateral cerebral oedema, resolved within 3 months. The diffuse nature of the cerebral vasospasm and the presence of bilateral brain oedema suggested that cerebral vasospasm was due to RCVS rather than SAH.<br>
Conclusions In patients with Castleman disease, RCVS may occur due to IL-6-dependent chronic cerebral vascular inflammation, either as a primary condition or as a complication of tocilizumab treatment.

A 69-year-old Japanese man with acute leukemia received post-transplant cyclophosphamide-based haploidentical stem cell transplantation (PTCY-haplo-SCT) but was readmitted with dyspnea and ground-glass-opacities of the lungs. Bronchoscopy showed inflammatory changes with no signs of infection. He received steroids but required intubation as his condition deteriorated. In addition to antithymocyte globulin and cyclophosphamide, we administered ruxolitinib but failed to save him. Autopsy findings revealed fibrotic nonspecific interstitial pneumonia (NSIP) without evidence of organizing pneumonia or infection. Thus, we diagnosed idiopathic pneumonia syndrome (IPS). As far as our knowledge, this is the first case of IPS with NSIP histology after PTCY-haplo-SCT.

Vaccine-preventable disease (VPD) infections are more severe in immunocompromised hosts. Vaccination against measles, mumps, rubella, and varicella zoster (VZV) (MMRV) is therefore recommended for hematopoietic stem cell transplantation (HCT) recipients. However, studies on adult HCT recipients with VPD infections are limited. At our institution, we have systematically conducted serological MMRV tests as a part of check-up examinations during long-term follow-up (LTFU) after HCT since 2015. This retrospective study aimed to evaluate changes in the serostatus between before and 2 years after allogeneic HCT. Among 161 patients, the pre-transplant seropositivity was 82.7% for measles, 86.8% for mumps, 84.2% for rubella, and 94.3% for VZV. Among 56 patients who underwent LTFU including serological MMRV tests at 2 years after HCT, the percentages maintaining seroprotective antibody levels for measles, mumps, rubella and VZV were 71.5% (40/56), 51.8% (29/56), 48.2% (27/56), and 60.7% (34/56), respectively. Vaccination was recommended for 22 patients, and 12 were vaccinated. Among the 12 vaccinated patients, rates of seroconversion were examined in 2-6 patients for each of the four viruses. They were 100% (3/3) for measles, 33.3% (1/3) for mumps, 50% (3/6) for rubella, and 0% (0/2) for VZV. Further studies are warranted to clarify the effect of vaccination in adult HCT recipients.

The aim of this study was to investigate the awareness and experience, among dental practitioners, of adverse events resulting from dental treatment of patients undergoing therapy with drugs that affect the immune system [angiogenesis inhibitors, biological agents, immunosuppressants, and disease-modifying anti-rheumatic drugs (DMARDs)]. For this purpose, a nationwide questionnaire survey was conducted. Questionnaires were sent to 2,050 dentists, of which 206 (10.1%) were completed and returned. The results showed that most dentists were aware of complications associated with dental treatment of patients treated with drugs that affect the immune system, and about half had actually experienced such complications. Delayed wound healing, osteonecrosis of the jaw (ONJ), and postoperative infections were reported. Whereas approximately 50% of dentists did not discontinue the drugs during dental treatment, about 18% did. During temporary drug discontinuation, some patients experienced aggravation of the primary disease, such as worsening of rheumatism, growth of tumors, and rejection reactions of transplanted organs. As for medical cooperation, only less than half of the dentists were asked for oral hygiene management by a physician prior to starting the drug treatment. Prospective studies are needed because evidence for dental treatments in patients treated with these drugs remains limited.

Cystatin C-guided vancomycin (VCM) dosing is useful in critically ill patients. Its usefulness in septic patients with bacterial meningitis remains unknown, as there are no published reports. In this study, we sought to clarify its benefit. Cystatin C was used to guide VCM dosing in a septic bacterial meningitis patient with normal kidney function, according to therapeutic drug monitoring (TDM). Using cystatin C, the Bayesian method-based TDM made optimal VCM dosing possible, and decreased the predicted error (4.85 mg/L) compared to serum creatinine (16.83 mg/L). We concluded TDM of VCM using cystatin C can be considered in sepsis patients with bacterial meningitis with normal kidney function.

