| Abstract | Mutation is one of the important operators in genetic algorithm. In traditional genetic algorithm, mutation is activated stochastically. In this way it is unknown and cannot be controlled for which individuals to be mutated. Therefore, it is unavoidable that some good individuals are destroyed by mutation and then the evolutionary efficiency of the genetic algorithm is dampened. Owing to this kind of destructivity of mutation, the operator of mutation has to be limited within a very small probability, and the potentiality of mutation is consequently limited. In this paper, we present an evolutionary chain-based mutation and a control strategy of reasonable competition, in which the heuristic information provided by the evaluation function is well utilized. This method avoids the blindness of stochastic mutation. The performance improved in this method is shown by two examples, a fuzzy modeling for the identification of a nonlinear function and a typical combinatorial optimization problem-the traveling salesman problem. |

| Abstract | This paper proposes a concept of a mobile type maintenance robot (MMR in shortly) that is controlled by the tele-operation for assuring the safety and health improvement of work forces. The main components of this system are the industrial robot, an image processing unit, vehicle and computers for tele-operation. Our focus is paid on a method that determine a pathway to move every places where troubles would occur. This method makes it possible that the MMR could arrive any place in the factory with three times of turns at most. Additionally, turning radius of the vehicle is considered for correcting the pathway near the corners to make it possible that it arrives to the place accurately. |

| Abstract | Fresnel zone plate lens (FZPL) has widely been used in electromagnetic antenna applications. Most analysis method based on the potential (scalar) wave approximation has been applied to a few very limited and simplified cases. The present paper analyzes the FZPL in more general form including the diffraction and transmission using the method of moments (MoM). The focusing gain characteristics in the oblique incidence as well as in the normal incidence are considered. The MoM solution using the three-dimensional vectorial formulation requires a large memory space for the FZPL as it is operated at a short wavelength. This is simply overcome by using an iterative conjugate gradient method for the numerical evaluation. The MoM solutions are compared with the other solutions. |

| Abstract | In electric power industry, there is a problem of achieving stable joint in different components using high productive and efficient technologies. One type of these components is packages of slices for magnetic circuit of electric motors, transformers etc., which need reliable means for their fixing. Laser welding is proposed to solve this problem as an alternative for existing technologies. The development of the laser welding process is presented based on process simulation, study of heat history and comparison with experimental results. Laser beam additional scanning technique is proposed to improve the quality and efficiency of the joining operation. |

| Abstract | 今まで内科的治療が主体であった肺気腫に対して,近年外科的治療(Volume Reduction Surgery 以後VRSと略す)が行われるようになった｡VRSを受ける患者の看護では,手術前後において, リハビリテーション(以後リハビリと略す)や機能回復に向けてのケアが必要不可欠とされる。そこで,本研究は肺気腫患者の適切な看護援助を検討するために,手術前後に示す肺気腫患者のリハビリや活動に対する反応を明らかにすることを目的とした｡対象者は当病棟に入院している肺気腫患者8名(VRS手術前8名,手術後はそのうちの6名である)で, リハビリ, 日常生活活動,呼吸,手術などについて,面接ならびに観察による調査を行った｡分析の結果,手術前においてリハビリでは≪義務感≫,頑張って行えば呼吸が楽になるという≪期待感≫,≪サポート≫,≪不安≫が,活動では≪活動の制限≫,手術では呼吸が楽になるのではという≪期待感≫,≪おまかせ≫,≪いちかばちかの賭け≫,≪不安≫,≪回復に向けての欲求の高まり≫が明らかになった｡手術後においてリハビリでは≪呼吸が楽になるための手段≫が, 日常生活活動では≪今の状態よりは良くなると いう期待感≫が,手術では≪達成感≫ と≪身体的苦痛≫が,将来については≪ささやかな欲求≫が明らかになった｡ |

