In Japan, GIS-based evacuation maps are commonly used to support disaster preparedness, but they still have several limitations. In particular, they do not avoid flooded road segments and cannot generate multiple evacuation options at the same time. This study proposes an improved evacuation route method using the free and open-source software QGIS. The method combines flood-depth data with road network processing to remove roads where the predicted water depth is higher than 0.5 m. It also provides several evacuation paths to different shelters at the same time. A case study in Kurashiki City, Okayama Prefecture, demonstrates that about 1.37% of the road network becomes unusable during an inland-flood scenario. Several existing evacuation routes also pass through hazardous areas, but the QGIS-based method avoids these areas in most cases. Since the workflow uses only built-in QGIS functions and does not require programming or plug-ins, it is easy to reproduce and apply in other regions. However, IoT application systems have often been designed and deployed independently without considering the standards of devices, logics, and data communications. In this paper, we present the design and implementation of the IoT server platform called Smart Environmental Monitoring and Analytical in Real-Time (SEMAR) for integrating IoT application systems using standards. SEMAR offers Big Data environments with built-in functions for data aggregations, synchronizations, and classifications with machine learning. Moreover, plug-in functions can be easily implemented. Data from devices for different sensors can be accepted directly and through network connections, which will be used in real-time for user interfaces, text files, and access to other systems through Representational State Transfer Application Programming Interface (REST API) services. For evaluations of SEMAR, we implemented the platform and integrated five IoT application systems, namely, the air-conditioning guidance system, the fingerprint-based indoor localization system, the water quality monitoring system, the environment monitoring system, and the air quality monitoring system. When compared with existing research on IoT platforms, the proposed SEMAR IoT application server platform offers higher flexibility and interoperability with the functions for IoT device managements, data communications, decision making, synchronizations, and filters that can be easily integrated with external programs or IoT applications without changing the codes. An 80‑year‑old woman with one‑year history of left‑eye injection and hemorrhage experienced rapid growth of the ocular surface black mass. At the initial visit, a black, elastic hard, hemorrhage‑prone, thickened mass in the size of 30x40 mm with a presumed wide stalk covered the total area of the lid fissure on the left side. Biopsy of the mass demonstrated anomalous melanin‑containing cells in fibrin and hemorrhage, which were positive for cocktail‑mix antibodies against tyrosinase, melanoma antigen recognized by T cells‑1 and human melanoma black‑45, indicative of malignant melanoma. One month after the initial visit, the patient underwent proton beam therapy at the total dose of 70.4 Gy (relative biological effectiveness) in 32 fractions (~10 min each) in one and a half months. One month after the end of proton beam therapy, 3.5 months from the initial visit, the patient was found by computed tomographic scan to have multiple metastatic lesions in bilateral lung fields. With the evidence of absent BRAF mutation, the patient underwent intravenous administration of pembrolizumab 77.2 mg every three weeks five times in total. Then, three months after proton beam therapy, ocular surface melanoma almost subsided and the clear cornea allowed visualization of the intraocular lens inside the eye. In three weeks, spontaneous corneal perforation was plugged with iris incarceration. The patient died suddenly of unknown cause 7.5 months from the initial visit. The local control of giant conjunctival melanoma was achieved by proton beam therapy, leading to patient's satisfaction and better quality of life. An ADC reflects elements of free diffusion. Diffusion kurtosis imaging (DKI) has attracted attention as a restricted diffusion imaging technique. The ADC subtraction method (ASM) was developed to visualize restricted diffusion with high resolution by using two ADC maps taken with different diffusion times. We conducted the present study to provide a bridge between the reported basic ASM research and clinical research. We developed new imaging software for clinical use and evaluated its performance herein. This software performs the imaging process automatically and continuously at the pixel level, using ImageJ software. The new software uses a macro or a plugin which is compatible with various operating systems via a Java Virtual Machine. We tested the new imaging software’s performance by using a Jurkat cell bio-phantom, and the statistical evaluation of the performance clarified that the ASM values of 99.98% of the pixels in the bio-phantom and physiological saline were calculated accurately (p<0.001). Solvent-resistant microdevices, fabricated by frontal