This paper traces the historical development of decision-making models in healthcare while exploring the meaning and practical significance of “patient participation” within the shared decision-making (SDM) framework. SDM is a recommended approach to clinical decision-making that emphasizes mutual information sharing and deliberation between physicians and patients. Traditional models often assume that patients can clearly articulate their values, preferences, and treatment goals. However, in actual clinical settings, particularly in cases of serious illness or life-threatening situations, patients frequently face emotional distress and psychological burdens, which can hinder their active participation in decision-making and the expression of their preferences. Based on SDM theory and practice reports, this study argues that SDM should not be viewed merely as a process that promotes patient choice. Even when patients choose not to actively participate and ultimately delegate decisions to healthcare providers or family members, such a choice can represent autonomous decision-making if it arises through meaningful communication and mutual understanding. This perspective calls for a more comprehensive and flexible interpretation of patient participation in SDM practice.

In the first part of this paper, it was confirmed that the term “kaigo” (nursing care) was coined and its meaning defined during discussions on enacting social welfare legislation accompanying societal aging, as the care aspect was being “differentiated” from the “family’s health and welfare functions.” The paper also examined how the term “kaigo gijutsu”(nursing care technique) has been defined and used. In the latter part, based on the author’s own definition of “kaigo gijutsu”(nursing care technology), an attempt was made to analyze examples of technology utilization in nursing care settings, focusing on papers published in specialized welfare and nursing care technology journals. Through this preliminary study, it was shown that the author’s definition of “nursing care technology” clearly distinguishes between the means for care activities—such as welfare equipment—and the care recipients and caregivers who make use of them, and that this definition is useful for grasping the essence of challenges in nursing care settings.

This cross-over controlled trial aimed to evaluate the effectiveness and safety of two types of cold compresses (towels and ice packs) in alleviating itching among patients with atopic dermatitis. The study recruited 19 participants diagnosed with atopic dermatitis and suffering from chronic itching for over 6 months. Each participant received both types of cold compress interventions. Itching sensations were assessed repeatedly using a visual analogue scale before and after the application of the cold compress. The mean and standard deviation of itching scores for the towel intervention were 16.9 ± 19.1 (baseline) and 11.4 ± 16.1 (post-application). For the ice pack intervention, the scores were 13.6 ± 14.7 (baseline) and 6.2 ± 9.8 (post-application). Although there was a reduction in mean itching scores following the application of cold compresses, the differences were not statistically significant for either intervention. Despite the lack of statistical significance, this study suggests that cold compresses, which are user-friendly and inexpensive, may safely reduce subjective itching in patients with atopic dermatitis without causing pain or discomfort. However, further research with a larger sample size is needed to confirm these findings.

In contemporary end-of-life care, it is difficult for patients to make decisions without the influence of society, family, and other factors. In many cases, patients have the capacity to make the decisions; nevertheless, they have difficulty expressing their own will because of the influence of their relationships and environment. Patient concerns about the burden of care and also the social and economic impacts on family members often hinder their use of imagination and decision-making. Therefore, this study has examined how patients with decision-making capacities could achieve autonomy under the influence of their relationships with their surroundings. The method of decision-making support provided by nurses to patients was examined using relational autonomy theory. Relational autonomy theory attempts to reconceptualize autonomy through feminists who criticize individualist theories of autonomy.

In order for companies to innovate through business co-creation, it is necessary to explore a wide range of external knowledge and technologies. However, there is no clear answer as to how much time should be spent for exploration. Under these circumstances, companies must take into account constraints such as the amount of management resources that can be invested, and make decisions about the time to spend for exploration. The purpose of this paper is to clarify the process of how companies that have introduced corporate accelerator program recognize the relationship between the program period and the results of business co-creation, and how they make decisions about the program period. We conducted a case study of several companies that have introduced corporate accelerator program in Japan. In addition, this paper established a hypothesis about decision-making about the time for exploration from case studies.