Background<br/>
The locomotive syndrome risk test was developed to quantify the decrease in mobility among adults, which could eventually lead to disability. The purpose of this study was to establish reference values for the locomotive syndrome risk test for adults and investigate the influence of age and sex.<br/>
Methods<br/>
We analyzed 8681 independent community dwellers (3607 men, 5074 women). Data pertaining to locomotive syndrome risk test (the two-step test, the stand-up test, and the 25-question geriatric locomotive function scale [GLFS-25]) scores were collected from seven administrative areas of Japan.<br/>
Results<br/>
The reference values of the three test scores were generated and all three test scores gradually decreased among young-to-middle-aged individuals and rapidly decreased in individuals aged over 60 years. The stand-up test score began decreasing significantly from the age of 30 years. The trajectories of decrease in the two-step test score with age was slightly different between men and women especially among the middle-aged individuals. The two physical test scores were more sensitive to aging than the self-reported test score.<br/>
Conclusion<br/>
The reference values generated in this study could be employed to determine whether an individual has mobility comparable to independent community dwellers of the same age and sex.

Acid mine drainage (AMD) is acidic and generally enriched with iron, aluminum, sulfate and heavy metals, such as lead and cadmium. AMD is a growing problem of emerging concern that cause detrimental effects to the environment and living organisms. Yanahara mine in Misaki Town, Okayama, Japan, had mainly produced pyrite for sulfuric acid manufacture. Although it was closed in 1991, AMD is being generated from the mine now. A passive treatment based on the biological oxidation of ferrous iron is a promising strategy for AMD remediation. AMD from Yanahara mine is treated in a plant using iron-oxidizing bacteria, Acidithiobacillus ferrooxidans and Ferrovum spp. The AMD generation continues for several centuries with dramatic consequences on the receiving environments. Therefore, the development of sustainable and cost effective treatment process is required. A development of the effective biological treatment process with an iron oxidation reactor operated at pH 3.5 is described in this report. Economic aspects are also discussed.

Acidithiobacillus ferrooxidans is one of the most widely used microorganisms in bioleaching operations to recover copper from low-grade copper sulfide. This bacterium uses ferrous iron and reduced inorganic sulfur compounds (RISCs) as energy sources. Transcriptions of genes thought to be involved in the oxidation of RISCs have been known to be highly activated in A. ferrooxidans cells grown on RISCs, while transcriptions of genes involved in the iron oxidation were repressed in the cells grown on
RISCs. A gene encoding a putative chromosome segregation and condensation protein (ScpB) with a
helix-turn-helix motif was found in the upstream region of sulfide : quinone oxidoreductase gene, whose expression was up-regulated in cells grown in sulfur and tetrathionate. A semi-quantitative PCR analysis using cDNA prepared from iron-, sulfur-, or tetrathionate-grown cells revealed that the transcription of scpB was up-regulated in cells grown on sulfur or tetrathionate as the energy source. Electrophoretic mobility shift assays were employed to examine whether the ScpB functions as a transcription factor. The result indicated that the recombinant His-tagged ScpB protein was able to nonspecifically bind in
vitro to DNA. This is the first report on a direct association of ScpB with DNA.

An OmpA family protein (FopA) previously reported as one of the major outer membrane proteins of an acidophilic iron-oxidizing bacterium Acidithiobacillus ferrooxidans was characterized with emphasis on the modification by heat and the interaction with peptidoglycan. A 30-kDa band corresponding to the FopA protein was detected in outer membrane proteins extracted at 75A degrees C or heated to 100A degrees C for 10 min prior to sodium dodecyl sulfate-polyacrylamide gel electrophoresis (SDS-PAGE). However, the band was not detected in outer membrane proteins extracted at a parts per thousand currency sign40A degrees C and without boiling prior to electrophoresis. By Western blot analysis using the polyclonal antibody against the recombinant FopA, FopA was detected as bands with apparent molecular masses of 30 and 90 kDa, suggesting that FopA existed as an oligomeric form in the outer membrane of A. ferrooxidans. Although the fopA gene with a sequence encoding the signal peptide was successfully expressed in the outer membrane of Escherichia coli, the recombinant FopA existed as a monomer in the outer membrane of E. coli. FopA was detected in peptidoglycan-associated proteins from A. ferrooxidans. The recombinant FopA also showed the peptidoglycan-binding activity.

