| Abstract | Oral fluoropyrimidines (FUs) have certain advantages over intravenous FUs, such as longer intervals between outpatient visits, no requirement for central venous port (CVP) implantation, and lower incidence of neutropenia. We previously reported the efficacy of S-1/oxaliplatin (SOX) with bevacizumab therapy as a first-line treatment for advanced colorectal cancer (CRC) in a prospective phase-II multi-institutional clinical trial (HiSCO-02 study). However, our prognostic data at the time lacked a sufficient observation period. Herein, we analyze the longer-term follow-up data, focusing on the status of eventual CVP implantation via an open-label, non-randomized, multicenter study. This study enrolled 55 patients (mean age, 64 years), of whom 43 died (41 of primary cancer). The median overall survival was 22.7 months (95% CI: 20.1-34.7 months). Post-treatment regimens after failure of first-line treatment were initiated in 43 patients; CPT11-based regimens were selected in most cases, and other oral FU combinations in nine. CVP was implanted in 35 patients prior to first-line treatment; eleven of the remaining 20 patients did not require CVP implantation. In conclusion, we report here the final prognostic update of the Phase II clinical trial examining the efficacy of SOX plus bevacizumab therapy, the results of which confirm the clinical efficacy of this regimen. |

| Abstract | 本論文では，塚本千秋先生がこれまで相談室紀要に執筆された論文をレビューした。それぞれの論文から，①クライエント・セラピスト双方に流れる時間を意識する重要性，② DV被害者という困難なケースに相対する際に構造を安定させる不断の努力が必要であること，③事例報告において各回のセッションを記述する作業それ自体が臨床研修として有用であること，④助言を行う前にまず目の前のクライエントその人の思いに焦点を当てること，⑤面接の中で思いついた介入を十分吟味すること，⑥試行カウンセリングの臨床現場とは異なる特殊性，⑦精神科医療の動向に，我々心理臨床家が注意を払う必要性，⑧不祥事や倫理的問題を自分ごととして考える重要性，⑨研修における協議が活発になるような架空事例作成の工夫，⑩14回の講義に通底する先生の思想や哲学など，を学ぶことができる。 |

| Abstract | 本稿では，私立幼稚園における保育心理臨床業務の一つとして行った園内研修の実践について，その概要とアンケート結果を報告する。園内研修の対象者は幼稚園教諭８名で，新版K式発達検査の概要と，園児の発達検査の結果を共有する形で行った。アンケートでは，＜発達検査の理解の深まり＞＜子ども理解の深まり＞＜今後の研修の在り方＞の３つの大カテゴリーが抽出され，検査を理解することで，結果を通した子ども理解が深まること,クラス担任それぞれが検査結果を通して子ども理解を深めるのみならず，園全体で検査結果を子ども理解に役立てる視点が重要であることが示された。 |

| Abstract | キャリアのある医療従事者は，多様な課題を抱え職場適応に困難を感じ，職場定着しづらい場合があるものの，組織的な支援については，ほとんど検討されていない。そこで本研究では，キャリアのある新任医療従事者へ心理教育“サクセスフル・セルフ”を活用した研修を行い，本取り組みの意義等について検討することを目的とした。対象は，A 医療機関に採用されたキャリアのある新任職員のうち，本研修会に全回参加した14名である。研修会に関するプロセス評価分析を行った結果，各セッションおよび研修全体における本研修の目標到達度もしくは役立ち度および感想が示され，研修１回目から２回目の質問において同僚の輪にうまく入る，困難に打ち勝つ自己効力感，他者との関係性に関する自己効力感の肯定的変化が見られた。以上より，本研修会の有用性が示唆された。 |

| Abstract | 貧困や虐待など，子どもを取り巻く危機状況の課題の一つに，「当事者自身が置かれている状況を問題として認識していない」ことが挙げられる。そこで本研究では，児童生徒の危機と，児童生徒の危機への認知能力について教職員がどのように評価しているか調査した。その結果，教職員の視点から「危機や課題を児童生徒自身が認識できている」と判断されるのは半数以下にとどまっていることが明らかとなった（研究Ⅰ）。さらにそれを踏まえ，認知的発達が十分ではないと推測される小学生を対象に，貧困などの「不利益場面」をどの程度認知できるか架空事例を用いて検討したところ，低学年ほど危機について正しい認識ができていないこと，危機察知が低い傾向にある児童は，他者へ援助を求めることの必要性を感じにくいことが示唆された（研究Ⅱ）。 |

| Abstract | よい臨床家とはどのような臨床家だろうか。2023年夏，当大学院の修了生たちに講話を依頼されたので，その機会をとらえて，よい臨床家について考えてみた。考える素材を得るために，最近，関心を集めている生成型人工知能ChatGPTに「よい臨床家とはどのような臨床家か」という問いを投げかけたところ，間違いとはいえないが，とても違和感の強い回答を得た。本稿では，その違和感に焦点を当てながら，筆者の考える“よい臨床家”を紹介したい。 |
