Abstract Traumatic neuroma is an abnormal proliferation of injured nerves resulting from trauma or surgery. We present a case of traumatic neuroma arising in the cystic duct after cholecystectomy. A 66-year-old man was referred to our department due to a biliary tumor. He had undergone cholecystectomy 20 years prior. Cholangioscopy showed an elevated lesion covered with smooth mucosa. Histological examination revealed normal bile duct mucosa. Although benign disease was suspected, the possibilities of malignant disease could not be excluded. Extrahepatic bile duct resection was planned to include intraoperative rapid-freezing of a biopsy specimen followed by histopathological examination. These intraoperative histology results showed proliferation of nerve and fibrous tissue only, resulting in the diagnosis of traumatic neuroma, so no lymph nodes were removed. To avoid excessive surgical intervention, histopathological examination of an intraoperative rapid-frozen biopsy specimen may be important for diagnosing traumatic neuroma.

Title Alternative Why Is Intermediate Organization Necessary?

Abstract  This paper challenges a fundamental question, ‘Why is an intermediate organization necessary?’ Due to transaction costs and market failures on the one hand and the limitations of organizational control mechanisms on the other hand, many‘ intermediate organizations’ are observed in the real world. How can we tackle to explain the governance mechanism considered to be‘ intermediate?’  If we are to discuss such socioeconomic orders, this paper assumes that we should not be able to link micro-level explanations and macro-level ones concerning the third mode of governance mechanisms all at once. We need to stick to the meso-level at fi rst. The theoretical elaboration since Ouchi’s(1980) discussion of clan-type governance and cumulative empirical research on industrial agglomerations have allowed us to construct a more sophisticated theory called community capital.  In effective communities, members are ‘embedded as insiders’ who serve the purpose of the community, share experiences of failures and successes, and find and deepen their common identity. This limited membership is bound by‘ mutual trust to rely on each other’ for‘ distribution of short-term risks.’ In contrast to social norms that need to be abstract enough to be widely shared, the communal norms that are concrete enough to allow the members to understand without hesitation how they should behave in localized contexts are cumulatively cultivated along socializing process. Among the norms, sense of mutual obligation to incur intermittent costs for the whole community is a crucial norm for the sustainable development of the community. However, as a practical matter, membership control, mutual trust and short-term risk allocation may serve the communities in the short run, but they do not guarantee long-term accumulation of shared capital. As a result, the limits of community capital may need to be discussed once again, especially today when market liquidity is increasing, and its failures tend to become more apparent in a variety of areas.

Abstract Severe febrile thrombocytopenia syndrome (SFTS) is a tick-borne infectious disease that is endemic in parts of eastern Asia. Few pediatric cases have been reported. We describe a case of SFTS in a seven-year-old girl who presented with prolonged fever and gastrointestinal symptoms. Leukopenia and thrombocytopenia on hematology, and a history of outdoor activity led us to diagnose SFTS, although the patient had no tick bite marks. We also review the literature and discuss the characteristics of pediatric SFTS. Physicians should consider SFTS in the differential diagnosis of fever with thrombocytopenia in children living in endemic areas.

Abstract Radon is a known risk factor for lung cancer; however, it can be used beneficially, such as in radon therapy. We have previously reported the enhancement of antioxidant effects associated with trace amounts of oxidative stress as one of the positive biological effects of radon inhalation. However, the biological effects of radon inhalation are incompletely understood, and more detailed and comprehensive studies are required. Although several studies have used proteomics to investigate the effects of radon inhalation on body proteins, none has focused on brain proteins. In this study, we evaluated the expression status of proteins in murine brains using proteomic and multivariate analyses to identify those whose expressions changed following two days of radon inhalation at a concentration of 1,500 Bq/m3. We found associations of radon inhalation with the expressions of seven proteins related to neurotransmission and heat shock. These proteins may be proposed as biomarkers indicative of radon inhalation. Although further studies are required to obtain the detailed biological significance of these protein alterations, this study contributes to the elucidation of the biological effects of radon inhalation as a low-dose radiation.

Title Alternative Firm Entry and Exit in the First Stage of Regional Vitalization: Revolving Door Economy or Creative Destruction

Abstract  The growth of the regional economy needs an economic metabolism in which high-productivity firms newly enter the market, while low-productivity firms exit the market, resulting in a shift in labor and other production factors. A“ revolving door” economy is an economy in which firms that enter the market have a short existence period, withdraw and enter the market repeatedly, and new entrants do not contribute to productivity improvement. This means that if new entrants are not sufficiently innovative compared to incumbents, even if the rate of entry into business rises, they will simply be replaced by firms whose productivity level has not changed much, and this will not lead to job creation or improving productivity. A contrasting concept is the replacement of firms by Schumpeter's “creative destruction.” The high level of technology and productivity of new firms entering the market drives inefficient incumbents out of the market. Looking at the statistics, there is a tendency for both large cities to have higher business entry and exit rates, but the difference between the entry and exit rates is greater in metropolitan areas. Although it depends on the regional characteristics, location competitiveness is generally higher in metropolitan areas, and there is a tendency for the turnover rate to be high or the survival period to be short. Before and after regional revitalization, we will examine whether or not there is a departure from the revolving door economy by industry and region, using economic census and TSR (Tokyo Shoko Research) data.