Anti-anaphylactic effect of sinomenine was studied in guinea-pigs sensitized with bovine serum. Fairly constant severity of shock was obtained in the control animals by determining challenging dose based on Prof. Ogata's so-called binding zone: the highest mutiplication of the antigen giving positive reaction with the highest dilution of the antibody. Standard dose for challenge was calculated by the formula. body weight (g)×1/13×1/binding zone. Sinomenine was effective in preventing the guinea-pig's anaphylactic shock, but not so remarkably. Five daily subcutaneous injections of 50mg/kg sinomenine were required to prevent death from the active anaphylactic shock induced by the standard antigen dose. Under this pretreatment only about half number of the animals have survived death from the passive anaphylaxis shocked by the same antigen dose. Therefore, sinomenine seemed to be less effective in preventing guinea-pig anaphylaxis than canine anaphylaxis, against which marked protection was observed by one of us (R. N.) In the guinea-pig injected sinomenine 50mg/kg for 5 consecutive days, histamine content of skin of the head region was reduced by ca. 26%, but no reduction was observed in the abdominal skin and lung tissues. This degree of histamine release was much less than in the dog. The guinea-pig became more susceptible to histamine after the repeated injection of sinomenine, but this phenomenon is not specific to this species. Sinomenine did not inhibit the precipitin reaction in the guinea-pig anaphylaxis. From these, it may be concluded that the protective effect of sinomenine against guinea-pig anaphylaxis is less marked than against canine anaphylaxis and this is chiefly due to lesser ability of sinomenine in releasing histamine in this species.

With a view to explore and make a comparative study of various drugs as regards their activity in liberating skin histamine a quantitative determination method of in vitro histamine release from chopped dog skin has been devised and a study on the conditions influencing histamine release has been carried out. Abdominal skin was removed from dog and cut in a fixed thickness by a chopper of my own contrivance, consisting of razor blades placed at given spaces and after eliminating the majority of histamine spontaneously diffused out by immersing in Tyrode solution, the skin pieces were shaked in a glass tube containing drug solution to be tested along with a glass bead for one hour at 37°C, and then the histamine content diffusing out in the drug solution and residual histamine in the tissue were determined separately. The histamine releasing activity of drugs depends on the thickness of tissue pieces, temperature, and pH of the solution. In the case of thinner tissue pieces the rate of histamine release either by releaser solution or of spontaneous release proved to be greater. Below 45°C, the rise in temperature enhanced the release by a releaser more markedly than spontaneous release. At 60°C, however, all histamine in tissues diffused out within 60 minutes in the Tyrode solution not containing releaser. In the isotonic buffer solution of pH 6-8 spontaneous release proved to be least, while at the pH either more acidic or basic the rate of release increased. On the alkaline side the effect of releaser of alkaloid salt increased to a greater degree than spontaneous release. This fact suggests that the penetration of alkaloids into the cell wall takes place readily in the form of base. When the tissue is acted upon by the Tyrode solution or by 0.9% saline solution of 78 drugs tested, at 37°C for 60 minutes, these listed in the following released histamine from chopped skin rather markedly: Sinomenine, quinine, quinidine, cinchonine, cinchonidine, optochine, quinoline, ethylmorphine, thebaine, dibucaine, tropacocaine, betaeucaine, sod. cholate, sod. taurocholate, sod. desoxycholate, Irgapyrine, dibenamine, Priscol, jalapa resin, ethylalcohol, acetone. HgCl(2), phenol, thymol, alkylresorcinol, oil of sinapis, oil of chenopodium and ichthammol.
On the other hand, salicylic acid, acetylsalicylic acid and cocaine·HCl, under acidic condition, rather suppressed spontaneous release of histamine. The rates of histamine release from dog skin shown by these drugs were found on the whole to be higher than those obtained by Kamimura (Folia pharmacol. japon., 53, 836, 1957) of our laboratory using guinea-pig lung tissues. On the basis of these findings, the significance of histamine release in relation to symptoms of the so-called drug allergy or in the known pharmacological actions of certain drugs were discussed.

