| 抄録 | アレルギー反応の4型, 1型 (lgE依存型反応), 1型 (細胞障害型反応), III型 (免疫複合体型反応), IV型 (細胞免疫型反応) を眼科疾患に当てはめると, I型はアレルギー性結膜疾患, II型は眼類天庖瘡やモーレン (Mooren) 角膜潰瘍などの自己免疫疾患, III型はぶどう球菌アレルギーによる周辺部角膜浸潤, IV型は角膜や結膜ブリクテン, 接触性皮膚炎による眼瞼炎, となる. その他, アレルギー関連疾患として, アトピー性皮膚炎がある. アトピー性皮膚炎は, 小児期の疾患であり, 通常, 皮膚は乾燥している. しかし日本では, 思春期から青年期, さらには成人になってから発症する型がある. この型は, 顔面におこることが多く, しかも皮膚は乾燥しておらず, 湿潤している. アトピー性白内障は, 顔面, 特に眼瞼に皮膚炎がある場合に見られる. 問診すると, 特に就寝時に眼瞼をたたく癖がある. アトピー性網膜剥離は, 通常, 白内障がある眼に起こっている. 硝子体手術を行うと, 硝子体基底部の綿状のきたない混濁がみられ, その近辺に鋸状縁断裂, 毛様体無色素上皮剥離や上皮裂孔がある. 一方, アトピー性白内障がある眼では, 前房水中のフレア値 (蛋白濃度) が上昇している. 以上の事実から, アトピー性皮膚炎の炎症が前眼部に波及し, それに眼瞼をたたくという外傷が加わって白内障や網膜剥離がおこると考えられる. |
