By carrying out follow-up examinations for the period ranging from two to six years after the radium therapy, the author obtained the following results. 1) In the examination around the nasopharyngeal regions by nasopharyngoscopy no sear formation could be recognized even in the cases given two rounds of the treatment and likewise no subjective complaint as a side-effect such as feeling of driness was revealed. 2) During the course of follow-up examinations there occurred 22 relapses (28.6% ) out of the 77 ears. In the majority the relapse occurred generally within six months after the treatment, but in some it occurred after more than six months to one and a half years afterward. 3) As for the findings concerning the conditons of the auditory tube in the examinations either by nasopharyngoscopy or of the patency of the Eustachian tube, all showed a marked improvement. 4) Concerning the hearing recovery at the time of re-examinations conducted during the periods from two to six years after the treatment, severe cases showed a morn marked improvement rather than the mild cases. Of them the majority revealed the average hearing loss of about 10 db in speech range. Furthermore, these re-examined cases revealed a better hearing recovery than the recovery obtained at two to six weeks after the treatment.

The author studied hearing acuity, findings of nsopharyngoscopic examinations and changes in the patency of the Eustachian tube in the 277 ears of the children with tubal stenosis, ranging in age from 5 to 15 years, who had received various tseatments without success in our Better Hearing Clinic, Departmment of Oto-Rhino-Laryngology, Okayama University Medical School and who has been finally submitted to the radium irradiation therapy 1) As for the method of radium irradiation, a piece of radium needle containing 5.5 mg. radium is put in a brass holder 1 mm. thick and it is inserted up to the orifice of the Eustachian tube through the nose and fixed in the nasopharyngeal cavity. As for the amount of irradiation, an individual case receives 3 to 4 times of 33 mg. hour of irradiatiou for six hours at a time at the interval of seven days, which is considered as a round of teeatment. Usually only one round of such treatment has been given, but in some cases two or three rounds have been administered with 2 to 5 months' interval between the rounds. 2) Out of the 216 ears examined by an an audiometer 144 ears (66.7%) have recovered to the normal hearing; and the 61 ears given infant speech tests have likewise yielded approximately the same results. 3) In order to obtain a better result in the radium treatment, it is necessary to give first such treatments as the treatment for the fluid in the middle-ear cavity, adenoid, or sinusitis. 4) Out of 156 ears, 104 (60,6%) improed by the irradiation treatment. In these 104 cases the majority of them showed granular change of the Eustachian orifice or residual adenoid in the nasopharynx, and furthermore, these types were the ones to whom the irradiation treatment proved to be most effective. 5) As for the patency of the Eustachian tube, 63 ears (60%) of 105 cases with severe stenosis showed improvement after the irradiation, while 38 ears (80%) of 47 cases with mild stenosis improved, 6) As the relapse is apt to occur even more than one year after the treatment, it is necessary to carry on the periodical follow-up examinations.

We report a surgical case of intestinal anisakiasis in which we identified a complete larva. A 48-year-old man complaining of epigastralgia after eating roasted mackerel was admitted to the hospital with a diagnosis of ileus with peritonitis. Abdominal CT showed dilatation and partial thickening of the small intestine. An emergency operation was carried out. On laparotomy, severe stricture of the jejunum was revealed. The affected intestine was resected. A larva whose head was invading the mucosa was found in the resected portion of the jejunum, and we diagnosed the case as intestinal anisakiasis.

Platelet aggregation was studied on 670 samples of 20 healthy controls and 279 patients in the late period after cardiovascular surgery. Aggregating agents used were 0.5 and 2.0μM ADP as well as 2mM arachidonic acid (AA). Among the healthy controls, a maximum aggregation rate over 18% (ADP 0.5μM) was considered hyperaggregability, those under 47% (ADP 2μM) or under 78% (AA 2mM) consid-ered hypoaggregability. Maximun aggregation rate in patients not given antiplatelt agents widely varied from hyperaggregability to hypoaggregability. There was a positive correlation botween the first sample and the second one gained from each patient. This suggested that individual variation existed on platelet aggregation. Maximum aggregation rate significatly decreased by ticlopidine for ADP and by aspirin for AA. Both ADP and AA should be used for platelet aggregation. Comparison among three groups of different ticlopidine doses revealed that the doses should be decided on the basis of platelet aggregation for each patient. Thrombotic or hemorrhagic complications were observed in only 9 cases. Four of 7 cases of thrombotic complicatiion showed hyperaggregability and only 1 case showed hypoaggregability. Both cases of hemorrhagic complication showed hypoaggregability. These findings suggest that indication for antiplatelet therapy and doses of antiplatelet agent should be decided in consideration of individual variation on platelet aggregation.