The lymphagogue effect of extract of lobster muscle, which is one of the first class of HEIDENHAIN'S lymphagogues, is essentially the action of adenine nucleotides liberated from the tissues by the injection of this extract. And also it is proven that these nucleotides are liberated in a larger quantity from organs in other parts of the body rather than from viscera. This conclusion is based on the following evidences. 1) Systemic-venous injection of the extract accelerate the outflow of a uncoagulable, highly proteinized lymph from the thoracic duct of dog, accompanied by a fall in arterial blood pressure and a rise in portal pressure. All these phenomena are also observable by the intravenous injection of adenosine triphosphate (ATP). 2) Repeated injections weaken the reactions to this extract (Tachyphylaxis). 3) Immediately after vascular administration of the extract adenine nucleotides including ATP are detected in blood plasma spectrophotometrically as well as paperchromatographically. Quantitatively the content in nucleotides is higher in the plasma from the femoral vein after systemic-venous injection than from the hepatic vein after portal injection. 4) No definite increase of histamine in blood can be recognized after systemic-venous injection of the extract.

Intravenous injection of 2% normal saline solution of Congo red in a dosage of 5 cc/kg in a dog resulted in marked increase of lymph flow from the thoracic duct over several hours and the lymph was apparently less coagulable after injection of the dye. In a majority of cases, arterial pressure showed only a slight fall by this injection, while portal pressure showed a marked elevation accompanied with an increase of liver volume. Such changes in the flow of lymph and circulation could not be reproduced by the injection of a saline solution isosmotic with the dye solution. Appearance of a fairly large amount of hemoglobin in the plasma and lymph after injection of the dye solution was observed by spectrophotometer. This hemolysis was completely protected by the previous injection of heparin into the vein before injection of the dye solution but the increased flow of lymph and elevation of portal pressure effected by the dye solution could not be modified. In a dog administered with heparin to remove the effect of hemolyis, injection of the dye solution was observed to cause increase in refractive index of the lymph and decrease of the same of the plasma. This is an indication that a large amount of plasma protein had filtrated into the lymph. After injection of the dye solution, histamine content in the lymph was practically unchanged. Antihistamines failed to suppress the increased flow of the lymph and elevation of portal pressure. In a non-heparinized dog, appearance of a substance or substances assumed to be adenine nucleotides was observed in the plasma by spectrophotometry and paper chromatography immediately after injection of the dye solution. It was concluded from these results that the increased flow of the thoracic lymph caused by Congo red is related to congestion of blood in the liver and other organs in the abdominal cavity, and further it was deduced that the cause common to these phenomena is not blood coagulation and liberation of toxic substances (including adenine nucleotides) from blood cells into plasma, which are considered to be the cause of acute dye shock, but is some kind of injuries to peripheral vessels, especially to vascular system in the abdominal organs.