| Abstract | 口腔腫瘍術後患者は,程度に差はあるが,嚥下･摂食障害をきたしやすい｡その経口摂取開始初期の栄養について,患者が満足して,より快適に摂取でき,経口のみで栄養が満たされるような食事を工夫した｡そのためには,食べやすさを増すためにトロメリンでとろみをつけ, また,食べ残しがないように患者の嗜好を取り入れた｡そして,この嗜好を取り入れたとろみ食を, 口腔腫瘍術後患者一例に提供した｡その結果, とろみ食は,食べやすく,全量摂取でき,良好な栄養状態を維持し,口内の移植皮弁へのトラブルもなく有効であるという結果が得られた｡また,患者が意欲や自信を持つという精神面への効果も見られた｡ |

| Abstract | 食道癌患者は腫瘍による狭窄のため,程度の差はあるものの,術前より通過障害を認めることが多い｡食道癌と診断され,手術を自己決定した患者の多くは,手術をして罹患前のように食事ができるようになりたいと思っている｡しかし,患者の多くが術後に嚥下感覚の変化に遭遇する｡現在,手術前に実施されるインフォームドコンセント(以下IC)では,術後に起こる嚥下感覚の変化については説明が行われていない｡そのため患者は,経口摂取が可能になった時点で嚥下感覚の変化を自覚し,強い不安を感じる｡今回, 5人の患者に術後の嚥下感覚の変化に関するインタビューを実施するとともに,術後の嚥下活動を頸部電気インピーダンスおよび嚥下音を用いて測定した｡インタビューの結果, 5人全てが,術後に嚥下感覚の変化を自覚し,不安を感じていた｡また,青年健常者と比較し,IPG波形の多相化と,嚥下音の咽頭期以外での発生が認められた｡患者が,術後に起こる嚥下感覚の変化についてのICを望んでいることからも,術後の嚥下感覚の変化についてのICを行なう必要がある｡ |

| Abstract | 食道がん患者が食道切除術を受けることを自己決定する場合には,食道癌であることの告知と､食道癌に対する治療方法に関するインフォームド･コンセントが重要となる｡食道がん患者の治療法選択における自己決定を指向したインフォームド･コンセントの有り様について,質的帰納的に分析した文献は少ない｡今回,食道がん患者に対して,半構成的質問用紙を用いて,面接を実施することで,患者が治療法として手術を自己決定するために役立つインフォームド･コンセントとはどのようにあればいいかを明らかにした｡その結果,患者が食道切除術を受けることを決めるためには,①患者の理解度にあわせ,何度かに分けてインフォームド･コンセントが行われる必要があること,②術後に起こってくる症状,特に患者にとって苦痛を伴う検査,処置等については,患者のパーソナリティーを考えながら,術後のイメージがわくような説明が必要であること,③患者の自己決定に至る過程のみならず,インフォームド･コンセント後においても,患者や家族を支援する体制が重要であること,が明らかになった｡ |

| Abstract | 岡山大学医学部附属病院に入院中で,主に消化器系の疾患を持ち手術を受けた患者,主に消化器内で治療中の患者,主に血液内科で治療前･治療中の患者,主に腎･内分泌内科など慢性疾患の治療を受けている患者で,1週間以上入院しており,了承の得られた92名の患者を対象に,自己申告による病院食の摂取量とその関連要因について調査した結果以下の4点が明らかになった｡ 1.病院食の摂取量が半分以下であると答えた患者は約40%であった｡ 2.病院食の摂取量は,治療方法,身体症状と強い関連がみられた｡ 3.病院食の摂取量が多い者は食事の時間を楽しく感じ,食事にも満足していた｡ 4.食事に対する知識･興味･行動･有益性と病院食の摂取量との間には有意な関係はみられなかった｡ |

| Abstract | 本研究の目的は,1994年～2003年9月までに発表された手術を経験する子宮がん患者の看護実践領域における研究について,文献件数および研究領域の観点からその動向を概観し,今後の研究課題を明らかにすることである。JMED,CINAHL,MEDLINEの3つを文献情報データベースとして用い,｢子宮がん｣and｢看護｣and｢子宮摘出術｣および｢uterine cancer｣and｢nursing｣and｢hysterectomy｣の検索式により文献集合を絞り,原著論文に限って検討した｡その結果,以下の9つの課題, 1)行った研究を誌上発表する 2)子宮がんの早期発見･予防,ターミナル期に焦点を当てた研究 3)術後合併症に注目した研究 4)患者のもつ力に注目し,それを引き出し,高めていくことを支援する看護に関する研究 5)術後のセクシュアリティに関連した研究 6)退院指導プログラムの開発およびクリティカル･パスの作成 7)子宮がん患者の自己効力感に関連した研究 8)cancer survivorに関する研究 9)介入研究,が明らかになった｡ |

