Abstract Since the report of the agglutination test with Streptococcus by Cecil in 1931 there appeared many reports dealing with the serum reaction involved in rheumatoid arthritis. Among these the modified form of sensitized sheep blood cell agglutination test as proposed by WAALER (1940) and ROSE (1948) had been for a long time the only method applied to the diagnosis of rheumatoid arthritis. However, as this method had disadvantages of its complex manipulation and of being relatively sensitive, generally satisfactory results could not be obtained with it. Since then the latex agglutination test by PLATZ and SINGER (1957) was used in conjuction with the bentonite agglutination test by BOZIEVICH (1957). By the RA-test it means the test where agglutination is attained by the combined reaction of the human serum to be tested and the polystyrene latex granules bound to human gamma-globulin, and it is being widely used in daily clinical examinations. For the purpose to know the positive rate of this test in disesaes other than rheumatoid arthritis, we applied this test to the patients with various diseases both visiting our out-patient clinic and those hospitalized, and studied the results.

Author Ezawa, Hidemitsu|
Abstract There was a time when the adrenocortical hormones therapy was only available treatment for rheumatoid arthritis, but since it does not give lasting effect on chronic rheumatoid arthritis, opinions are varied as to its efficacy, so that at present we are still conducting studies about this problem. Ever since the reports on the gold therapy for rheumatoid arthritis of LANDE (1927) and FORESTIER (1929) the literature is replete with the results of gold therapy both in Europe and America. In Japan, however, it was with reports by OSHIMA (1957) and HASHIMOTO(1961) that first aroused the interest of general investigators in this gold therapy. Nevertheless, since we find no convincing methods of treatment, we attempted to give combined balneo- and chryotherapys therapy to the pateints coming to the Department of Internal Medicine of Misasa (Hot-springs) Hospital, which is located in Tottori Prefecture and is a Branch of Okayama University Hospital, over the period of the past five years, and studied the efficacy of such treatment. Our subjects were consisted of 93 cases of rheumatoid arthritis with duration of 4.8 years in average. For the balneotherapy the patients were made to rest for the first the bathing week, the combined with mud treatment and massage once or twice a day was given in the second week, and then the gold therapy was commenced. Gold thioglucose oil emulsion (Solganal B. product of Schering) was used. In the first week, 5-10 mg of Solganal B were given twice. 10 mg were given twice in the second week, and thereafter the doses were gradually increased until the total dosage reached 1.0~1.5g. By the combined balneo- and chrysotherapy applied to the 93 cases of rheumatoid arthritis, we obtained the results as follows. 1. The improvement in the erythrocyte sedimentation rate was observed in 77% of them. 2. The improvement in the CRP test was observed in 46%, but the aggravation was found in 8.7% of their conditions. 3. There were 53 cases (57%) whose RA-test was either weakened or turned negative. 4. Judging from the changes in the erythrocyte sedimentation rate and the doses of gold thioglucose given, the effects of such a treatment appear around the tenth week after the start of the treatment and it seems that the total dosage of 500 mg and over gold salt is most appropriate. 5. By the combined treatment there were 10 (48%) of 21 cases whose conditions had improved well enough as to withdraw corticosteroids treatment and remaining 11 cases (52%) who had improved to the extent where we could reduce the amount of corticosteroids. 6. By applying the evaluating system of HASHIMOTO, the efficacy rate of the treatment proves to be 89% in the cases given over 500 mg gold thioglucose, and out of these 49% showed a marked effect. Therefore, it seems that the optimal dosage is over 500 mg. 7. These results amply indicate that for the treatment of chronic rheumatoid arthritis the balneotherapy combined with some organic gold compound or salt (Solganal B in the present study) is highly beneficial.

Abstract Human venous blood pH, P(CO(2)) and P(O(2)) were measured with I. L. Meter. Its reproducibility and response rate were evaluated as well as methods of the procedures. 1) Time required to reach the stability of pH, P(CO(2)), and P(O(2)) reading after the sampie injection was 1~1.5 min., 2~2.5 min. and 45~75 sec., respectively. It is recommended in the measurement of these parameters at the same time that the sample injection starts with P(CO(2)) electrode, followed by pH and P(O(2)) in this order and that readings are recorded in the order of pH, P(O(2)) and P(CO(2)). 2) Range of differences between the two values measured in the interval of 3~5 min. were pH : -0.010~0.020 (mean: 0.003), P(CO(2)) : -1.0~1.0mmHg (mean: 0.4) and P(O(2)) : -1.0~0.0mmHg (mean: -0.5). Their 5% rejection limits were 0.021≧x(o)≧-0.015, 2.0≧x(o)≧-1.2mmHg and 0.3≧x(o)≧-1.3mmHg, respectively. 3) The pH, P(CO(2)) and P(O(2)) of the heparinized venous blood stored in ice water showed no significant changes in 60 min. and they gave practically the same results as the measurement just after shedding.