<p>We determined plasma human atrial natriuretic peptide (hANP) levels in normal pregnancy and pregnancy induced hypertension (PIH). The plasma hANP levels slightly decreased in the first trimester of normal pregnancy and tended to recover as pregnancy advanced, although these changes were slight. However, the plasma hANP level in puerperium was higher than that in the third trimester of normal pregnancy. The plasma hANP level in mild PIH was not significantly higher than that in the third trimester of normal pregnancy. In contrast, the plasma hANP level in three cases of severe PIH was approximately 200% higher than those in the normal third trimester and mild PIH.</p>

<p>Previously, we pointed out the importance of the kynurenine metabolism in fetuses and neonates. We examined localization and developmental change of indoleamine 2,3-dioxygenase activity in human placenta. The indoleamine 2,3-dioxygenase was found localized in syncytiotrophoblast in the placenta. The indoleamine 2,3-dioxygenase activity was not detected in placenta in the early stage of gestation. It was first detected at around 14 weeks of gestation, increased rapidly thereafter and was maintained at high levels till near term. The indoleamine 2,3-dioxygenase activity was significantly lower in placenta with retarded intrauterine development. These results suggest the importance of placental indoleamine 2,3-dioxygenase during fetal development.&#60;/P&#62;</p>

<p>Concentrations of tryptophan (free and protein bound) and its metabolites in plasma of maternal vein at delivery, umbilical vein, umbilical artery, neonatal vein and breast milk were determined by high performance liquid chromatography. The plasma levels of tryptophan and most of its metabolites in umbilical vein and artery were significantly higher than those in maternal vein. The concentration of total tryptophan in plasma of neonatal vein showed marked decrease at 24 h after birth in comparison with that at birth, but the total kynurenine concentration was not decreased in plasma of neonatal vein. The colostrum contained a high level of total tryptophan. There were high ratios of free to total tryptophan in colostrum, transitional and mature milk. In the blood, ratios of free to total of tryptophan and kynurenine were kept at constant level throughout the perinatal period.</p>

Also similar analyses were conducted with blood aspirated from the median cubital vein of a woman during menstruation as well as from a man as the control, also with extracts of lochial blood
from a woman after normal delivery, and of the blood obtained at arrtificial abortion. Animal fibrins (from rabbit, mouse, steer, and guinea pig) were also used to see the lytic action of the bloods. The following are the results of the present experiments. 1. The identification of menstrual blood by means of paper electrophoresis
is a simple method in legal medicine and its electrophorogram is an excellent method to offer an evidence of proof for mentrual blood. 2. By this method it is possible to identify the menstrual bloodstain even
after the lapse of time as much as 6 months. 3. It is possible to identify even putrefied menstrual bloodstain.
4. In the case where the material stained with menstrual blood is found in water, it is not possible to identify the menstrual blood by this method. 5. When the menstrual blood is heated at 60°C over 30 minutes, it becomes
impossible to identify it by this method. 6. In the case of venous blood during menstruation fibrinolytic product
can be detected only on the first day of menstruation, but since it appears only in trace, it is easy to differentiate it from menstrual blood. 7. As for lochial blood the fibrinolytic product can be detected only in the
blood obtained on the first and second days of puerperium, but the amount being so slight that it can readily be distinguished from menstrual blood. 8. In the case of the blood obtained at artificial abortion fibrinolytic product appears just as much as in the case of menstrual blood, and thus it is impossible to differentiate it from mentrual blood by this method. 9. As for the use of human fibrin it is best to employ it while it isdresh, but the human fibrin up to 6 days old can be used. However, the older is the human fibrin the lesser the fibrinolytic product detectable. 10. In the case using animal fibrins mixed with the extract of menstrual blood some do produce fibrinolytic product in trace, but since there is a danger
of also producing the fibrinolytic product-like substance in venous blood, it is advisable not to use animal fibrins.</p>

<p>Experiment on the animals proved that in the case of the death by drowning planktons always immigrate into the bone marrow and some kinds of them can be detected in the bone marrow even after a long period of time, suggesting that the detection of these planktons in the bone marrow of the dead person will give the important clue for the determination of the cause of death by drowning.
Actually applying this method in a decayed corpse, we could successfully show the cause of death is due to drowning in which the cause of death was long argued in the court.</p>

The authors have experimented to detect the planktons from the drowned dead bodies under the special conditions. Rabbits were thrown into the ditch water after they were injured the carotid artery, during the terminal breathing by strangulation and concussion of the brain. It was tried to detect the planktons after the incinaration of organs and bones. Results are following; 1) Planktons could not be detected from organs and bones of rabbits that were put the ditch water into stomach with catheterization. 2) Planktons could be detected from the organs and bones of rabbits that were drowned to death after injured to the carotid artery, but longer the time after injured, lesser the plankton can be detected. 3) Excepting lung, there were no planktons in the organs and bones of the rabbits that were thrown into the river after death by bleeding. 4) From the organs and bones of the rabbits that were thrown into the river under standstill of therminal breathing by strangulation, several plankton could be detected though in small number. 5) Planktons could be detected from organs and bones of the rabbits that were drowned to death in the river under concussion of the brain.

