Cojugated steroids in placenta A) Estrogen (μg/placenta) Estrogen as sulfate were present one tenth of free type in placenta. There were a little content of 16-epi-Estriol and Estriol glucuronide. B) Gestagens (μg/placenta) P. nolone and 17-OH-P. nolone that were ⊿5-3β-ol type steroids were estimated as sulfate. C) Androgens DHA+Andr. fraction as sulfate was estimated higher content than free type. All androgens glucuronide were not meassurable in placentar tissue. D) Corticoids Tetrahydro. comp. that were THF, THE, THS etc, as glucuronide were estimated considerably high. Above all, THE glucuronide was estimated higher content than free type of THE. kn-abstract=従来,内分泌臓器としての胎盤からステロイドホルモンを抽出分離定量している報告は数多くあるが,そのいずれもが単一のステロイドのみを定量したものか,あるいは大量複数の胎盤を使つてのステロイドの抽出分離定量したかのいずれかにすぎなかつた.しかし胎盤組織内でのステロイドの代謝過程が次第に解明されつつある現在単一のステロイドのみ測定や大量複数の胎盤を使つてのステロイド測定の意義は徐々に失なわれつつある.そこで著者は正常満期産1個の胎盤におけるmetabolitesを測定することは胎盤における個体差及びその特徴を知る上に重要と考え,従来の定量法の難点を解決し満期産1胎盤組織中から測定しうる各種ステロイド(21種)を同時に抽出分離精製定量する新定量法を確立した.この方法により正常満期産胎盤16個について遊離型ステロイド21種を測定することができた.更に結合型ステロイドであるGlucuronide, Sulfateについても独自の方法により3胎盤を1つの測定単位として抽出分離定量し遊離型ステロイドと対比した.その結果中性遊離型ステロイドでは活性型以外に5位の飽和された不活性型が相当みとめられ△4-typeと5位の飽和型の比は1:1.7であつた.結合型ステロイドについてはエストロゲンは少量ながらGlucuronide, Sulfate共にみとめられたがP. nolone, 17-OH-P. nolone, DHA+Andr.にはSulfateが認められ,中でもDHA+Andr.では遊離型より多く認められた.Tetrahydro. However, side effects of massive transfusion have been reported in large number of cases and several attempts of medicamental treatment to reduce an opportunity of the exchange transfusion have also been evaluated. In this connection, effects of ACTH and Glycyrrhizin on neonatal jaundice were determined in mature infants during first 7 days of life. 1) Serum bilirubin was significantly decreased by ACTH administration, while suppression of serum bilirubin increment, characteristic changes in neonatal period, was observed following Glycyrrhizin administration. 2) ACTH administration resulted in hyperglycemia, increased urinary electrolytes, 17 KGS and bilirubin excretion. However, amounts of increased urinary bilirubin excretion was found to be one tenth of amounts of decreased serum bilirubin. On the contrast, no remarkable changes in blood sugar, electrolytes, urinary electrolytes, 17 KGS, and bilirubin excretion was noted following Glycyrrhizin administration. 3) On the treatment of hyperbilirubinemia, serum bilirubin were significantly decreased in cases of induced hyperglycemia. 4) Characteristic decrease on red blood cell counts, osmotic fragidity, Hb. and Ht. during neonatal period were improved by Glycyrrhizin, while no effect was found on reticulocyte counts. It may be summarized that ACTH has wide effects on bilirubin metabolism including a interaction of glucose metabolism and Glycyrrhizin relate a prevention of hemolysis increasing osmotic fragidity of red blood cell. Case 2 had mental defficiency, obesity, polydactylia and Retinitis pigmentosa, and was recognized constant mental symptoms (lacking in spontaneity, mutism, negativism and so on). The mental symptoms of Laurence-Moon-Biedl syndrome seems to be classified as two groups. That is, the first group has constant mental symptoms and the second group has periodic mental symptoms. It is said that there was a psychotic case in the relatives on the paternal side of the patient. Her hereditary history remains obscure, but her own history is uneventful. A review was made of this case who had episodes of stuporous state six times, and episodes of delutional-hallucinatory state twice since the age of 13. Clinical and EEG observations were carried out from Nov. 1965 to Aug. 1966, and from March 1967 to July 1967 when she was admitted to our clinic. The results are breifly summarized as follows. The stuporous state which presented mutism and akinesia as the principal symptom was difficult to differentiate from the stupor observable in schizophrenia, and it developed in a day or two at the onset, which lasted for about two weeks and then was alleviated. During this period of episodes she showed no noteworthy symptom of disturbed consciousness and she could recall what had happened to her precisely. Her EEG revealed that paroxysmal abnormal waves characterized by spike wave were inhibited during the stuporous state, but in the stage of remission paroxysmal abnormal waves appeared spontaneously and were