Background: Atopic Dermatitis (AD) is a chronic inflammatory skin disease characterized by intense itching, significantly impacting patients' psychosocial well-being worldwide. Objective: This study aimed to elucidate the multifaceted and multilayered burden of itch in adult AD patients, exploring strategies to capture the complexity of the disease and its symptoms comprehensively. Methods: The study utilized an array of Patient Reported Outcome (PRO) measures and conducted an exploratory analysis of patients' subjective descriptions of their itch experiences. Results: Twenty-three participants (female: n=16) were enrolled in the study. The Analyses revealed that none of the participants experienced in the same way about physical, emotional, and psychological burden of itch and limitation in social life. Additionally, it was found that PROs capture only partial aspects of itch and do not provide a comprehensive understanding. Discussion: The findings highlight the necessity of employing multiple PROs and conducting in-depth patient interviews to understand the daily life challenges associated with itch. The potential of interdisciplinary research to address the complex burden of itch in AD patients is also emphasized. Conclusion: A comprehensive understanding of the symptom of itch in AD patients requires the use of multiple PROs.

This study is based on a historical analysis of the technological development and introduction of artificial intelligence for medical use in China, based on the progress of application of artificial intelligence technology development to the medical field of traditional Chinese medicine and the experience of many experts in clinical practice. Through the application history of Traditional Chinese Medicine（TCM）expert systems technology, we will review its historical characteristics and the necessity of TCM expert system technology. By examining the application cases of TCM expert systems, we can find that Traditional Chinese Medicine（TCM）expert systems play an important role as an auxiliary means of diagnosis and treatment for doctors. This paper finds that the development of Traditional Chinese Medicine（TCM）expert systems is directly related to the change of Chinese government's science and technology policy through the investigation of the application history of Traditional Chinese Medicine（TCM）expert systems. At the same time, because the science and technology policy directly related to the traditional Chinese Medicine（TCM）expert systems appeared late, so the implementation technology of the traditional Chinese Medicine（TCM）expert systems is still insufficient. The technical level adopted in the implementation of the existing traditional Chinese Medicine（TCM）expert systems is still in a relatively low state. Therefore, whether in hardware or software, we should develop a new integrated traditional Chinese Medicine（TCM）expert systems based on the treatment theory of many Traditional Chinese Medicine（TCM）expert systems, so as to meet the needs of modern Traditional Chinese Medicine（TCM）expert systems diagnosis, people's need for healthy life, and medical care. Finally, when studying the historical issues of Traditional Chinese Medicine （TCM）expert systems, the Chinese government's science and technology policy changes are also an integral part.

In contemporary Japan, the elderly often say that they do not wish to be a burden on anyone, and they make important decisions with this motivation (here called feelings of "Meiwaku/Burden"). This paper seeks to categorize feelings of "Meiwaku/Burden" according to its relationship to the type of motivation they express. For this purpose, we focused on feelings of "Meiwaku/Burden" of elderly people in France. As there is no easily corresponding phrase in French for our word "Meiwaku/Burden", the French use several different words to express this sense "Meiwaku/Burden". Here we draw examples from Une mort très douce by Simone de Beauvoir, as well as her La Ceremonie des adieux.<br>
The analysis of examples given in these books indicates that "Meiwaku/Burden" of the elderly is motivated by three factors: relationships (déranger), emotions (ennuyer), and body/physical (gêner). In this paper, according to these three motivations we categorize feelings of "Meiwaku/Burden" three types: the feelings of "Meiwaku/Burden" about relationships, the feelings of "Meiwaku/Burden" about emotions, and the feelings of "Meiwaku/Burden" about bodily/physical things.

This study aimed to clarify the meaning of feelings of “Meiwaku/Burden” in Japan, which is expressed as “I do not want to bother others” or “I do not want to be a burden.” This is examined specifically in the context of medical care for the terminally and chronically ill. The first section of this paper reviews the core meaning of autonomy and the situations in which feelings of “Meiwaku/Burden” is focused in Japan. Section 2 examines the meaning of “Meiwaku” and “burden,” which is understood in regards to “autonomy,” particularly in the context of patients’ terminal suffering and spiritual pain in palliative care in Japan. Section 3 verifies that “Meiwaku” and “burden” in Japan are sometimes understood as “self-perceived burden (SPB).” Section 4 examines the meaning of “burden” in the discussion of SPB. Section 5 attempts to clarify the meaning of feelings of “Meiwaku/Burden” in medical care, specifically in the context of “autonomy.”