| Abstract | 研究目的は女性生殖器がん患者の家族に対する思いを明らかにし,患者に対する看護援助について考察することである｡対象は女性生殖器がんと診断された後,手術を経験した入院患者11名｡半構成的面接にてデータ収集を行い,内容の分析は質的分析帰納的方法に基づいて行った｡女性生殖器がん患者の家族への思いについては『家族ががんの病気に自分と同じように衝撃を受ける』『心配をかけないために家族を気遣う』『家族関係が密になる』『病気に立ち向かう力を家族から与えられる』『家族に対する考えが変化する』のカテゴリーに分類された｡罹患の体験から関係を見つめなおし,家族関係の親密さを増す｡家族からの励まし,理解と支援を受けることによって患者は病気に立ち向かう力を家族から与えられ,治療に臨む｡看護者は患者と家族双方の衝撃を理解し,罹患をきっかけとした家族関係の変化が患者にとって治療の意欲につながるよう援助すること,家族内役割調整が進むようチーム体制で援助する必要性が示唆された。 |

| Abstract | 当科では3年ほど前よりQCT法を用いて骨塩量測定を行い,骨粗鬆症の管理治療を行っている｡264名の測定値について分析をしたところ,40歳頃から60歳にかけて急速に低下すること,肥満度が増すにつれ骨塩量も増加する傾向がみられた｡また,閉経前に卵巣を両側摘出した場合ホルモン補充療法を行っているが,卵巣を温存した場合と差がでなかった｡治療に結合型エストロゲンを用いた群の12カ月後の骨塩量の増加率は,任用しなかった群にくらべ有意の差があった｡ |

| Abstract | This paper examines to what degrees employees' work contribution to Japanese companies is currently reflected in their retirement program, by using interview as research method (July−Dec., 2008 ; on eight companies and two public offices). It also examines the content of the programs, such as social environment surrounding the system, company policies, and problems and solutions involved in the operation, as well as the transition of the system after Meiji era. Major findings include the followings. Firstly despite some changes occurred in the system as a result of diversification of employment types and styles and also alteration of company policies, the system itself will be maintained in the future. Secondly, although many companies modified conventional seniority−based salary systems for a retirement program that reflect employees' work contribution more clearly through an adoption of 'point system' and other measures, they still find some problems in new systems in their operation and practicality. |

| Abstract | 膵癌の早期診断を行うために,最近の進歩の著しい遺伝子診断を用いて,膵液中のK-ras遺伝子の点突然変異の検討がなされている｡われわれは膵液の細胞診は陰性であるが,K-ras遺伝子の点突然変異を認め,膵全体に多発する膵嚢胞の1例を経験した｡本例は悪性であるとの確診が得られないことや切除するとなれば膵全摘となることなどのために,経過観察を行っているが,18カ月後の現在,嚢胞の増大など認めていない｡膵癌の遺伝子診断の文献的考察を含め,報告する｡ |

| Abstract | During underground space development, groundwater seepage mostly occurs which may seriously affect the stability of deep excavations as well as retard progress of construction. For smooth progress of work, effective construction and operation of a disposal facility for high level radioactive waste (HLW), it is important to control seepage into excavations by sealing off fractures or fissures and excavation disturbed zones (EDZ) to control groundwater inflow during the construction phase of such a facility. In this study, a series of experiments were conducted on ethanol/bentonite slurries in the laboratory with the aim of determining the effect of a hydrophilic solvent such as ethanol on the hydraulic and injection characteristic of bentonite slurry for the sealing of fissures to control ground water seepage during the construction phase of a repository. Preliminary results revealed ethanol/bentonite slurry as an effective grouting material capable of penetrating micro fractures (100μm or less) and with the permeability of the grouted medium being as low as 10E-7cm/s. The results also show that the effectiveness and efficiency of grouting is dependent on the type of injection and fissure size. Dynamic injection was observed to be efficient as it was able to inject high dense slurry. |