The study was performed by EMG. record of Parkinsonsism tremor and tremor-like movement of normal subject imitate to Parkinsonism tremor. H-reflex technique by electric stimulation on the posterior tibial nerve was combined. In grouping voltage of Parkinsonsism tremor as well as in voluntary tremor-like movement of normal subject, double discharge of the single NMU. was observed generally with a interval of 25 to 60 millisecond (ms.), and the double discharging unit was considered to be a kinetic NMU. H-reflex records in series by relatively long interval (low repetition) stimulation showed constant amplitude of H-reflex. But series by relatively short interval (high repetition) stimulation showed irregular amplitude, and by shorter than 30ms. interval stimulation H-reflex default at random. At some critical conditien e.g. 68ms. interval at gastrocnemius muscle, 95ms. at soleus mnscle the series of H-reflex showed two kind of amplitude., higher amplitude H-reflex and lower amplitude H-reflex appeared in each othor. Depending on the amplitude of repetitive M-reflex will not fluctuate even at short interval such as 20ms. as already described on other literatures, the auther would conclude that above mentioned fluctuation of H-reflex amplitude was caused by the character of the afterhyperpolarization of motoneuron. The paticular phenomen of amplitude fluctuation of H-reflex which appeared as higher amplitnde and lower amplitude in each other would be considered due to mixing existance of two kinds neuromuscular units those have shorter and longer hyperpolarization time, and those two kinds of units would adequate to kinetic and tonic neuromusclarunits.

Serial records of H-reflex of splnal motoneuron were obtained by the recording of action potential of soleus muscle, triggered by conditioning and testing electrical shocks on the posterior tibial nerve. The fascilitation and recovery process of the spinal motoneuron was studied by the serial records of the H-reflex. The clinical material was 30 cases including normal subjects, patients with pyramidal lesion, and patients in acute stage of cerebrel
intervantion. In commonly, the H-reflex triggered by test shock (H) was not observed by the test shock which was given within 3 millisecond (ms') following conditioning shock, but start to appeared at 4ms appeared in peak amplitude (maximal deflection) at 6ms., and disappeared by 9 to 13ms. Later than 9 to 13ms., H-reflex falled in a silent period. However, the detail of the abovementioned characteristics of the H-reflex appeared in some difference, i.e. on the patient with pyramidal lesion H-reflex continued to appear until 18ms. with maximal deflection of 150% of the H-reflex, on the normal subject those were 10ms. and on the patient in acute stadium of cerebral surgical intervention those were 9ms. and 50% . The author intended to describe the complate disappearance of H-reflex within 3ms. following conditioning shock as the first silent period, and presumed that the first silent
period would be the result of combination of refractory period of Group Ia fiber and inhibition of Group Ib fiber. The auther also would suppose the detailed characteristics of the H-reflex at the period of 4 to 10ms. following conditioning shock was caused by the afterhyperexcitability of the motoneuron by the conditioning shock as well as summation of the test shock, but the IPSP of Group Ib efferent impulses and influence of Renshow cell were other important mechanisms. The author intended to call abovementioned brocess the fascillitatry of human H-reflex.