Various immunological processes are suspected of having some relationship with the onset of diffuse panbronchiolitis (DPB). Therefore, the cellular and humoral immunity of patients with DPB were studied. Sera of patients with DPB showed a high frequency of positive RA factor and high titer of cold agglutinin. Delayed skin tests with phytohemagglutinin (PHA) and purified protein derivative (PPD) in patients with DPB revealed normal responses to PHA as nonspecific mitogen and suppressed responses to PPD as specific antigen, although lymphocyte blastogenesis in vitro showed suppressed responses to PHA. The proportion of lymphocytes in bronchoalveolar lavage (BAL) fluid of DPB patients equaled that of normal controls. Furthermore, the CD4/CD8 ratio of T-cell subsets in BAL fluid of DPB patients were significantly lower than that of normal controls, although the CD4/CD8 ratio in peripheral blood of DPB were significantly higher than that in normal controls. The percentage of B-cells in the peripheral blood of DPB patients was hihger than that in normal controls. These data suggest that the activation of cellular and humoral immune mechanisms could play an important role in the pathogenesis of DPB.

A nutritional and metabolic assessment of a cardiopulmonary bypass technique without donor blood was made in 23 patients undergoing open heart surgery (non-donor blood group). For comparison, 14 patients receiving cardiopulmonary bypass with donor blood prime (donor blood group) were also evaluated. 1)Serum transferrin level showed significantly more rapid recovery in the non-donor blood group compared to the donor blood group on the 7th post operative day. 2)Total protein, serum albumin, arm muscle circumference, creatinine-height-index and triceps skin fold showed no significant differences between the two groups. 3)Total lymphocyte count as an indicator of immune status showed a significant increase in the non-donor blood group on the 3rd post operative day. 4)The recovery from the postoperative hypermetabolic state was more smoothly achieved in the non-donor group blood group than in the donor blood group. In conclusion, the cardiopulmonary bypass technique without donor blood is a safe and effective means of open heart surgery, not only saving blood but also preventing undue, infectious and/or hypersensitive diseases.

Not only many food substances but also other substances such as house dust(HD) may serve as causative allergens of allergic gastroenteropathy. To clarify the causative allergens and the pathogenesis of allergic gastroenteropathy, an oral loading test using HD, was applied to two patients with allergic gastroenteropathy caused by an unknown antigen, and with HD-sensitivity, and 24 bronchial asthmatics with or without HD-sensitivity, ther were examined using the ultrasonography of stomach reported previously. Two patients with gastroenteropathy showed a positive skin test and IgE RAST to HD, although they had bronchial asthma in the past history. Peripheral blood eosinophils in the 1st and 2nd patients showed a high (max 78%) and slightly increased (9%) value. The precipitating antibody in the two cases was negative, but lymphocyte blastogenesis to HD was enhanced in one case. During the oral loading test using HD, both patients had abdominal pain at immediate and late phase and acceleration of gastric peristalic movements was confirmed by ultrasonography. One of the 2 patients showed a strong gastrointestinal reaction during the delayed phase. LTC4 and histamine were considered to be involved in the abdominal pain of these cases. One of the 12 patients with bronchial asthma and HD-sensitivity showed an immediate reaction in oral loading test without abdominal pain, suggensting a correlation of the hypersensitivity between the bronchi and the digestive tract, including 2 former cases.

A 63-year-old male died in the hospital from erroneous drip infusion of thrombin, a local hemostatic, after polypectomy for intestinal polyposis. At autopsy, thromboses were observed in the cutaneous veins of the right arm, the right axillary vein and the right subclavian vein, and soft fibrin clots in the superior vena cava, the right atrium, the right ventricle and the pulmonary arteries. Thrombi were also found in the rami of the arteries in both lungs under the microscope. Leucocyte infiltration was recognized in the thrombus, but no organization. This finding shows that the thrombi were fresh. The remainder (150ml) of the drip solution was analyzed for thrombin activity and 8 to 16 units of thrombin activity per ml were demonstrated. From these results, the cause of death was considered pulmonary thrombosis due to intravenous administration of thrombin.

DPB has been known as a chronic respiratory infection with poor prognosis until introduction of erythromycin (EM) therapy with low-dose and long-term administration. As symptoms and prognosis of DPB were improved enormously under EM therapy, EM therapy on DPB was analyzed to clarify the action and mechanisms. EM in a daily dese of 600 mg was administered to 9 patients with DPB for about 38 months and 7 normal volunteers for 2 months. Most patients with DPB showed the relief of symptoms within 2 months, although the EM concentrations in the blood and sputa were lower than the antibacterial therapeutic level. Immunological examination revealed the decrease in cold agglutinin titer and CD4/CD8 ratio after EM therapy. The neutrophil chemotactic factor (NCF) derived from the mononuclear cell culture of DPB patients was higher than that in the normal control. EM therapy decreased the level of NCF not only in patients with DPB but also in normal volunteers. These findings indicate that the immunological effects of EM play an important role in the treatment of DPB.

