Title Alternative | Regional left ventricular contractile dynamics in hypertrophic cardimyopathy estimated by magnetic resonance imaging

Abstract | To assess the regional myocardial function in hypertrophic cardiomyopathy (HCM), I examined the systolic wall thickening (% WT) and percent change of segmental wall area (% AR) using cine magnetic resonance imaging in 23 normal volunteers (G1) and 40 patients (G2) with HCM. Short axis images of the left ventricle were recorded at the base and the apex, and were divided into 5 segments, and % WT and % AR were measured for each segment. There were no significant differences in % WT and % AR among the segments in G1, while % WT of the posterior septum, posterior and lateral segments in the apex were higher than the according to end-diastolic wall thickness : G2a, ≦12mm ; G2b, 12<≦15 ; G2c, >15. At each slice level, % WT and % AR were highest in G2a and lowest in G2c.
These findings suggest that myocardial shortening in normal subjects is higher in the apex than in the base, and, in HCM, regional myocardial function decreases in association with an advance of hypertrophy, with a possible compensatory increased wall function of normal segments.