This paper examines elderly people’s Feelings of "Meiwaku / Burden" from the perspective of the act of gift-giving. In Japanese, the word "Morau" is used not only in receiving a concrete object, but also in having something done by someone. Both acts lead to the human relationship and the reciprocity. Both cases can cause a burden on receivers and others. We use then “Sumimasen”. This is not as the same as English words “Thank You”. But this usage of Japanese can be explained from a Gift-giving-theoretic point of view. Because, in terms of the outcome, they are the same thing from this theory. Therefore, we will examine the meaning of the word "receive" based on discussions by Kunio Yanagida, Michizo Toida, Ruth Benedict, Marcel Mauss, and others. We hope to clarify what is elderly people’s Feelings of "Meiwaku / Burden" by analyzing the meaning of human relationships brought about by the act of gift-giving in general.

This special feature is based on the symposium ,"Feelings of "Meiwaku/Burden" in Modern Japan’s Super-aging Society,” which was held on Monday, September 19, 2022. This symposium was sponsored by JSPS KAKENHI Grant Number JP20A00007. This project focuses on the fact that when people think about aging, end-of-life care, and death in modern Japan, many feel that “they do not want to be a burden to others.” The project is divided into three research groups: historic studies, studies of contemporary Japan, and field research. The symposium reported the results of the contemporary Japan research group, which is halfway through its research. The symposium consisted of two sessions, and this special feature is based on the three participants in the session titled “Feelings of"Meiwaku/Burden" : A Comparison with Other Countries.” Ryozo Suzuki’s study focuses on “the giving of gifts as being mutual actions of giving and receiving” to understand the elderly’s feelings of meiwaku and analyzes the significance and complex structures of the feeling of “being a burden to others” at the end of one’s life. Haruka Hikasa’s study considers the feelings of meiwaku in relation to the subject’s autonomy in the context of medical care provided to people with terminal or chronic illnesses. Natsumi Tanaka’s study examines the feelings of meiwaku among the elderly in France and is based on the work of Simone de Beauvoir. She determines the motivations behind the elderly’s feelings of meiwaku and categorizes them. The abovementioned studies present important research results and facilitate future comparative research on the feelings of meiwaku between Japan and other countries.

The Oki Islands, located about 60 km off the coast of the Sea of Japan, north of Shimane Prefecture, constitute an administrative division called Oki District. One of the four manned islands, Nakanoshima, which has an area of 33.43 ㎢ and a circumference of 89.1 km, is called Ama Town as a basic municipality, and it is said that about 7,000 people lived around 1950 at its peak time. The number is, however, currently less than 2,300. The population by over 60 reaches 47.73% and the islanders call this situation a "microcosm of Japan." The climate is relatively warm, and it is a semi-agricultural and semi-fishing island. As is often the case, a traditional local food became the detonator of the island revitalization. The recent popular souvenir "Sazae Curry" was once a home-cooked dish for the islanders using turban shells and delivers the exquisiteness particular to the land and nostalgia of a sort. This is, however, memorabilia of the old days when the "Sazae" was a substitute meat for unwealthy fishermen. Today, on the contrary, the island's agriculture, forestry and fisheries has become known to deliver local specialities such as Oki beef of range cattle, farmed rock oysters from the natural coves, and squids that utilize the latest quick-freezing technology, etc. This economic restructuring is not the only achievement that the administrative reform of Ama Town has demonstrated, which also covers local finance and public education. As a business case, this paper provides a clearer picture of the process of the reform that was initiated in 2000s by the former town mayor, Michio Yamauchi.

