| Abstract | An 86-year-old man was admitted to another hospital with ataxia and general fatigue. Brain MRI scan revealed a high-intensity area in the bilateral thalamus, and he was referred to Okayama University Hospital for detailed examination. Cerebrospinal fluid (CSF) and blood tests showed no evidence of infection or autoimmune encephalitis. An FDG-PET/CT scan revealed systemic lymphadenopathy, and cervical lymph node biopsy showed classical Hodgkin lymphoma. Neurological symptoms were suspicious of paraneoplastic neurological syndrome (PNS), but onco-neural antibodies were not detected in either serum or CSF. He was referred to our hospital, Okayama Red Cross Hospital, for treatment and administered 6 cycles of an ABVD regimen, and an FDG-PET/CT scan showed a complete remission. The brain lesion disappeared and neurological symptoms were improved, so he was diagnosed with PNS. PNS is rare complication associated with malignant lymphoma and early diagnosis is important because neurological symptoms can be improved with anti-cancer treatment. |

| Abstract | Vascular-access interventions are necessary for the continuation of hemodialysis, and they are performed under X-ray guidance. During interventions, patients’ accidental falls from the bed are a serious problem, and spe-cialized fixation systems for hemodialysis patients to prevent their falls from the bed have been lacking. We developed a new fixation plate made of polypropylene homopolymer and tested its ability to prevent such falls retrospectively. This plate, which we named the ‘vascular-access intervention assistance plate,’ offers functional features such as the concurrent fixation of the body and either arm and an arm space with serrations for fixing a forearm strap. We performed computer simulations to examine the strength of the plate, and we evaluated the efficacy of fall prevention by reviewing patients’ medical records. The results demonstrated that the functional design of the plate provides good operability via accurate concurrent fixations of the body and arm. The com-puter simulation analysis results indicated the plate’s sufficient strength. The medical records analysis revealed three accidental falls before the plate’s introduction (401 patients, 1,437 interventions), and none after plate introduction (683 patients, 1,872 interventions). Accidental falls were significantly prevented by use of the plate (p < 0.05). The dementia rate and type of procedure were not significantly different between the patients who fell and those who did not. This vascular-access intervention assisted plate provides good operability and safety by preventing accidental falls among hemodialysis patients. |

| Abstract | In this note, we establish an analog of the Mallows-Sloane bound for Type III formal weight enumerators. This completes the bounds for all types (Types I through IV) in synthesis of our previous results. Next we show by using the binomial moments that there exists a family of polynomials divisible by three, which are not related to linear codes but are invariant under the MacWilliams transform for the value 3/2. We also discuss some properties of the zeta functions for such polynomials. |

| Abstract | Let (A, m) be an excellent normal domain of dimension two. We define an m-primary ideal I to be a pg -ideal if the Rees algebra A[It] is a Cohen-Macaulay normal domain. If A has infinite residue field then it follows from a result of Rees that the product of two pg ideals is pg . When A contains an algebraically closed field k ∼= A/m then Okuma, Watanabe and Yoshida proved that A has pg -ideals and furthermore product of two pg -ideals is a pg ideal. In this article we show that if A is an excellent normal domain of dimension two containing a field k ∼= A/m of characteristic zero then also A has pg -ideals. |

| Abstract | Let G be a finite group and let V and W be real G-modules. We call V and W dim-equivalent if for each subgroup H of G, the H-fixed point sets of V and W have the same dimension. We call V and W are Smith equivalent if there is a smooth G-action on a homotopy sphere Σ with exactly two G-fixed points, say a and b, such that the tangential G-representations at a and b of Σ are respectively isomorphic to V and W . Moreover, We call V and W are d-Smith equivalent if they are dim-equivalent and Smith equivalent. The differences of d-Smith equivalent real G-modules make up a subset, called the d-Smith set, of the real representation ring RO(G). We call V and W P(G)-matched if they are isomorphic whenever the actions are restricted to subgroups with prime power order of G. Let N be a normal subgroup. For a subset F of G, we say that a real G-module is F-free if the H-fixed point set of the G-module is trivial for all elements H of F. We study the d-Smith set by means of the submodule of RO(G) consisting of the differences of dim-equivalent, P(G)-matched, {N}-free real G-modules. In particular, we give a rank formula for the submodule in order to see how the d-Smith set is large. |