In an attempt to improve the chemotherapy of acute leukemia in adults, the author has studied the induction therapy on 63 cases with acute leukemia with Neocarzinostatin (NCS; N) in combination with conventional antileukemic agents; Vincristine (V), Daunorubicin (D), Cytosine Arabinoside (A), 6-Mercaptopurine-Riboside (M) and Prednisolone (P). Eighteen cases with acute lymphocytic leukemia (ALL) was treated with protocol A (NVMP, NDMP, NAMP). Out of 45 cases with acute non-lymphocytic leukemia (ANLL), 25 was treated with protocol A and 20 treated with protocol B (NADP, NAVP), and obtained the following results. 1) Of all 63 cases 51 (81.6%) attained complete remission and 9 (14.3%) partial remission. 2) Complete remission was obtained in 15 cases (83.3%) out of 18 with ALL and 36 cases (80.0%) of 45 with ANLL; the latter cases with ANLL consisted of 18 of 25 cases (72.0%) treated with protocol A and 18 of 20 cases (90.0%) with protocol B. 3) Refractory varieties included cases with acute myelogenous leukemia who had passed through atypical hematological findings, cases with monocytic leukemia, cases with WBC count over 50,000 per μl or with nucleated cell count in bone marrow over 500,000 per μl before treatment and cases over 50 years old. 4) In this induction therapy with NCS in combination, antileukemic effects was remarkable, while suppression on normal hematopoiesis, especially on thrombocytes, was very slight. 5) As to the other side effects no noteworthy disturbances other than occasional nausea, anorexia and liver damage in protocol B were observed. From these results it can be concluded that NCS in combination is one of the most potent remission induction therapies in acute leukemia in adults.

Neocarzinostatin (NCS) is an antitumor antibiotic isolated from streptomyces carzinostaticus by Ishida et al. in Japan in 1957. The chemical structure of NCS is a single chain made of acidic polypeptide consisting of 112 amino acids. Its antitumor activities have been demonstrated in various experimental tumors. Its mode of action is considered to inhibit DNA synthesis at early stage of S phase and to block it at G(2) phase. The agent has been used clinically for solid tumors without remardable effects, although never been used in treating human leukemia. For the first time the author has given NCS alone to patients with acute leukemia in the dose ranging from 0.04-0.06 mg/kg/day for 4-5 days as one course of treatment. In case no sufficient remission was induced further courses of treatment were repeated after an internal of 4-10 days, and obtained the following results. 1) Of 18 cases 7 (38.9%) attained complete remission and 4 (22.2%) partial remission. 2) Complete remission was obtained even in the patients with refractory varieties of acute leukemia, i.e., acute promyelocytic leukemia, monocytic leukemia and leukemia in aged group. 3) No noteworthy side effects other than occasional nausea or anorexia were observed and suppression on normal hematopoiesis was very slight. From these results it can be concluded that NCS is one of the most potent antileukemic agents for inducing remission in acute leukemia.

To evaluate the prognosis of the sustained biochemical responder after interferon (IFN) therapy, we retrospectively studied 252 chronic hepatitis C patients who were treated with IFN. Patients were divided into four groups: group A, sustained virological responders (n=84); group B,sustained biochemical but not virological responders (n=43); group C, incomplete responders (n=64); group D, non responders (n=61). The levels of several liver function tests were evaluated at the end of the observation period (4.2±1.6 years, mean±SD) compared with those at just before IFN therapy. The levels of cholinesterase, albumin, γ-globulin, zinc sufate turbidity test, platelet count and clearance rate of indocyanine green test improved in group A (p<0.05), became worse in group D (p<0.05) and did not change in group B. The incidence of hepatocellular carcinoma was significantly higher in group D than in group B (p<0.01);Kaplan-Meier method, log-rank test). The hazard ratio for hapatocarcinogenesis of the patients in group A and B was significantly lower than that in group C and D (hazard ratio: 0.27, range of 0.08-0.98; p=0.046) adjusted for age, gender, stage and total alcohol consumption.
These results suggest that the progress of liver disease and liver carcinogenesis was more suppressed in sustained biochemical responders than in non reponders.