activated easily by hyperventilation. This stuporous state could be alleviated transiently by the administration of amytal sodium, and also with a large dose of valium it was improved. On the contrary, anti-epileptic agents had no effect at all on the stuporous state. From the characteristic features of the psychotic state, this case could be construed as a type of atypical psychosis, but paroxysmal abnomality on EEG was similar to that of epilepsy, and so this was the case that had clearly shown to stand on the boundary line between atypical psychosis and epilepsy. Therefore, it seemed that epileptic factors of atypical psychosis should be dicided only after long, continuous observations of the case. Some discussion was made on physiological mechanism inducing a stuporous state. Expermitents were devided into four groups; whole blood group, 20% hemodilution group and 70% hemodilution group. In 50% and 70% group, the perfusion cooling was initiated with 20% diluted blood and at 25℃ of esophagno temparature (E. T.) the perfused blood was partially replaced with diluents, resulting in 50% hemodilution or 70% hemodilution. On rewarming the large volume hemodilution was adjusted to 20% by replacing with fresh heparinized blood at 25℃ of E. T. The results obtained were as follows. 1) Four of 14 cases of 70% hemodilution group were died of pulmonary edema at the end of perfusion. Two of 14 cases survived inlong term. One of 5 cases of 50% hemodilution group survived long term and others of all group were died within 24 hours post perfusion. 2) High grade metabolic acidosis occured in 50% and 70% hemodilution group at 10℃ of E. T. but by replacing with fresh heparinized blood at 25℃ of E. T. was improved. In several cases of 50% and 70% hemodilution group it still continued at the end of perfusion. 3) In all cases arterial PO(2) was kept higer than normal value during the whole perfusion. 4) Serum Na and K concentration were not changed during perfusion with whole blood below 25 C of E. T. 5) Blood pooling volume was increased higher in whole blood group than in hemodilution groups. No significant difference was found among all hemodilution groups. 6) There was no significant change of arterial blood viscosity between 50% and 70 group during perfusion. In whole blood group, viscosity was increased higher on cooling and decreased on rewarming. In 20% hemodilution group, viscosity was increased relatively higher on cooling and decreased to preoperative value on rewarming. 7) In whole blood group blood flow in carotid artery maintained 32% of the preoperative value at 10℃ of E. T. when blood pressure was 39% of the preoperative level. In 70% hemodilution group; the flow and pressure were 42% and 26% of the preoperative values respectively. 8) From the results of the studies optimal hemodilution ratio below 26℃ of E. T. was thought to be between 20% and 50% 9) Clinical study on profound hypothermia were performed in 12 cases by closed or open chest method with 35-40% hemodilution below 25℃ of E, T. Following results were obtained. 1) High flow perfusion is superior to low flow in all aspects. 2) Low molecular weight dextran is effective to prevent peripheral intravascular pooling of blood resulting in steady venous return, to maintain high flow rate and normal blood pressure, and also to inhibit increase in excess lactate. Dilution rate ranging from 15 to 25 per cent is pertinent. 3) Correction of pH with either sodium bicarbonate or Tris-buffer is preferable to compensation by respiratory alkalosis. 4) Survival time of the experimental animals can be prologed by blood replacement with fresh blood at the termination of the perfusion. However, no experimental animals survived the experiment. The same was studied on patients who underwent open heart surgery. All survived the operation and the results were as follows. 1) The metabolic changes are most prominent immediately after the perfusion and a tendency to recover appears 3 hours after the procedure. The first axis (X) shows places, the second (Y) years and the third (Z) degrees of pollution expressed by disorbed oxygen (DO), chemical oxygen demand (COD). biochemical oxygen demand (BOD) and iodine consumptions. This device has been proved possible to evaluate the degree of pollution simultaneously concerning places, laps of years.
