Title Alternative An evaluation of pancreatographic findings in the differential diagnosis of chronic pancreatitis and pancreatic cancer

Abstract In an attempt to evaluate the pancreatographic findings, particularly of the pancreatic duct branches, in the differrential diagnosis of chronic pancreatitis and Pancreatic cancer, twenty seven post mortem pancreases were subjected to retrograde pancreatography and histological examination. The materials consist of 15 cases with chronic pancreatitis and 12 cases with pancreatic cancer. The results obtained are as follows. 1) In cases with chronic pancreatitis, abnormal findindings such as irregular dilatation, straightening-hardening, narrowing-stenosis of the pancreatic duct branches were more common. 2) Incases with pancreatic cancer, abnormal findings such as diminishing, irregular dilatation, narrowing-stenosis of the pancreatic duct branches were more common. 3) Abnormal findings in cases with chronic pancreatitis reveled the dendency of diffuse expansion, on the other hand, in case with pancreatic cancer revealed the tendency of localized expansion. 4) When the fine and clear pancreatography was obtained, it was possible to predict the presence of chronic inflammation or tumor cell infiltration from above findings and tendencies. On the contrary, in cases with localized, severe chronic pancreatitis and in cases with complicated, it was difficult to differentiate the pancreatic cancer from chronic pancreatitis.

Abstract Histamine release from basophils induced by anti-IgE was examined in 14 asthmatic subjects with blood eosinophilia. The subjects were divided into two groups; group I (with blood eosinophilia, positive skin test and positive RAST to house dust) and group II (with blood eosinophilia, negative skin test to various allergens and negative RAST to house dust). 1. Serum IgE levels in group I were much higher than those in group II. 2. Maximum percent histamine release induced by anti-IgE was much higher in group I than in group II. 3. Dose-response curve of anti-IgE-induced histamine release in group I showed consistent increase as anti-IgE concentrations increased. while, dose-response curve in group II was very low. These findings suggested that blood eosinophilia in group I might be elicited relating to IgE-mediated reaction. However, mechanism causing eosinophilia in group II was not clear in the present study.

Title Alternative A case report of a family with Peutz-Jeghers syndrome

Abstract A 58-year-old female was admitted because of colicky abdominal pain. Physical examination revealed firm abdominal wall, increased bowel sounds and multiple pigmented macules on the lips, oral mucosa, soles and volar aspects of the fingers and toes. Hyperventilation and tetanic rigidity of the extremities were also noted. The symptoms were successfully treated by intravenous injection of butropium bromide and diazepam. Roentgenological and fiberscopic examination revealed multiple polyps in the stomach, small intestine and colon ; the small intestine was most heavily loaded with polyps. Biopsy specimen revealed only inflammatory changes. The patient had undergone a resectien of the terminal ileum with polyps 15 years previously because of ileo-ileal intussusception. Histological re-evaluation of the polyp showed an actively proliferating papillary adenoma. The characteristic history and gastrointestinal findings lead us to the examination of the family, which revealed that her father, son and grandson had similar pigmentation of the skin and polyps in the gastrointestinal tract. These characteristic findings and family history permitted us to make a diagnosis of Peutz-Jeghers syndrome. The diagnosis had been missed supposedly because of unawareness of the skin lesions and the lack of hamartoma-like findings in the polyp. The tetanic rigidity of the extremities was probably hyperventilation syndrome induced by severe abdominal pain. The patient has been followed up for possible recurrence of symptoms and a potential malignant change.

Title Alternative Effects of serial bathing on fatty acid composition of adrenal and serum cholesterol ester in rats

Abstract The effects of serial bathing (10 minutes bathing in a city water or hot spring in Misasa, 40℃, once daily for 1-4 weeks) on fatty acid composition of cholesterol ester of the adrenal glands and serum was studied in both normal and hypercholesteremic rats. Although the adrenal cholesterol ester levels were relatively unaffected in rats treated with serial bathing as well as non-treated rats, the proportion of 18 : 2 was increased in rats treated with hot spring bathing for 3-4 weeks. The serum cholesterol ester levels in both normal and hypercholesteremic rats tended to decrease after serial bathing for 3-4 weeks compared with non-treated rats. The fatty acid composition of serum cholesterol ester in normal rats was unaffected. The composition in hypercholesteremic rats showed an increase in the proportion of 16 : 1 and 18 : 1 and a decrease of 18 : 2 and 20: 4 compared with normal rats. When those of hypercholesteremic rats were treated with serial bathing for 3 weeks, particularly with hot spring bathing, the proportion of 16 : 1, 18: 2 and 20 : 4 showed a tendency to approach that in tormal rats. The findings indicate that serial hot spring bathing for 3-4 weeks, unlike city water bathing, exerts an influence on the fatty acid composition of cholesterol ester of the rat adrenal glands and it also improves the fatty acid compsition of serum cholesterolester of hypercholesteremic rats.