photopolymerization, encapsulate binary polymer (P)/solvent (S1) mixtures by a carrier solvent phase (C) to form plugs with well-defined radii and low polydispersity; the suspension is then brought into contact with a selective extraction solvent (S2) that is miscible with C and S1 but not P, leading to the extraction of S1 from the droplets. The ensuing phase inversion yields polymer capsules with a smooth surface but highly porous internal structure. Depending on the liquid extraction time scale, this stage can be carried out in situ, within the chip, or ex situ, in an external S2 bath. Bimodal polymer plugs are achieved using asymmetrically inverted T junctions. For this demonstration, we form sodium poly(styrenesulfonate) (P) particles using water (S1), hexadecane (C), and methyl ethyl ketone (S2). We measure droplet extraction rates as a function of drop size and polymer concentration and propose a simple scaling model to guide particle formation. We find that the extraction time required to form particles from liquid droplets does not depend on the initial polymer concentration but is rather proportional to the initial droplet size. The resulting particle size follows a linear relationship with the initial droplet size for all polymer concentrations, allowing for the precise control of particle size. The internal particle porous structure exhibits a polymer density gradient ranging from a dense surface skin toward an essentially hollow core. Average particle porosities between 10 and 50% are achieved by varying the initial droplet compositions up to 15 wt % polymer. The spark plug sensor was adapted to a compression-expansion machine, and the CO2 concentration near the spark plug was measured by adding a gas mixture, including CO2 to imitate EGR. Next, the EGR ratio was changed from 10 to 40%, and the CO2 concentration was measured. The effect of the CO2 on the flame propagation was investigated by visualizing the bottom view of the compression-expansion machine. The measurements of CO2 mass concentration are in agreement with those predicted by direct-absorption spectroscopy fundamental theory from the crank angle −60 to −15 deg ATDC. We measured histidine-rich glycoprotein (HRG), a new biomarker of aSAH, in cerebrospinal fluid (CSF) to investigate whether HRG might be an early predictor of CVS. A total of seven controls and 14 aSAH patients (8 males, 6 females aged 53.4±15.4 years) were enrolled, and serial CSF and serum samples were taken. We allocated these samples to three phases (T1-T3) and measured HRG, interleukin (IL)-6, fibrinopeptide A (FpA), and 8-hydroxy-2’-deoxyguanosine (8OHdG) in the CSF, and the HRG in serum. We also examined the release of HRG in rat blood incubated in artificial CSF. In contrast to the other biomarkers examined, the change in the CSF HRG concentration was significantly different between the nonspasm and spasm groups (p<0.01). The rat blood/CSF model revealed a time course similar to that of the human CSF samples in the non-spasm group. HRG thus appears to have the potential to become an early predictor of CVS. The method consists of a plug–plug reaction between lignin peroxidase and its substrate, veratryl alcohol, the separation of the product, veratraldehyde, from the other components including the enzyme and the culture medium, and the determination of the enzyme activity from the peak area of veratraldehyde produced by the plug–plug reaction. This method is more sensitive than conventional spectrophotometry since the background originates from the enzyme and the culture medium can be removed via MEKC separation. Veratraldehyde was separated at −10 kV in a background electrolyte containing 50 mM tartrate buffer (pH 2.5) and 50 mM sodium dodecyl sulfate (SDS) after a plug–plug reaction in the capillary for 5 min. The calibration curve of veratraldehyde was linear up to 4 pmol (500 μM) with a limit to quantification of 0.026 pmol (3.2 μM) (SN = 10). The activity of lignin peroxidase was directly measured from the peak area of veratraldehyde. The term "ectopic varices" is used to describe dilated portosystemic collateral veins in unusual locations other than the gastroesophageal region. We recently experienced a rare case of ectopic varices that developed in the gastroduodenal anastomosis after subtotal gastrectomy. A 70-year-old male with liver cirrhosis due to hepatitis C virus infection was admitted for hematemesis and tarry stool. He had received a subtotal gastrectomy with the Billroth-I method for gastric ulcer at 46 years of age. Although emergency endoscopy revealed esophageal and gastric fundal varices, there were no obvious bleeding points. After removal of the coagula, ectopic varices and a fibrin plug were observed on the gastroduodenal anastomosis. During the observation, blood began to spurt from the fibrin plug. N-butyl-2-cyanoacrylate with lipiodol injection succeeded in hemostasis. Splenic angiography showed gastric varices feeding from a short gastric vein and the posterior gastric vein. The blood flow around the bleeding point, as indicated by lipiodol deposition, had decreased, and no feeding vein was observed. Endoscopic and angiographic findings are shown and the treatment for such lesions is discussed.

