Abstract ステロイド依存性重症難治性喘息56例について免疫アレルギー学的検討を行った。その結果41才以降発症のLate Onset Asthmaが約半数を占め,血清IgE値が比較的低値であり,ハウスダストに対しての皮内反応,特異的IgE抗体価,ヒスタミン遊離はいずれも全般的に低く,カンジダに対しての皮内反応,特異的IgG抗体価はむしろ比較的高く,肺機能では末梢気道閉塞の傾向が窺われ,臨床分類では,気管支攣縮+過分泌型,および細気管支閉塞型が多数を占める等の特徴を有することが明らかにされた。
Abstract Alternative Clinical studies on steroid-dependent intractable asthma were carried out in 56 patients relating to immuno-allergic aspects. 1. In most half of the patients with steroid-dependent intractable asthma, age at onset of the disease was 41 years old or over (so-called "Late Onset Asthma"). 2. The frequency of positive skin reaction for house dust was relatively low. whereas that for Candida was relatively high. 3. The serum IgE concentrations of these patients were in general low. Many patients showed low serum IgE concentrations of less than 300IU/ml. The concentrations of specific IgE antibodies for house dust and Candida albicans were also low. 4. The concentrations of specific IgG antibodies for Candida albicans were high compared to those for house dust. 5. Basophil histamine release induced by house dust and Candida albicans was not so remarkable in majoriy of these patients, while anti-IgE-induced histamine release from basophils ranged widely. 6. In these patients, small airway obstruction was suggested in ventilatory function test. 7. In clinical classificatin of asthma, type Ⅰa; bronchospasm type was a few, while type Ⅰb; hypersecretion and type Ⅱ; bronchiolor obstruction type were dominant in the cases studied here.

Abstract Histamine release from basophils induced by anti-IgE was studied in 8 patients with chronic bronchitis and 50 patients with bronchial asthma by analyzing doseresponse curves. As the result, there were no significant differences in maximum percent histamine release from basophils among three groups of healthy subjects (24.7± 14.2%), patients with chronic bronchitis (27.7±22.1%) and those with bronchial asthma (28.4±17.0%). In the patients with bronchial asthma, the maximum percent histamine release was higher in accordance with higher serum IgE levels, and low maximum percent release was observed in patients with intrinsic asthma (14.1±7.2%). Study of dose-response curves of anti-IgE-induced histamine release showed that a negative slope from E(2) to E(1) was observed in both healthy subjects and patients with chronic bronchitis. The majority of asthmatics with serum IgE levels of 501IU/ml or over showed a positive slope from E(2) to E(1).
Abstract Alternative 気管支喘息50例,慢性気管支炎8例を対象に,抗ヒトIgE添加時の好塩基球からのヒスタミン遊離を全血法により行ない,その臨床的評価について検討を加えた。抗ヒトIgE海底時のMax % histamine releaseの平均は,健康人24.7±14.2%,慢性気管支炎27.7±22.1%,気管支喘息28.4±17.0%であり,3者間に有意の差はみられなかった。すなわち,抗ヒトIgE添加により健康人や慢性気管支炎患者の好塩基球からも有意のヒスタミン遊離が見られた。気管支喘息のなかでは,内因性喘息症例においてヒスタミン遊離(14.1±7.2%)の低い傾向が見られた。Dose-response curveの検討では,健康人,慢性気管支炎症例では全例E(2)からE(1)へかけてのnegative slopeを示した。気管支喘息症例では,血清IgE値500IU/ml以下の症例ではnegative slopeを示す症例が多く,一方501IU/ml以上の症例ではpositive slopeを示す症例がより多く見られた。