Abstract The CO(2)-H(2)O isotopic equilibration technique was studied for the routine analyses of the oxygen isotopic ratios of hot spring water. A reaction vessel containing 2 ml of water and 0.16 m mole of tank CO(2) was shaken for 18 hrs. in a constant-temperature bath at 25.0℃ (Figs. 1, 2, and 3), and the CO(2) was analyzed for the oxygen isotopic ratio by a MCKINNEY type mass spectrometer. Several aliquots of 1, 2 and 5 ml from a same water were each analyzed three times by successive equilibration (Table 1). The observed values differ depending on the volume of water but the corrected values by equation (6) indicate excellent agreement, implying the whole processes to be well controlled. The reproducibility of the isotopic analyses is better than ± 0.1‰ (Table 2) in most cases, and the accuracy would not be worse than ± 0.2‰ as demonstrated by the interlaboratory comparison of some standard samples (Table 3). Oxygen isotopic ratios of water from more than 70 hot springs in Japan are presented (Table 5). Although the results will be discussed in the following issues of this series of paper, most hot spring water have the δ(18)O values similar to those of meteoric waters in Japan. However, spring water from Arima Hot Springs, Hyogo-Pref., which has been known by its abnormally high chloride and low sulfate concentrations is of an exceptionally high δ(18)O value.

Title Alternative Studies on the uptake of (75)Se-selenomethionine by the pancreas Attempt to augment by the effects of protocols
FullText URL pitsr_040_019_031.pdf
Author Tokunou, Tsuneo|
Abstract Since the introduction of the pancreatic scanning using (75)Se-selenomethionine, there has been various protocols proposed to increase the pancreatic uptake of this isotope. In this experiment several protocols; as diets high carbohydrate meal, high protein meal, high fat meal, and as medicaments pancreozymin, secretin and phydroxyphenylsalicylamide (PHPS), were tested for determining the effects on the uptake of (75)Se-selenomethionine by the pancreas and the liver of the dog. The animals given protocols were sacrified at each of a number of selected times to compare P/L ratio on a percentper-gram concentration of (75)Se-selenomethionine and the total organ concentration of (75)Se-selenomethionine in the pancreas and in the liver. A controlled group of animals were fasted for 24-hour and sacrificed at each of same times. On P/L ratio basis it was found that animals with high carbohydrate meal had better ratio than those with no preparation during first 4-hour post administration. However, animals with each of high protein meal. fat meal and PI-IPS had lower ratio than those with no preparation. On the blood concentration, animals with each of high carbohydrate meal and secretin had lower variation of blood concentration than controlled group. Thus, from the results mentioned above, the protocol by high carbohydrate meal is proved to be useful in pancreatic scanning.

Title Alternative Clinical studies on electromyographic changes in rheumatoid arthritis Part III. Effect of radioactive hot spring baths
FullText URL pitsr_040_001_017.pdf
Author Ikegami, Tadaoki|
Abstract The author observed the changes in electromyographic patterns of 14 patients with rheumatoid arthritis (13 cases of classical rheumatoid arthritis and one case of definite rheumatoid arthritis) bathed 2 or 3 times daily in hot spring water at Misasa Spa, at a temperature of 40±1℃, 5-10 minutes. The electromyographic examination was performed on the biceps brachii, opponens pollicis and the abductor pollicis brevis. Intramuscular temperature was measured in the deltoid muscle. The results were as follows. 1) The intramuscular temperature in cases of rheumatoid arthritis did not deviate from normal subjects. 2) After the general spring bathing the intramuscular temperature increased about 1℃ and it had no difference between the patients with rheumatoid arthritis and normal subjects. 3) With increasing intramuscular temperature by the general spring bathing in cases of active rheumatoid arthritis the mean potential amplitude decreased and the mean potential duration increased. 4) With increasing intramuscular temperature by the general spring bathing in cases of inactive rheumatiod arthritis the mean amplitude increased and the mean duration decreased. 5) The patterns of potential duration and amplitude before and after a bath on re-examination, one month after a series of the general spring baths, showed a tendency of reverse of the initial patterns. 6) After two months of a series of the general spring baths, the patterns of electromyographic changes of patients with rheumatoid arthritis following the bathing were similar to the initial patterns.