The precise identification of the mucopolysaccharides and their distribution within the protein plugs may clarify the pathogenesis of the plugs. Pure pancreatic juice from five patients with chronic pancreatitis was obtained by endoscopic retrograde catheterization of the papilla of Vater. Enzymes for digestion of the plugs included hyaluronidase of the bovine testes and streptomyces hyalurolyticus, chondroitinase ABC and AC, and sialidase (neuraminidase). Our study indicated that: I) Sialic acid is distributed throughout the plugs and may be a major component, followed by a lesser amount of chondroitin sulfate B. 2) Chondroitin sulfate A, C, D and E and chondroitin may be minor components. 3) Hyaluronic acid is negligible in the plugs.

High mobility group box 1 (HMGB1) is released from necrotic cells as well as macrophages and plays proinflammatory roles. In the present study, we examined whether HMGB1 would exhibit angiogenic activity in a matrigel plug assay in mice. HMGB1 in combination with heparin strongly induced angiogenesis, whereas neither HMGB1 nor heparin alone showed such angiogenic activity. The heparin-dependent induction of angiogenesis by HMGB1 was accompanied by increases in the expression of tumor necrosis factor-alpha (TNF-alpha) and vascular endothelial growth factor-A120 (VEGF-A120). It is likely that the dependence of the angiogenic activity of HMGB1 on heparin was due to the efficiency of the diffusion of the HMGB1-heparin complex from matrigel to the surrounding areas. VEGF-A165 possessing a heparin-binding domain showed a pattern of heparin-dependent angiogenic activity similar to that of HMGB1. The presence of heparin also inhibited the degradation of HMGB1 by plasmin in vitro. Taken together, these results suggested that HMGB1 in complex with heparin possesses remarkable angiogenic activity, probably through the induction of TNF-alpha and VEGF-A120.

Background factors such as age and sex of the A/K amputees and the period after amputation were also surveyed. Of the 84 amputees surveyed, 74 (88.1%) were over 40 years old and 40 (47.6%) were over 60 years old. There were 10 women (11.9%) and 74 men (88.1%). The period after amputation was under 25 years in 58 (69.0%) cases. Regarding the type of A/K prostheses, one-third of the prostheses was of the exoskeletal type and two-thirds were of the endoskeletal type. Although the endoskeletal type is becoming more popular recently, elderly A/K amputees tend to use the exoskeletal type. Thirty-one (36.9%) had plug-fit sockets which are preferable for those who follow the Japanese practice of sitting on the floor, especially for elderly amputees. Thirty-seven (44.0%) had a lock-knee, 27 (73.0%) of which were used by amputees over 60 years old. Seventy-three (86.9%) had a single-axis ankle which is generally considered to be the most stable ankle. Thus, the most common combination of prosthetic components for elderly A/K amputees was the plug-fit socket, lock-knee joint and single-axis ankle.

The endoscopic removal of protein plugs from the pancreatic duct resulted in remarkable improvement in symptoms, laboratory findings and ERCP findings. We consider this procedure to be an important new treatment of chronic pancreatitis.

In September, 1979, he had an acute onset of pancreatitis followed by pseudocyst formation and the following combined operations were performed: resection of the pseudocyst, resection of the body and tail of the pancreas along with the spleen, partial resection of the stomach and the transverse colon, and gastroenterostomy according to Billroth I method. He had been followed up at out-patient clinic for chronic pancreatitis and diabetes until January 14,1979 when he was hospitalized for anorexia, loss of weight, pretibial edema, and upper-abdominal pain. The patient's condition was characterized by emaciation (height 152 cm, weight 37 kg), anemia (Hb 7.9g/dl), hypoproteinemia (serum protein 4.2 g/dl) , and hypolipidemia (total cholesterol 73 mg/dl). Abdominal ultrasound, abdominal CT and ERCP revealed stones in the pancreatic duct. Symptoms and objective findings were only partially improved by medical regimens. Subsequently we performed endoscopic cannulation of the pancreatic duct for elimination of pancreatic stones. Successful elimination of large numbers of protein plugs resulted in dramatic improvement of symptoms and objective findings. PPJ was collected from within the main pancreatic duct by endoscopic retrograde catheterization of the papilla. With increasing experience, it has been revealed that various numbers of protein plugs are contained in PPJ obteined from patients with CP or suspected CP and that endoscopic elimination of protein plugs can provide them with dramatic relief from abdominal pain and back pain. The purpose of this report is to evaluate the role of endoscopic elimination of protein plugs in the treatment of CP. Clinico-pathological