Abstract 「平成30年7月豪雨」は,西日本を中心に多大な被害をもたらした。大災害で被災した子どもたちの心は長期にわたって影響を受けることが知られているが,子どもたちへの支援を考える上では,その保護者への支援も欠かせない。本研究では,平成30年7月豪雨の際に特に被害の大きかった地域において,被災から4か月後の11月に,子育てに支援が必要な保護者を対象としてペアレント・プログラムを実施すると共に,情報交換会を行った。全6回のプログラムを実施した結果,大半の参加者の抑うつ得点が低下した。今後,有事の際での事後的な支援対応だけではなく,平常時から孤立しがちな保護者への支援体制を予防的に構築しておくことが求められる。

Abstract 2019年12月発生した新型コロナウイルス感染症はパンデミックに拡大し、世界中の社会・経済、医療現場や日常生活に甚大な影響を及ぼしている。多くの大学でオンラインによる遠隔授業が取り入れられ、本学でもLMS(Learning Management System)や遠隔会議システムを活用し、全額でオンライン授業が実施された。本学のCTE(Center for Teaching Excellence)部門は、オンライン授業実施のための支援に加わり、年間FD研修プログラム等を通して有益な情報共有に取り組んだ。本稿では、FDの取り組みや、オンライン授業の工夫、またそれらを通して教員としてどのような発見や成長があったのか、CTE部門の教員が述べる。

Abstract Polymorphisms in methotrexate transporter pathways have been associated with methotrexate toxicities and clearance. Recent genome-wide association studies have revealed that the SLCO1B1 T521C variant is associated with methotrexate elimination. We present a case of a pediatric patient with acute lymphoblastic leukemia who suffered from persistently high plasma methotrexate concentrations and acute kidney injuries after the admin-istration of a medium dose of methotrexate. Subsequent genetic analysis showed that he was a carrier of dys-functional genetic variants associated with methotrexate clearance. This case highlights that polymorphisms of methotrexate transporter pathways can adversely affect methotrexate elimination in a clinically significant manner.

Title Alternative Chylothorax in a 3-month-old boy with respiratory syncytial virus infection

Abstract  Chylothorax is a condition in which chyle leaks into the thoracic cavity. The causes of chylothorax can be nontraumatic, traumatic or idiopathic. The most common cause of nontraumatic chylothorax is obstruction of the thoracic duct by tumor. Traumatic chylothorax is caused by disruption of the thoracic duct, most often due to cardiac surgery or a thoracic surgical procedure, although in rare cases by cough or vomiting. We report the case of a 3-month-old boy with respiratory syncytial (RS) virus infection who presented with right chylothorax. No obstructive tumor mass was identified in the thoracic duct. Clinical symptoms of Noonan syndrome and Down syndrome, which often accompany lymphatic anomaly, were not found. Therefore, we speculate that his severe cough caused a high thoracic pressure and a disruption of the thoracic duct. He was treated by thoracocentesis and MCT milk. One month later, pleural effusion disappeared and did not recur. RS virus infection might be a rare cause of chylothorax in infants.

Abstract To clarify the relevance of prolactin (PRL) to clinical parameters in patients who visited our general medicine department, medical records of 353 patients in whom serum PRL levels were measured during the period from 2016 to 2018 were retrospectively reviewed. Data for 140 patients (M/F: 42/98) were analyzed after excluding patients lacking detailed records and patients taking dopaminergic agents. Median serum PRL levels were significantly lower in males than females: 6.5 ng/ml (IQR: 4.2-10.3) versus 8.1 ng/ml (5.9-12.9), respectively. Pain and general fatigue were the major symptoms at the first visit, and past histories of hypertension and dyslipidemia were frequent. Male patients with relatively high PRL levels (≥ 10 ng/ml) had significantly lower levels of serum albumin and significantly higher levels of serum LDH than those with low PRL (< 10 ng/ml). There were significant correlations of male PRL level with the erythrocyte sedimentation rate (R=0.62), serum LDH level (R=0.39) and serum albumin level (R=−0.52), while the level of serum CRP (R=0.33) showed an insignificant but weak positive correlation with PRL level. Collectively, these results show that PRL levels had gender-specific relevance to various clinical factors, with PRL levels in males being significantly related to inflammatory status.