Title Alternative The effect of pH of water on the measurement of oxygen isotopic ratio by means of CO(2)-H(2)O isotopic exchange technique
FullText URL pitsr_041_001_002.pdf
Author Matsubaya, Osamu|
Abstract The effect of pH of water on the measurement of oxygen isotopic ratio by means of CO(2)-H(2)O isotopic exchange technique was studied. In this study, a new type of reaction vessel was used (Fig. 1). The reaction time for equilibration using this new reaction vessel was nearly equal to that in the previous study (Fig. 2). The oxygen isotopic ratios of each two samples of pH 2, 4, 7, 8, 9, and 10 were measured. There is no significant difference between the observed δ(18)O values of these samples (Table 1). Therefore it is concluded that the effect of pH of water needs not to be taken into account on the measurement of oxygen isotopic ratio by means of CO(2)-H(2)O isotopic exchange technique.

Title Alternative The involvement of ear and throat in rheumatoid arthritis II. On the Rheumatoid Arthritis of Cricoaryenoid Joint
FullText URL 047_025_032.pdf
Author Oda, Rei|
Abstract Until 1955 a few reports had been made concerning this problem. On December 1955. Montgomery, Perone and Schall reported four cases of rheumatoid arthritis of cricoarytenoid joint. It seemed to have interested other authors so that Saunders, Pearson, BakerBywaters, Copeman and Polisar reported their clinical findings. In 1957 Pearson presented a histological section of cricoarytenoid joint, showing villous synovial proliferation. In 1959 Montgomery reported another series of additional cases and microscopical findings of the cricoarytenoid joint of two patients with rheumatoid arthritis. This study did not show a complete fibrous ankylosis but severe arthritic changes were seen. In the same year Copeman et al. reported an autopsy case of rheumatoid arthritis with ankylosis of cricoarytenoid joint. In early 1960's Grossman et al. examined 55 patients with rheumatoid arthritis and found 18 patients with some suggestive evidence of cricoarytenoid arthritis. Lofgren and Montgomery examined 100 patients of rheumatoid arthritis, finding 26% of them had involvement of cricoarytenoid joint. They stated that this disease seemed to be more common and more severe in female. Bienenstock stated 17 cases of 64 randomly selected patients with rheumatoid arthritis had one or more symptoms considered to be characteristic of cricoarytenoid arthritis. He claimed that arthritis of cricoarytenoid joint occured much more frequently in patients with rheumatoid arthritis than had been suspected. At postmortem examination Grossman saw histological changes of cricoarytenoid joint characteristics of rheumatoid arthritis in 5 of 11 autopsied patients with rheumatoid arthritis and Bienenstock found the same in 7 of 8 patients. In 1963 for the third time Montgomery discussed of cricoarytenoid arthritis, based on his own experiences and many available literatures. Pathologic changes range between the mere synovial thickning and the complete disruption. Montgomery believed as others did, that in ankylosing chronic stage inspiratory bowing of both vocal cords and absolute fixation of arytenoid cartilage were diagnostic signs of rheumatoid arthritis of cricoarytenoid joint. In 1958 Darke, Wolman and Young reported five cases of laryngeal stridor, where in four cases tracheotomy became necessary. Two of them were brought to autopsy and histological examination. They found evidence of the nerve degeneration and mobile cricoarytenoid joint. The cause of nerve degeneration in the second case was an ischaemic neuropathy from rheumatoid arteritis of vasa nervorum. These findings were different from those of Montgomery and others. In 1965 Wolman, Darke and Young had an opportunity of six more autopsy examples. They confirmed from their own pathologic findings that laryngeal stridor occuring in rheumatoid arthritis was due mainly to arteritis of the vasa nervorum causing ischaemic neuropathy and that polymyositis and joint disease might augment this effect. Woldorf and Webb et aI. introduced both of these opinions. Anyhow, in U. S. A. and in England 25%-28% of rheumatoid arthritis seems to have rheumatoid arthritis of cricoarytenoid joint. On the other hand in our country there has been seen no report on rheumatoid arthritis of cricoarytenoid joint. In order, therefore, to investigate this problem author examined in medical clinic of Misasa branch hospital of Okayama university school of medicine 76 patients of classical and definite rheumatoid arthritis. Their complaints were as follows. 1) dryfeeling of throat 17 (22.4%) 2) slight hoarseness 11 (14.5%) 3) choking by long conversation 1 (1.32%) 4) slight dysphagia 2 (2.6%) The larynx was mirrored indirectly by Türk's, Killian's and Avellis's position. Any redness, swelling of mucous membrane, abnormality of vocal cords and arytenoid cartilage were not found.
