A case of acoustic neurinoma with huge cysts was reported clinico-pathologically. A male developed at the age of 30, the left-sided hearing difficulty. At the age of 48, he noted the staggered gait which after a while almost entirely cleared up. At the age of 54, he was again unable to walk because of ataxia. Neurological symptoms at the admission to hospital were almost agreed with those in acoustic neurinoma. The symptoms showed complete remission partly because of medication, but they became worse rapidly again, The patient died from bronchial pneumonia at the age of 60. Autopsy disclosed a tumor with huge cysts at the left cerebello-pontine angle which was diagnosed neurinoma microscopically. The most unusual clinical point lies on the course. It lasted thirty long years and moreover displayed an intermittent course showing remission and exacerbation. Another point of interest is the huge cysts found in the tumor at autopsy. The authors discussed the cases manifestating atypical clinical course and tumors with huge cysts in the literature.

The study was performed to examine the relationship between defense reaction of skin to local stimulation and enzymological change of the exudates produced by the application of Unguentum vesicans. The activities of glutamate oxaloacetate transaminase (GOT), glutamate pyruvate transaminase (GPT), lactic dehydrogenase (LDH) and LDH isozyme were measured about exudates and sera in hepatitis, cancer, lung tuberculosis, nephritis and other conditions. The results of the study are summarized as follows. (1) The activities of GOT, GPT and LDH of exudates were higher than the activities of sera in almost cases. The GOT activity of exudate was paralleled to elevation of GOT activity of serum in hepatitis and cancer, and similar correlation of GPT activity was observed in hepatitis. There was not correlation of changes of LDH activity between exudate and serum. (2) There was not the relationship between the strength of local inflammatory responses and the enzyme activities of exudates. (3) LDH isozyme of the exudate was similar to isozyme pattern of epidermis. (4) The most of GOT and GPT of exudate were released from blood, but LDH of the exudate originated in epidermis.

In order to study the difference of defense reactions to local stimulation in various diseases, an experiment was performed to examine cellular characteristics of exudates produced by the local application of Unguentum vesicans.
The results of the study are summarized as follows. (1) The average time to the onset of blister formation following the application of Unguentum vesicans was 13.3 hours, and had a tendency to be delayed by aging process, particularly in an old age group with lung tuberculosis. (2) The average volume of the exudates at 24 hours after the application was 0.09cc/cu, and had a tendency to be decreased by aging process and in patients with consumptive diseases. (3) The average number of cells in the exudates at 24 hours after the application was 2410/m㎥, and had no relationship to the age or disease processes of patients. (4) The majority of the cells in the exudates consisted of polymorphonuclear neutrophils with a few non-segmented neutrophils, lymphocytes, monocytes and eosinophils. The cellular composition in the exudates was little altered by the age or disease processes of patients. (5) The investigation of the exudates produced by the application of Unguentum vesicans may be useful in the evaluation of local inflammatory responses in various disease states.

Leiomyoma of the esophagus is a rare lesion. On this case that we reported, the right oblique chest radiogram showed the soft tissue mass in the mediastinum. The diagnosis was made by the barium swallow study. In this report, we chiefly discussed about the radiologic findings of the esophageal leiomyoma and differential diagnosis with extraesophageal tumors.

Leiomyoma of the small intestine is a rare lesion, and most cases are found during operation and during autopsy. This patiant who we reported had a history of melena. Oval filling defect was detected in the jejunum by means of conventional barium examination. The histologic type of this neoplasm could be suggested by its radiologic findings and probed by operation.

In order to investigate the relationships between whole blood cells and liver slices on lipid metabolism in chronic CCl(4) intoxicated rats, the author studied in vitro incorporation of (14)C-acetate Na into gross lipid fracitons, major lipid fractions and total fatty acids. The results obtained are summarized as follows: 1) (14)C incorporation by liver slices into total lipids, unsaponifiable lipids and total fatty acids was markedly increased at 2 weeks, but at 12 weeks it was not so much increased except for total fatty acids than controls. On the other hand, (14)C incorporation by whole blood cells into each of them was similar to that obseved with controls at 2 weeks, while it was slightly decreased at 12 weeks. 2) Not only in acute CCl(4), intoxication but also in the chronic stage, as mentioned in Part I, it is considered that fatty liver would be caused by enhancement of TG and NEFA synthesis in liver, and at the same time lipid metabolism in whole blood cells would contribute to fatty liver. 3) An increae of percent (14)C incorporation by liver slices into fatty acid group 14:0+16:0, namely an increased acitivity of malonyl CoA pathway was observed at 2 weeks as well as at 12 weeks while whole blood cells incorporated relatively more radioactivity into fatty acid group 14:0+16:0 at 2 weeks, but at 12 weeks the percent radioactivity in oleic, 20 carbons' and more longer chains' fatty acids, derived from mitochondrial pathway, showed a significant relative increase. 4) As mentioned in Parts I and U, lipid metabolism in liver slices was influenced strikingly following administration of CCl(4), but that in whole blood cells showed few differences between CCl(4) administered rats and controls. It may be possible to conclude from these results that whole blood cells would contribute to maintain the homeostasis of lipid metabolism in vivo.

The present study was designed to investigate some role of whole blood cells in lipid metabolism compared to that of liver in acute carbon tetrachloride intoxicated rats. In order to determine this, in vitro incorporation of (14)C-acetate Na into gross lipid fractions, major lipid fractions and total fatty acids was studied. The experimental animals employed were male Wistar rats, which were subjected to subcutaneous injection of 50% CCl4 in olive oil, 0.2ml/100gm. of body weight. Animals were sacrificed at time intervals of 4 and 48 hours following treatment. The results obtained are summarized as follows: 1) (14)C incorporation by liver slices into total lipids, unsaponifiable lipids and total fatty acids was markedly decreased at 4 hours, but at 48 hours it was strikingly increased in accordance with liver cell regeneration. On the other hand, (14)C incorporation by whole blood cells into each of them showed a trend toward an increase at 4 hours, although there were few differences between carbon tetrachloride administered rats and controls at 48 hours. 2) Percent (14)C incorporation by liver slices into TG and NEFA increased significantly following CCl(4) treatment, while whole blood cells incorporated relatvely more radioactivity into NEFA than that of controls. Consequently it is considered that fatty liver would be caused by enhancement of TG and NEFA synthesis in liver, and at the same time lipid metabolism in whole blood cells would contribute to fatty liver. 3) In acute CCl(4) intoxication of rats, a decrease of percent (14)C incorporation by liver slices into fatty acid group 14:0+16:0, namely the decreased activity of malonyl CoA pathway was observed, while that into 20 carbons and more longer chains, fatty acids, formed via mitochondrial pathway, showed a significant relative increase; the results being similar to those observed with whole blood cells.