features comprise intractable abdominal pain, maldigestion secodary to exocrine pancreatic insufficiency and diabetes secondary to endocrine pancreatic insufficiency. Death is not infrequent during follow-up. Pathogenesis and evolution of alcoholic CP were discussed in connection with "Big Duct Theory", "Toxic-Metabolic Theory", and "Protein Plug Theory". Diet and congenital predisposition as well as alcoholic comsumption play important roles in various combinations in the pathogenesis and evolution of CP. Important initial lesins of alcoholic CP are probably : (1) pancreatic ductular Obstruction due to proetin plug formation resulting from increased protein and mucoprotein concentration in pancreatic juice ; (2) damage and sloughing of ductular epithelia caused by protein plugs or by a direct toxic effect of ethanol secreted into pancreatic juice, and subsequent ductular stenosis and peri-ductular fibrosis. Parenchymal destruction with interstitial fibrosis is induced upstream to the ductular lesions. These, structural abnormalities and multiple biochemical abnormalities of pancreatic juice further lead to self-perpetuating evolution. The following results were obtained. 1) No evident correlation between urinary excretion of estrogens and the results of routine liver function tests occured, but ratios of estriol/estrone + estradiol appeared to decrease on aggravation of liver functions. 2) Urinary excretions of estrogens and ratios of estriol/estrone + estradiol were not in distinct correlation with histological classification of liver diseases: acute hepatitis, chronic hepatitis and liver cirrhosis. 3) Urinary excretions of total estrogens and estradiol were distinctly increased to reduce ratios of estriol/estrone + estradiol in patients with deposition of bile pigment in liver cells and bile plugs in bile ducts as notable histological findings. On the other hand, urinary excretions of both estrogens and ratios of estriol/estrone + estradiol appeared to increase in patients with regeneration of liver cells, distortion of lobular architecture, and increase of connective tissue accompanied with its collagenation. 4) At acute icteric stages of most acute hepatitis urinary excretions of total estrogens were increased. On improvement of jaundice, estradiol decreased; estriol increased; and ratios of estriol/estrone + estradiol increased to their normal ranges in all four cases of acute hepatitis. On deterioration of four cases of liver cirrhosis, in two cases of which estrone and estriol in urines increased; and in another two cases decreased, and estradiol increased in three out of four cases. In all four cases of aggrevated liver cirrhosis, increases of total estrogens in urines paralleled with pronounced decreases of ratio of estriol/estrone + estradiol. 5) In patients with vascular spider, significant increases of estrone; estradiol; and total estrogens in urines, accompanied with significant decreases of ratios of estriol/estrone + estradiol, were observed. The total number of inguinal herniorrhaphies for the past 9 years was 376, and the numbers of operative treatments by PHS, MPT and the Bassini procedure were 79, 291 and 6, respectively. There were no significant differences either in the operating time or the hospitalization days. No postoperative complications were observed in the PHS group; however, 8 complications were found among the 291 cases (2.75 %) of the MPT group (hematoma, 3 ; seroma, 2 ; testitis, 1 ; mesh infection, 2). Out of the total 376 cases, the number of initial and recurrence cases was 355 and 21, respectively, and out of the 21 recurrence cases the number of initial surgeries by Bassini and MPT was 14 and 7, respectively. Protein plugs were obtained by endoscopic cannulation of the papilla from four patients with calcified chronic pancreatitis (CP), four with non-calcified CP and four with suspected CP. Macroscopic observation revealed two kinds of plugs: opague, whitish plugs of irregular shape (Type I) and translucent jellylike plugs (Type II). The essential fine structure of an amorphous substance filling a reticular network composed of thin, cottony fibrils about 0.1 μ in diameter was shared by both types of plugs. The reticular network of type I plugs were accompanied by thick, smooth fibrils about 0.5 μ in diameter in addition to the cottony fibrils. In four patients, spores of gram-negative bacilli contributed, as smooth thick fibrils, to the network formation of type I plugs. Protein plug formation was considered to start from small aggregates of a loose interlacing cottony network of fibrils with some filling by an amorphous substance and degenerating epithelial cells, followed by maturation with more compact interlacing of fibrils and denser filling by the amorphous substance. Plugs grew most probably by adherence of smaller aggregates and also by layered depositions of the reticular network and