This study aims to improve the quality of home nursing care for the elderly. The report presents web-based information about devices and services for remote monitoring and health management using information and communication technology and about how they can improve home care nursing station efficiency in Japan. The search terms "elderly people," "watch over," and "service," plus "sensor" were used to narrow the search. After repeating the search using a chained search for materials and links extracted from those search results, services for home care stations particularly addressing the remote monitoring of elderly people were selected and extracted. Early detection of abnormalities for support by home health nurse responsibilities for observing the general condition of the patient was chosen as the main evaluation index of usefulness. The contractor and service cost were secondary evaluation axes. Results show 9 information communication devices and services for real-time monitoring of the elderly persons’ vital signs in real time for 24 hr, 2 of which are highly useful for early detection of abnormalities and subsequent response. Results suggest that other services might be more useful, depending on how they are used, and that general-purpose services with minimum functions and low cost of introduction and maintenance can provide more opportunities for utilization. Providing these services on a BtoBtoC model and responding to patients’ medical needs can supplement home care nursing station work, increase human resource and time efficiencies, and give patients "the feeling that a nurse is always at their bedside,” even when far away.

In the work process of nurses with high risk, it is very urgent to take countermeasures against the “lack of confirmation” of the main factors. Therefore, the purpose of this study is to clarify the details of the factors leading to the lack of confirmation, grasp the number of near misses, and to explore what kind of factors the individual characteristics of the nurse who acts as a stopper. We recruited 500 nurses on the Web and asked about the frequency of near misses due to lack of confirmation, the awareness of the causes of lack of confirmation, self-evaluation of stoppers, and the reasons why near misses did not lead to accidents. As a result, three factors were extracted regarding the recognition of the factors of lack of confirmation: "lack of consciousness", "inability to concentrate", and "lack of knowledge / experience". Three factors were extracted for self-evaluation of stopper characteristics: "awareness," "verification," and "cautiousness". In order to reduce the lack of confirmation, it is necessary to secure and allocate appropriate personnel and create an environment where treatment can be concentrated. Nurses should also raise knowledge and the awareness of the importance of confirmation. In addition, it is necessary to improve the clinical judgment of new nurses in order to prevent near misses. And it is thought that new nurses will
become more confident by accumulating experience and affirming their abilities from their superiors, seniors and colleagues.

The Shimanami Kaido Expressway was visioned as a national project in the early 1970s after two tragic maritime accidents in the Setouchi Inner Sea. Since the express way connecting several islands between Hiroshima and Ehime prefecture was placed in use in 1999, it has always been a valuable means of transportation for islanders and commuters to date. In addition, with the proactive efforts made by local governments, such as Imabari City, Onomichi City, let alone with the central and prefectural governments, it has come to be acknowledged to be a sanctuary for sport cyclists. Eventually, it attracts attentions from all over Japan as a successful benchmark for both businesspeople as well as policy makers who are interested in destination management practices as part of promotion policies of reginal economies. This eventual success, however, had not been envisioned as such for a long time since the commencement of its public usage. It would rather seem that the plan had taken on its shape after a long period of many twists and turns. Furthermore, while local governments have been closely cooperating with different types of stakeholders including the actual islanders to promote the development plans, their ways of thinking are still far from unanimous. The purpose of this article is to provide business and policy students with materials that draw lessons for viewing lengthy socioeconomic processes like this case which eventually converted a nation-level, large-scale infrastructure development plan into an area-specific destination management project.

When a global pandemic of infectious diseases occurs, demand for medical supplies such as masks, disinfectants, and personal protective equipment will exceed the production capacity in normal times, consequently leading to serious shortages. Stockpiling is necessary to prepare for such situations. However, because pandemics are infrequent and might occur at any time, stockpile freshness is an important concern. As described in this paper, a “rolling stockpile” of medical supplies is proposed to prevent the medical supply stockpile from becoming too old. A formulation of a model and simulations based on it is presented. Results demonstrate that the method is applicable to the design of warehouses for rolling stockpiles, monitoring stockpile depletion during emergencies, and monitoring the process of recovery from emergencies.