Title Alternative Effect of spa-drink therapy on exocrine pancreatic function

Abstract 膵外分泌機能におよぼす飲泉(三朝温泉水)の効果を飲泉群および非飲泉群を対象として検討した。膵外分泌機能検査法としては,糞便中キモトリプシン活性比色測定法(モノテストカイモトリプシン)を用いた。その結果,糞便中キモトリプシン活性は,飲泉開始2週後に40 % の患者で上昇し,4週後は2週後と変わらなかった。以上より,2週間の飲泉は膵外分泌機能を改善させる効果があることがわかった。
Abstract Alternative The effect of spa-drink therapy (Misasa hot spring) on exocrine pancreatic function was studied on 18 patients after their physical and psychological conditions were stabilized about two weeks following hospitalization. Patients were randomly divided into two groups : eight patients into a control group (continuance of spa-bathing) and 10 patients into a spa-drink therapy group (continuance of spa-bathing plus commencement of spa-drink therapy). Spa-drink therapy refers to drinking 200mℓ of warm spa-water (42℃) two times a day : between meals in the morning and in the afternoon. Exocrine pancreatic function was evaluated by fecal chymotrypsin activity, using a colorimetric method (Boehringer-Mannheim). Following results were obtained : 1) With 2-week's spa-drink therapy, fecal chymotrypsin activity was significantly enhanced in 40 % of the patients, while none of the controls showed an increase in fecal chymotrypsin activity. 2) Fecal chymotrypsin activity at the time of 4 weeks was similar to that obtained at the time of 2 weeks in both groups. In conclusion, spa-drink therapy for 2 weeks was effective for the enhancement of exocrine pancreatic function.

Abstract 抗アレルギー剤(脱顆粒抑制剤)の1つであるtranilastの肥満細胞の遊離機序に対する抑制作用について検討を加えた。1.抗原刺激時の肥満細胞の(45)Ca uptakeおよびヒスタミン遊離に対して,tranilastは有意の抑制作用を示したが,comp.48/80刺激時にはtranilastの(45)Ca uptake,ヒスタミン遊離に対する抑制作用はほとんどみられなかった。2.phosphatidylserine添加時には,(45)Ca uptakeに対するtranilastの抑制作用は減弱傾向を示し,この傾向はヒスタミン遊離に対する作用に比べより高度であった。3.抗原および抗ヒトIgEによる好塩基球からのヒスタミン遊離に対して,tranilastは明らかな抑制作用を示さなかった。
Abstract Alternative Tlanilast is clinically used for bronchial asthma as one of the prophylactic agents for asthma attacks. In this study, anti-allergic actions of tranilast were examined in (45)Ca uptake of and histamine release from target cells of IgE after stimulation with antigen, anti-IgE and camp. 48/80. 1. (45)Ca uptake of and histamine release from rat peritoneal mast cells stimulated by antigen were significantly inhibited by preincubation of the cells with tranilast. Inhibition by tranilast of increased (45)Ca uptake of mast cells by addition of phosphatidylserine were less, although inhibition of histamine release was not so affected by addition of phosphatidylserine. No significant inhibition by tranilast of (45)Ca uptake and histamine release was not observed when the cells were stimulated by camp. 48/80. 2. Tranilast showed any inhibitory effects on basophil histamine release induced by antigen and anti-IgE.

Abstract 近年,各種膵疾患に対する検査法の発達には目を見張るものがあるが,実際の運用にあたっては各検査法の適応,限界,組合わせおよび実施する順序等に迷うことが少なくない。本研究では,最近我々が経験した慢性膵炎Ⅰ群116例の診断過程を振り返ることにより,慢性膵炎の診断における理想的な検査法の組合わせとそれぞれの役割を検討し,同時に診断名のみならず病態と病期が容易に理解できる表現方法を提案した。画像診断法,膵外分泌機能検査法(EX),膵内分泌機能検査法(EN)を比較検討し,以下の結果を得た。①画像診断法は必須である。②内視鏡的逆行性膵胆管造影(ERCP)を軸とした腹部超音波検査(US)あるいは腹部CTスキャン(CT)の組合わせは欠くことができない。③EXもまた必須の検査法である。④ENは慢性膵炎の診断に必要不可欠ではないが,治療方針を決めるうえで重要である。⑤純粋膵液(PPJ)の生化学的検査と組織化学的検査は膵炎の診断と病態生理の解明に重要である。⑥PPJ中の細胞診は慢性膵炎に合併した膵癌の検索に有用である。⑦以下に示すような診断の記載方法を提案した。"Calcified chronic pancreatitis, alcoholic, diffuse, ERCP-3, US-2, CT-3, EX-2, EN-1, Lf(+)"。
Abstract Alternative Endoscopic retrograde cholangiopancreatography (ERCP), ultrasonography (US), computed tomography (CT), exocrine pancreatic function test (EX), endocrine pancreatic function test (EN), and analysis of pure pancreatic juice (PPJ) are currently available for the investigation of pancreatic diseases. The purpose of the present study is to evaluate the roles and preferable combinations of these tests in the diagnosis of chronic pancreatitis and then to propose a way to describe the results of the tests along with diagnosis for a better understanding of the disease process. Comparative studies of imaging, EX and EN led to the following conclusions. 1) imaging procedures (ERCP, US and CT) are mandatory, partly because they were frequently the test that showed diagnostic changes and partly because they often gave key information for operative treatment. Among the imaging procedures, ERCP combined with US and/or CT is mandatory because these combinations alone can reveal detailes of structural changes attendant on chronic pancreatitis. 2) EX is also mandatory, because it was occasionally the only test that showed diagnostic changes and partly because it often gave important information for medical treatment. 3) EN is also important, not because it was indispensable for detecting chronic pancreatitis, but because it often gave important information for medical treatment. For btter understanding of the disease process as well as diagnosis, we propose, as suggested by Seligson, that the test used for the establishment of the diagnosis and the degree as well as extent of abnormalities. An example is "Calcified chronic pancreatitis, alcoholic, diffuse, ERCP-3, US-2, CT-3, EX-2, EN-1, Lf(+)". This attitude will lead to a more rational approach to both diagnosis and treatment.