It has been reported that both iron oxidase and hydrogen sulfide: ferric ion oxidoreductase (SFORase) were involved in bacterial leaching of metal ions from sulfide ores, and the amount of Cu2+ solubilized from copper ore by iron-oxidizing bacterium differed from strain. The activities of iron oxidase SFORase of iron-oxidizing bacteria isolated from the natural environments were determined. Iron-oxidizing activity and SFORase activity of 200 strains ranged from 1.20-1.65γmol/mg/min and from 0.11-2.80 γmol/mg/min, respectively. The findings that a remarkable difference was observed in the levels of SFORase activity, but not in iron-oxidizing activity, suggest that SFORase, but not iron oxidase, is the enzyme that determines the bacterial leaching activity of this bacterium.

The world's oceans cover 70% of the eath's surface, with about 3,800m of average depth.Altough　the deep-sea environment with its high pressure and low temperatures is too extreme for most terrestrial and marine surface microorganisms, many barotolerant and barophilic bacteria have been found inhabiting the deep-sea.It is exyremely important for barophilic or barotolerand deep-sea bacteria to maintain the physiological functions of cytoplasmic membrane, which serves many vital functions. The fluidity of this cytoplasimic membrane composed of phospholipids and poteins is essential for the physiological functions of cells. As higher hydorstatic pressure raise the melting point of lipids and cause phase transition
of lipid under pressurs of up to 100MPa,barotolerant and barophilic bacrteria under high hydostatic pressure appear to regulate the composition of their membrane phospholipids. Therfore the characrization of cytoplasmic membrane under high pressure is indispensable to clarify the mecanisms of bacteria adaptation to the deep-sea enviroment. The effects of pressure and temperature acid compositon of barotolerant deep-sea bacteria were investigated.Deep-sea bacteria maintained their membrane fluidity by increasing the content of unique fatty acid in phospholipids under high hydrostatic pressure.Gene expression seems to be necessary for the synthesis of unique fatty under high hydrostatic pressure.

The oxidation of reduced sulfur compounds was studied by using resting cells of sulfur-grown Acidithiobacillus caldus strain GO-1. The optimum pHs for the oxidation of thiosulfate, tetrathionate, sulfur, sulfite and sulfide were 2, 3, 3-6, 7 and 7, respectively. The highest oxidation rate was observed with sulfite. The oxidation rates of the reduced sulfur compounds were measured in the absence or presence of inhibitors and uncouplers. 2, 4-dinitrophenol (DNP) and carbonyl cyanide-m-chlorophenylhydrazone (CCCP) strongly inhibited the oxidations of sulfur and sulfite. N-Ethylmaleimide (NEM) strongly inhibited the oxidation of tetrathionate and sulfur. 2-heptyl-4-hydroxy-quinoline-N-oxide(HQNO) inhibited the oxidation of sulfur and sulfite. The results suggested that tetrathionate was oxidized in the periplasmic space, and sulfur and sulfite were oxidized in the cytoplasm. Pyridine ferrohemochromes prepared from the membrane of strain GO-1 cell revealed the involvement of cytochromes b and c. Ubiquinol oxidase activity was detected in strain GO-1 cell, but cytochrome c oxidase measured by using mammalian cytochrome c as an electron donor was not detected in the cell. On the basis of the results a model for the metabolism of the reduced sulfur compounds by At. caldus strain GO-1 was proposed.

The PCR-amplified 16S-23S rDNA intergenic spacer regions (ISRs) of Acidithiobacillus ferrooxidans, A. thiooxidans, and A. caldus strains were scquenced and evaluated for differentiation and identification of these bacteria. The total length of the 16S-23S ISRs of A. ferrooxidans and A. thiooxidans strains and A.caldus GO-1 were 441, 456, nd 379bp, respectively. Two genes. encoding tRNA and tRNA, and the box A-like sequences were highly conserved in the ISRs of all Acidithiobacillus species. The restriction fragment length polymorphism (RFLP) profiles of the PCR-amplified 16S-23S rDNA ISRs digested by HaeIII and AluI could clearly discriminate A. ferrooxidans from A. thiooxidans and A. caldus. The results indicated that RFLP analysis of the 16S-23S ISRs is an easy and rapid method for discrimination and identification of Acidithiobacillus species.
