Abstract The results of submucosal electrocoagulation (SEC), a new radical operation for prolapsed hemorrhoids, in 403 patients with third- or fourth-degree hemorrhoids are reported. After resecting the anal skin tags that coexisted with prolapsed hemorrhoids, the hemorrhoidal varices could be resected and electrically coagulated through the wound without cutting the anal canal epithelium by using a fine needle-type electric knife. The results of this series indicated that SEC could dramatically reduce the incidence of the postoperative complications that sometimes occur after conventional hemorrhoid-ectomy, such as severe anal pain, massive anal bleeding and anal stenosis. Moreover, SEC could ensure that operated patients make an early return to social activities and have a satisfactory quality of life. Relapse of prolapsed hemorrhoids after SEC was rare.

Abstract Among patients that underwent total knee arthroplasty from June, 1990 to January, 1999, 61 cases (44 patients) that could be followed for more than 10 years were included in this study. The patients were divided into a patellar retention group and a patellar resurfacing group, and were compared with regard to their clinical and radiological outcomes. In patients undergoing primary TKA, a selective patellar resurfacing protocol was used. The indications for patellar retention were a small patella, nearly normal articular cartilage, minimal preoperative patellofemoral pain, poor patellar bone quality, and young patient age. When patellar retention was performed, osteophytes of the patella were removed and marginal electrocauterization was carried out. There were 25 cases (20 patients) in the patellar retention group and 36 cases (29 patients) in the patellar resurfacing group. The mean follow-up period was 140.7 months in the patellar retention group and 149.0 months in the patellar resurfacing group. The selective patellar resurfacing with total knee arthroplasty had a favorable outcome;there were a significant difference noted between the 2 groups in the functional scores, which showed better outcomes in the patellar resurfacing group than in the patellar retention group.

Abstract It is not known whether changing from isoflurane to desflurane during the latter part of anesthesia shows early emergence and recovery in long surgery. We therefore evaluated the effects of changing isoflurane to desflurane on emergence and recovery. Eighty-two patients were randomly assigned to receive isoflurane (Group I) or desflurane (Group D) or to change from isoflurane to desflurane anesthesia (Group X). At the point when there was an hour until the operation would end, isoflurane was replaced with 1 MAC of desflurane in Group X, and isoflurane and desflurane were maintained at 1 MAC in Groups I and D. When the operation ended, we compared the emergence and recovery characteristics among the 3 groups. Compared with Group I, Group X showed faster emergence and recovery. Group X and Group D showed similar emergence and recovery. In conclusion, changing isoflurane to desflurane during the latter part of anesthesia improves emergence and recovery.

Title Alternative BALNEOTHERAPEUTIC EXPERIENCES IN GYNECOLOGY (IV) A STATISTICAL INVESTIGATION SPA VISITORS WITH GYNECOLOGICAL DISEASES

Abstract Investigation on 45 women, taking a cure at Misasa Spa because of various gynecological complaints without advice of doctor, proved that; (1) The vast majority of these women had suffered from their diseases for years, and no medical treatment up to this time could show any favourable effect. (2) Their chief complaints were lower abdominal pain (25%), vaginal discharge (21.9%), lumbago (16%), vaginal bleeding (10.9%), abnormality of menstruation (6.3%), infertility (6.3%), etc.. (3) Pelvic examination in our clinic revealed adnexitis (25%), vaginitis (21.2%), hypoplasia uteri (11.6%), retroflexio uteri (9.6%), infertility (7.7%), etc.. But as the most remarkable fact there were 5 cases (11.6%) of contraindications, (cancer of uterus 2, bleeding myoma of uterus 1, acute phase of adnexitis 2), shown tendency to become worse after thermal baths. (4) In some cases of adnexitis, vaginal discharge, hypoplasia uteri, etc., spa treatment in Misasa proved very effective, and as an "accident thermal" 2 cases of functional bleeding were noted. The author emphasizes that balnetherapy is very useful for various gynecological diseases, but should be supervised and controled by doctor as in Western countries.

Title Alternative A STATISTICAL OBSERVATION ON CHOLECYSTOPATHIA

Abstract 266 consecutive ca~es of cholecystopathic patients in our clinic, mostly ambulant, were statistically investigated. In the majority cholecystopathic patients had a complaint of digestive troubles similar to that of gastritis or peptic ulcer, such as epigastralgia (68%), especially epigastralgia after the meal (30%), hunger pain (9%), anorexia (41%), obstipation (32%), heartburn (30%), nausea (18%), vomitus (15%), etc. Attack of high pyrexia (3%), jaundice (5%), or upper abdominal pain of colic character were less frequently observed. X-ray, duodenal and gastric juice examinations were necessary for differential diagnosis, The most frequent complication was ascariasis (27%) and ankylostomiasis (22%). The swelling of liver was seen in many patients (38%). Tenderness of upper abdomen was often restricted to right epigastrium (62%), but gall bladder was relatively seldom palpable (7%). Urobilinogen reaction in urine was positive in 46%. Gastric hyp- and anacidity prevailed (60%) in cholecystopathia, contrary to 25~30% in peptic ulcer. Concentration of bile in gall bladder was decreased in 78%. Erythrocytes sedimentation rate was generally accelerated. Takata-reaction in serum proved to be highly positive.

Abstract 2-5cc. of 1% Rodium chondroitin-sulfate solution subcutaneouRly or 2cc. of 5% solution intraveneously was administered to 37 cares of painful disorders such as headache, backache or arthralgia. A marked improvement was obtained in 12 cares and a moderate result was seen in 16 cases. No definite change was proved concerning blood pressure in the patients and detoxicating effect in animal experiment.