Abstract A 24-year-old Japanese female with anorexia nervosa presented to our hospital for bilateral pneumothorax, and 12-Fr thoracostomy catheters were inserted into the bilateral pleural cavities. On hospital day 9, a thoracoscopic bullectomy was performed. However, air leakage relapsed on both sides on postoperative day 1. The air leakage on the right side was particularly persistent, and we switched the drainage to a Heimlich valve. Both lungs expanded gradually and the chest tube was removed on postoperative day 19. Passive pleural drainage might be an option for prolonged air leakage after a bullectomy in patients with anorexia nervosa.

Abstract Vertical transmission of hepatitis B virus (HBV), especially in Asia, is a key target in the global elimination of HBV. This study assessed the effects of tenofovir disoproxil fumarate (TDF) in pregnant women for mother-to-infant transmission of HBV. A total of 122 pregnant women at our hospital met the inclusion criteria for high HBV DNA viral loads. They were randomly divided into TDF-treatment (n=70) and placebo (n=52) groups. Maternal liver function and serum HBV DNA load were tested before and after treatment. Clinical and laboratory data of infants were assayed at delivery and 7-months post-partum visit and compared between the two groups. There was no difference in clinical characteristics of participants between the two groups. There were no significant differences in liver function markers, including alanine aminotransferase, total bilirubin, blood creatinine, and blood urea nitrogen levels before and after TDF treatment. The serum HBV DNA viral load of the TDF-treated group became significantly lower than those of the control group and their own pre-medication levels. Infants showed no significant difference in body growth, including weight, height, head size, and five-min Apgar score. At 7 months after birth, 94.29% of infants in the TDF group and 86.54% of control-group infants had protective HBsAb levels ≥ 10 mIU/ml (p>0.05). The HBV infection rate of infants in the TDF-treated group was lower than that in the non-treated group. In high-HBV-DNA-load pregnant women, TDF administered from 28 weeks gestational age to delivery was associated with a lower risk of mother-to-infant transmission of HBV.

Abstract The lungs are very complex organs, and the respiratory system performs the dual roles of repairing tissue while protecting against infection from various environmental stimuli. Persistent external irritation disrupts the immune responses of tissues and cells in the respiratory system, ultimately leading to respiratory disease. Neuropeptide Y (NPY) is a 36-amino-acid polypeptide and a neurotransmitter that regulates homeostasis. The NPY receptor is a seven-transmembrane-domain G-protein-coupled receptor with six subtypes (Y1, Y2, Y3, Y4, Y5, and Y6). Of these receptors, Y1, Y2, Y4, and Y5 are functional in humans, and Y1 plays important roles in the immune responses of many organs, including the respiratory system. NPY and the Y1 receptor have critical roles in the pathogenesis of asthma, chronic obstructive pulmonary disease, and idiopathic pulmonary fibrosis. The effects of NPY on the airway immune response and pathogenesis differ among respiratory diseases. This review focuses on the involvement of NPY in the airway immune response and pathogenesis of various respiratory diseases.

Title Alternative Descriptive Study of Itching Experienced by Adults with Atopic Dermatitis

Abstract Background: Atopic Dermatitis (AD) is a chronic inflammatory skin disease characterized by intense itching, significantly impacting patients' psychosocial well-being worldwide. Objective: This study aimed to elucidate the multifaceted and multilayered burden of itch in adult AD patients, exploring strategies to capture the complexity of the disease and its symptoms comprehensively. Methods: The study utilized an array of Patient Reported Outcome (PRO) measures and conducted an exploratory analysis of patients' subjective descriptions of their itch experiences. Results: Twenty-three participants (female: n=16) were enrolled in the study. The Analyses revealed that none of the participants experienced in the same way about physical, emotional, and psychological burden of itch and limitation in social life. Additionally, it was found that PROs capture only partial aspects of itch and do not provide a comprehensive understanding. Discussion: The findings highlight the necessity of employing multiple PROs and conducting in-depth patient interviews to understand the daily life challenges associated with itch. The potential of interdisciplinary research to address the complex burden of itch in AD patients is also emphasized. Conclusion: A comprehensive understanding of the symptom of itch in AD patients requires the use of multiple PROs.