Title Alternative Effect of aging on pancreatic exocrine function -Aging and fecal chymotrypsin activity-

Abstract 膵外分泌機能に及ぼす加齢の影響を検討する目的で,健常人43名,非膵疾患患者41名,老人ホーム入居者39名を対象として比色法により糞便中キモトリプシン活性(FCA)を測定し,以下の結論を得た。1)FCAは逐齢的に低下し,60歳以上は60歳未満より有意の低値を示した。しかし,分布図のretrospectiveな検討からは,膵外分泌機能の低下は,実際には65~70歳あたりから明らかになることが示唆された。2)FCAの正常下限値を14.5U/gに定めたが,高齢者の診療にあたってはこの正常値のみを根拠にして疾病の有無を判定すべきではない。3)老人ホーム入居者のFCAは特に低値を取ったがその主要因は高齢者が多いためと考えられた。
Abstract Alternative To examine the effect of aging on the pancreatic exocrine function, fecal chymotrypsin activity (FCA) was measured by the photometric method in 43 healthy controls, 39 controls in an old-age home and 41 patients with non-pancreatic diseases. Following conclusions were obtained. 1) Pancreatic exocrine function showed a significant decrease with aging as indicated by the present prospective study : ① a significantly lower FCA in the B group of 22 healthy controls (≧ 60 years of age) than the A group of 21 healthy controls (< 60 years of ago) ; ② significant correlationship between aging and FCA in the 43 healthy controls and also in the 41 patients with non-pancreatic diseases. 2) FCA in controls in an old-age home was especially low. Presumably it was due to their far-advanced aging. 3) The calculated lower limit of normal in FCA was 14.5U/g. However, the diagnosis of pancreatic diseases should not be coined solely on the basis of decreased FCA value.

Abstract The release mechanism of chemical mediators from basophils and mast cells was discussed when these cells were stimulated by different antigens and anti-IgE. 1. Ca(2+) influx into mast cells increased after stimulation by antigen. The increased Ca(2+) uptake by mast cells was inhibited by antiallergic agents, disodium cromoglycate (DSCG) and tranilast, and calcium antagonists, nifedipine and nicardipine. 2. The dose-response curve of histamine release by antigen was different from that by anti-IgE. The maximum release of histamine by house dust was, however, similar to that by anti-IgE. House dust- and anti-IgE-induced release of histamine increased with higher- serum IgE levels. On the contrary, the release of histamine by Candida albicans was not releated to serum IgE levels, and the release by C. albicans did not parallel therelease by anti-IgE.
Abstract Alternative ハウスダスト,カンジダなどの抗原および抗ヒトIgE刺激時の,抗塩基球および肥満細胞からの化学伝達物質遊離機序について,若干の検討を行った。1.肥満細胞へのCa(2+)の流入は,抗原刺激後に増加する傾向を示した。この肥満細胞のCa(2+)取り込みの増加は,抗アレルギー剤であるクロモリン(DSCG)やトラニラストやCa(2+)拮抗剤であるニフェジピンやニカルジピンによって抑制された。2.抗原(ハウスダスト)によるヒスタミン遊離の濃度依存性カーブは,抗ヒトIgEによるカーブとは異なっていた。しかし,ハウスダストが抗原である症例では,抗原による最高ヒスタミン遊離値は,抗ヒトIgEによる遊離値とある程度の相関を示した。そして,抗原(ハウスダスト)および抗ヒトIgEによる好塩基球からのヒスタミン遊離は,血清IgE値が上昇するにつれて,増加する傾向が見られた。一方,カンジダによるヒスタミン遊離は,血清IgE値との相関は無く,また,抗ヒトIgEによるヒスタミン遊離との間にも関連は見られなかった。