Title Alternative Effects of swimming training in the hot spring pool on ventilatory function in bronchial asthma

Abstract Effect of free swimming training in the hot spring pool (30℃) on ventilatory function were observed in nine patients with intractable asthma by measuring FVC, FEV(1.0), PEFR, MMF, V(50), V(25). 1. Swimming in the hot spring pool for three months allowed the reduction of glucocorticoid used for their asthma attack. 2. Although FVC was not affected by swimming training, EEV(1.0), PEFR, MMF, V(50), V(25) which represent obstructive ventilatory function were gradually improved by swimming training. The results show that free swimming training in the hot springpool is clinically available in intractable asthma.

Abstract 1) 炭酸塩と,コ-ク酸からなる錠剤型の「炭酸ガス浴剤」の保温作用を健康な男子8名について,サ-モグラフィーを用いて測定した. 入浴10分後の比較で明らかに,炭酸ガス浴はよく温まった結果,表面温度が高くなっている. 2) 腰痛,四肢冷感,その他の患者24名での臨床評価の結果,患者の90%以上が手足が温まり,湯ざめしにくいことを認めた.また,患者の85%は痛みがやわらぐことを認めた. 3) 主婦664名を対象とした使用評価の結果,常時手足の冷感を訴える者の63.6%,身体の疲労感・だるさ56.5%に効果を認めていることがわかった. 4) 副作用は全く認められなかった.

Title Alternative Properties of materials under hydrothermal conditions: II. Permeability to aqueous NaCl and decomposition of poly(tetrafluoroethylene) (teflon)

Abstract A piece of poly(tetrafluoroethylene) (PTFE) enclosing NaCI powder and having the dimensions shown in the inset in Fig. 1 was placed along with water in a deformable gold cell in a Dickson-type hydrothermal apparatus, and heated stepwise up to 410℃ under a constant pressure of 1 kbar. During the heating, small fractions of the solution in the cell were sampled and analyzed gas chromatographically for H(2), O(2), CH(4) and CO(4), and ion chromatographically for F(-) and Cl(-). No evolution of gases (H(2), CH(4), CO(2)) due to decomposition of PTFE, other than that due to decomposition of organic impurities, was observed over the temperature range of the experiment. The ion chromatographic analysis showed (Fig. 1) that : (1) Cl(-)leaching from the PTFE test piece continued even at 340℃ ; (2) F(-) leaching was small in amount and completed within the first step at 200℃ and 12 hours ; (3) F(-) formation due to partial decomposition of PTFE became measurable from 340℃, was linear with time, and was remarkably accelerated at temperatures above 400℃ ; (4) migration of the enclosed NaCl (partly hydrolyzed during the course of the experiment) did not occur even at 410℃ ; but (5) osmosis of water caused a puncture of the test piece within 2 hours after the temperature reached 410℃. The test piece recovered after the run was found to be retaining the original luster and hardness.

Abstract Histamine releasse from basophils induced by house dust and C. albicans was examined in 30 patients with bronchial asthma. House dust and C. albicans caused a significant amount of histamine release in subjects with a RAST score to corresponding allergen. A close correlation was found between house dust- and anti-IgE-induced histamine release. However, histamine release induced by C. albicans was considerably different from the release induced by anti-IgE.

Title Alternative Spa therapy for patients with chronic obstructive lung disease

Abstract Thirty-six patients with chronic obstructive lung disease (34 cases with bronchial asthma, one case with chronic bronchiolitis and one case with pulmonary emphysema) have received spa therapy. Clinical effects of sa therapy on patients with bronchial asthma depended on patient age and asthma types classified by allergic reactions and clinical symptoms. Spa therapy was effective in the cases with ages more than 31 years and the cases with non-atopic type of bronchial asthma. Regarding asthma type classified by clinical symptoms, spa therapy was more effective in the cases with bronchiolar obstructive type and the cases with bronchospasm + hypersecretion type than in the cases with bronchospasm type of bronchial asthma.