Bone marrow cell cycle analysis was performed on 23 patients with myelodysplastic syndrome (MDS)[12 with refractory anemia(RA), two with RA with ringed sideroblasts(RARS), three with RA with excess blasts(RAEB), six with RAEB in transformation(RAEBt)], and 14 normal volunteers. Cells were stained with propidium iodide and analyzed by flow-cytometry. Patients with RA and RARS (low-risk MDS) showed higher rates of S-phase (21.2±4.9%) and patients with RAEB and RAEBt (high-risk MDS) showed lower rates of S-phase (9.9±5.9%) compared with normal controls (15.2±3.1%). Although the rate of S-phase decreased with disease progression, it was normalized with complete remission. An intermediate correlation (r=0.64, p<0.01) existed between the rate of S-phase and the percentage of erythroblasts in MDS. The rate of S-phase clearly reflects the severity of MDS. The high rate of S-phase in low risk MDS was concidered to be due to erythroid hyperplasia with ineffective hematopoiesis.

The superoxide released from neutrophils works as a bacteriocidal agent in the biological protection system. On the other hand, superoxide also has been known to progress inflam-mation, tissue damage and fibrisis. Therfore, the superoxide production by neutrophils of patients with diffuse panbronchiolitis (DPB), idiopathic interstitial pneumonia (IIP) and collagen vascular disease with interstitial pneumonia (CVD+IP) were studied in relation to disease activity and progression. Superoxide production was measured by deduction of cytochrome C with a spectrophotometer. The superoxide production was higher in DPB than in the normal controls. The superoxide in DPB showed a positive correlation with CRP and white blood cell count, but inverse correlation with arterial blood oxygen pressure. High levels of superoxide in DPB decreased following erythromycin therapy with clinical improvement. The averagr levels of superoxide in the patients with IIP and CVD+IP were lower than those in the normal controls. However, the superoxide level in the patients with IIP and CVD+IP in the active stage was higher than that in those in the nonactive stage. In the active stage, the increase of steroid dose was follwed by a decrease of superoxide production in IIP and CVD+IP. These findings indicate that measurement of superoxide production by neutrophils could be an useful means to evaluate activity in diffuse pulmonary diseases.

Purpose. To test whether encagement and volume load as stressors influence the intraocular pressure in rabbits.
Method. Rabbits were encaged for an hour in a horizontally-plastic tube with an internal diameter of 13.2 cm and a length of 33.2cm. After the encagement, rabbits received a rapid intraveonus drip infusion of 5% glucose solution, 20 ml per kilogram of body weight, for 5-6 minutes. All experiments were performed at the same time of day.
Results. The intraocular pressure after encagement (11.2±3.0mmHg as a mean and standard deviation) was significantly higher compared with that of control rabbits without encagement (9.2±1.0mmHg, Student t-test,p=0.046). This differnce became signifficantly larger when volume load was exerted on both groups of rabbits (p=0.0007). Serum levels of cortisol, adrenaline, and nordrenaline were significantly elevated afte encagement (n=6, p=0.0002, p=0.027, p=0.030, respectively). Venous pressure of the ear tended to increase in rabbits after encagement (15.8±3.1mmHg) compared with that of control rabbits (8.5±2.3mmHg,p=0.11), and the difference became significant when volume load was placed on each group of rabbits (p=0.021).
Conclusions.Encagement, especially in combination with volume losd, increased the intraocular pressure in rabbits. The elevation of peripheral venous pressure caused by an increase in serum catecholamines would underlie the elevation of intraocular pressure proh-ably through the increased outflow resistance.

This syudy was designed to clarify what factors influenced the ability to stop smoking in male smokers, from the viewpoint of life-style guidance and primary care. Of the people who attended abult health examinations between 1993 and 1998 in Okayama Ctiy, 987 male smokers were selected and categorized according to the number of cigarettes per day into three groups. The relation was analyzed statistically between the three groups and the life-style related factors : age, the degree of obesity, sleep, snacks, salt, exercise and alcohol. The results showed significant differences in age, the degree of obesity, sleep, salt, exercise. Second, the relation between threse three groups and the experience and duration of smoking cessation was analyzed. The results showed a significant differnce in the experience of smoking cessation, but no significant differnce in duration of smoking cessation. The conclusions are as follows : 1. Male smokers, who do not activedy try to improve of their life-style related factors, have fewer experiences of smoking cessation, 2. Male smokers who try to improve their life-style habits have a similar period od smoking cessation to those who do not. 3. Long term support for all male smokers to continue to be non-smoking is important.