amorphous substance. The incidence of AIP has been increasing worldwide, paralleling the increase in alcohol consumption. AIP manifests itself following a longterm consumption of large amounts of alcohol. There is no known threshold value of alcohol consumption in terms of the risk of developing AIP, although the logarithm of the risk of developing AIP is lineally correlated with the amount of alcohol intake. Why some alcoholics develop pancreatitis whereas others with equal consumption of ethanol are spared remains to be explained. Therefore, two additional factors are considered to play important roles in developing AIP : genetic predisposition and diet. The majority of AIP IS chronic pancreatitis (AICP), although a minority can be acute pancreatitis (AIAP). AIAP shows somewhat higher morbidity and mortality than the common variety of acute pancreatitis. If recovered from an attack, AIAP shows morphological and functional restoration. AICP manifests itself with an acute attack of abdominal pain, insidious onset of abdominal pam, or a pain-free variety. An acute attack in AICP resemble that m AIAP ; often these two can be differentiated only by follow-up studies. AICP shows no morphological and functional restoration, and often shows progressive deterioration. Abdominal pain with elevated serum pancreatic enzymes is a predominant clinical picture m the early stage of AICP, whereas in the late stage symptoms and signs deriving from exocrine insufficiency (maldigestion) and endocrine insufficiency (pancreatic diabetes) begin to dominate the clinical pictures. AICP is in the more advanced stage and shows more complications than nonalcoholic chronic pancreatitis at the time of diagnosis. In addition, AICP shows more rapid progress and higher morbidity and mortality. The incidence of microangiopathy in pancreatic diabetes resemble that in primary diabetes, being higher in patients with a longer history of diabetes, those on insulin treatment and those under poorer control. Main causes of death are development of cancer in the upper respiratory and gastrointestinal tract and diabetic complications (hypoglycemic shock, renal failure, and intractable pneumonia), and acute attack of pancreatitis leads to death less frequently. Partial or complete relief of pain was obtained in 64 patients with alcoholic CP (66%), nine with biliary CP (100%) and 29 with idiopathic CP (88%). Alcoholic CP showed a significantly lower ratio of pain relief than nonalcoholic CP. A long follow-up period (≧6 years revealed greater pain relief. Patients with discontinued or decreased intake of alcohol showed a significantly higher ratio of pain relief than those continuing drinking. These findings indicate that factors contributing to pain relief are etiological category of CP, drinking habit after diagnosis and length of the follow-up period. Deterioration in quality of life was observed in 42 patients with alcoholic CP, one with biliary CP and 13 with idiopathic CP. Main causes for the deterioration in quality of life were pain, diabetes and loss of interest in work. Abstinence is important for patinets with CP to maintain their quality of life. Endoscopic removal of protein plug contributed to pain relief. Surgical treatment was done in 54 patients with CP. Pain relief was observed in 41 operated patients, and deterioration in the quality of life in 17. Abstinence was important for operated patients as well as non-operated patients to obtain pain relief and maintain the quality of life. Clinically overt steatorrhea was observed in two patients with CP, which was improved by a large dose of enzyme replacement therapy. Aggravation of diabetes had been obsreved, and number of patients dependent on insulin therapy increased during the follow-up period. Mongrel dogs were placed on total CPB for 130 minutes without aortic cross clamping (Group Ⅰ). Others were placed on CPB for 120 minutes aortic cross clamping with intermittent administration of cardioplegic solution and core cooling (Group Ⅱ). Before reperfusion, saline, and 1 mg, 3 mg, 10 mg and 20 mg h-SOD per kilogram were administered via the aortic root as a bolus injection (Group Ⅲ,Ⅳ,Ⅴ,Ⅵ,Ⅶ). Reperfusion after hypothermic global ischemia with aortic cross clamping deteriorated cardiac function (cardiac index, left ventricular maximum dp/dt), increased myocardial water content and increased cardiac enzyme release (creatinine kinase MB isozyme, α-hydroxybutyric dehydrogenase). Administration of 3 mg/㎏ h-SOD significantly ameliorated this reperfusion injury, protected myocardial function early after CPB and gave a desirable peak serum h-SOD concentration. en-copyright= factor(TNF)-alphaを加えると心筋細胞で濃度依存性に活性酸素が発生した.さらにヒトの不全心筋においても実際に酸化ストレスの発生が増強しているかを検討した.過酸化脂質の代謝産物で,有害なアルデヒドである4-Hydroxy-2-nonenal(HNE)によって修飾された蛋白質を免疫染色にて調べたところ,拡張型心筋症患者の心筋において正常心機能者に比べ5倍以上増加していた.さらにβ遮断薬(Carvedilol)により治療を行ったところ心機能の改善とともに,HNE修飾蛋白質が40%低下した.以上の一連の研究により,