Abstract Morphological changes of blood basophils activated by antigen and anti-IgE were discussed in relation to release mechanisms of the cells. 1. Basophils activated by antigen and anti-IgE show two different types : A form (pear-shaped) and B from (swollen type), on smear preparartions. 2. Pear-shaped basophils(reactive basophils), which appear after stimulation by antigen and anti-IgE, can be observed by the staining solution for the direct count of basophils and eosinophils. 3. The incidence of reactive basophils increases after addition of antigen. Thus, observation of reactive basophils can be clinically applied for detecting allergen (DCRB ; direct connt of reactive basophils). 4. Observation of morphological basophil response by phase- contrast microscopic motion pictures demonstrates that basophils activated by antigen show two different reponses, increase in motility (oriented movement) and degranulation.
Abstract Alternative 抗原および抗ヒトIgE刺激時の好塩基球の形態的変化について,細胞の分泌機序(ヒスタミン遊離)との関連のもとに若干の検討を加えた。1.抗原や抗ヒトIgE刺激時には,塗抹標本上では好塩基球の2つの形態的変化,A-form (洋梨型)とB-form(膨化型)を観察することができる。2.抗原や抗ヒトIgE刺激時に出現する洋梨型好塩基球(反応好塩基球)は,好塩基球,好酸球同時直接 算定用染色希釈液で染色することにより,容易に観察することができる。3.抗原添加後に反応好塩基球の出現頻度は増加する。したがって,反応好塩基球の出現頻度を観察することにより,原因抗原を明らかにすることができる。4.位相差顕微鏡映画による,免疫下の好塩基球の形態的変化の観察からは,好塩基球は2つの反応を取ることがわかる。その1つは,運動亢進,すなわち oriented movementを示すことであり,他の1つは,膨化して脱顆粒現象をひき起こすことである。

Abstract Blood basophils in subjects with bronchial asthma migrate from blood stream into local allergic reaction sites after inhalation of antigen into airways. The phenomena can be observed by changes in number of basophils in the peripheral blood. The peripheral basophil count is at a normal level in non-attack stage of asthmatics as in healthy subjects. The basophil count increases in pre-attack stages, and decreases during attack stages. These findings suggest that number of basophils changes in close relation to asthma cycle, and that by observing number of basophils in the peripheral blood, it is possible to detect near future attacks. The decrease in number of basophils and histamine release by stimulation with antigen and anti-IgE can be observed in vitro in whole blood and basophils separated by density gradient centrifugation and by counterflow centrifugation elutriation. The decreased number of basophils in the peripheral blood represents migration of the cells into local allergic reaction sites.
Abstract Alternative 気管支喘息では,気道への抗原吸入後,好塩基球は末梢血よりアレルギー反応局所へと遊走する。この現象は,末梢血中の好塩基球数の変動により観察することができる。気管支喘息患者の好塩基球数は,非発作時には健常人とほぼ同じで正常範囲内にあるが,発作前段階で増加し,発作出現とともに減少する傾向を示す。これらの所見は,好塩基球が喘息発作と密接な関連を持ちながら変動すること,そして,これらの変動を観察することにより,近い将来の発作を予知することが可能であることを示唆している。抗原あるいは抗ヒトIgE刺激による好塩基球数の減少およびヒスタミン遊離は,全血法あるいは濃度勾配法やcounterflow centrifugation elutriation法により分離された好塩基球を用いてin vitroで観察することができる。末梢血中の好塩基球数の減少は,好塩基球がアレルギー反応局所へと遊走したことを示している。
